DEFINICION Se define como hematuria la presencia de sangre durante la micción (macroscópica). Se dice que hay hematuria microscópica cuando se detectan dos o más eritrocitos por campo en por lo menos dos exámenes del sedimento urinario. Es un signo de alerta tanto para el paciente como para el médico, ya que es indicio de enfermedad que puede ir desde una simple cistitis hasta una neoplasia maligna. EVALUACION DIAGNOSTICA Y ETIOLOGICA Cuando la hematuria se asocia con dolor lumbar, disuria, urgencia urinaria y malestar suprapúbico el origen del sangrado debe investigarse en el tracto urinario inferior. Cuando la hematuria se detecta al inicio o al final de la micción, la uretra y vejiga pueden estár comprometidas. La hematuria asociada con dolor renal tipo cólico se debe, con mucha probabilidad, a nefrolitiasis. En constraste, la hematuria microscópica secundaria a enfermedad renal difusa es habitualmente indolora y sólo identificable con el examen microscópico del sedimento urinario. La etiología en este caso es múltiple (Tabla No.1). Es necesario interpretar correctamente las hematurias bajo un contexto clínico:
  • En el curso de una patología urológica ya conocida, la aparición de hematuria puede ser el indicio de una recidiva (tumor de la vejiga, cálculos)

    The Cleveland Clinic Foundation Blood in the urine can originate anywhere along the urinary tract and both gross and microscopic hematuria may represent serious underlying disease. Gross hematuria is often alarming and will prompt the patient to seek medical attention. Likewise, the office practitioner may be faced with the incidental finding of asymptomatic microscopic hematuria. Patient characteristics and the clinical presentation will help guide the clinician in proper evaluation and diagnosis.

    Asymptomatic Microscopic Hematuria in Adults: Summary of the AUA Best Practice Policy Recommendations
    DEFINITION Hematuria is defined as the abnormal presence of red blood cells (RBCs) in the urine and is commonly divided into gross and microscopic hematuria. Gross, or visible hematuria can result from as little as 1 cc of blood in a liter of urine, and therefore, the color does not reflect the degree of blood loss.     Hematuria (Blood in urine) Hematuria is defined as blood in the urine. It can be either microscopic (can only be seen through the microscope), or gross (visible in the urine). There may be blood in the urine with almost any kidney or bladder problem. It can be caused by urinary tract infections, urinary stones, injury, cancer in the urinary tract, structural abnormalities, or diseases of the kidney. Hematuria may be an early warning sign for serious medical conditions. Signs and symptoms Microscopic hematuria cannot be seen by the naked eye; however, gross hematuria can make the urine look either "tea-colored," pink, or bright red. Hematuria may or may not be painful, depending on its cause. Other symptoms that could be associated with hematuria are abdominal pain, flank pain, burning with urination, or the frequent urge to urinate. Diagnosis Blood in the urine requires evaluation to identify the cause. A complete evaluation may include renal imaging (x-rays), cystoscopy (looking in the bladder) and urine cytology (laboratory analysis of the urine specimen). T reatment Treatment depends upon the cause of the hematuria. If the blood in the urine is caused by injury to the urinary tract, surgery may be necessary, or if cancer is the cause, treatment may include surgery, chemotherapy, radiation, or a combination of these. If a urinary tract infection is causing hematuria, antibiotics are usually prescribed. If the hematuria is caused by urinary stones, please see the section titled "Stones" on the Patient Information page.

    What is hematuria?
    Hematuria is the presence of red blood cells (RBCs) in the urine. Often, the urine appears normal to the naked eye, but examination under a microscope shows a high number of RBCs. In gross hematuria, the urine is red or the color of cola, which can be seen with the naked eye. What causes hematuria?
    Most of the causes are not serious, but, because hematuria may be the result of a tumor, or other serious problem, a physician should be consulted. Many people have hematuria without having any other related problems. To determine the cause of hematuria, or to rule out certain causes, a series of tests may be ordered. How is hematuria diagnosed?
    In addition to a complete medical history and physical examination, diagnostic procedures for hematuria may include the following:
    • urinalysis - laboratory examination of urine for various cells and chemicals, such as RBCs, white blood cells, infection, or excessive protein. blood tests - laboratory examination of blood for high levels of waste products.

[J Am Board Fam Pract 11(4):321-323, 1998. © 1998 American Board of Family Practice] Introduction Although sickle cell trait is usually considered a benign condition, it has frequently been associated with hematuria and an increased incidence of pyelonephritis. This case report highlights the complications that can occur with sickle cell trait in a pregnant patient. Pregnancy enhances the susceptibility for renal problems, and pregnant patients who develop complications require close monitoring for anemia, which can ultimately cause renal or uteroplacental insufficiency and pose a threat to the fetus. We would like to alert physicians doing routine antenatal care to serious complications that can arise in patients with a sickle cell trait. Case Report A 30-year old woman (gravida 4, preterm 0, term 2, abortion 1, alive 2) attended the family medicine clinic for routine antenatal care at 6 weeks' gestation. She had no history of any serious medical illnesses, she did have sickle cell trait (HbAS), and she denied a history of alcohol, tobacco, or drug abuse.
