High-risk Wilms tumor phenotypes can generally be grouped as "overgrowth" syndromes or "non-overgrowth" syndromes. 1. Overgrowth Syndrome The most common disorders in the overgrowth category are Beckwith-Wiedemann syndrome and isolated hemihypertrophy. A. Beckwith-Wiedemann Syndrome Beckwith-Wiedemann Syndrome (BWS) is characterized by pre and postnatal overgrowth, omphalocele, macro-glossia, nephromegaly and ear lobe creases. Somatic hemihypertrophy may or may not be present. While 80% of BWS cases are sporadic, 20% show a complex familial inheritance pattern explained by alteration of genes at 11P15. For BWS in general, over-expression of the insulin-like growth factor II gene located at 11P15 is believed to cause the characteristic overgrowth and perhaps confer increased risk of Wilms tumor. The overall risk of neoplasia with BWS may approach about 8%, and specifically the Wilms tumor risk appears to be in the range of 3-5%. B. Isolated Hemihypertrophy

The National Cancer Institute created the PDQ database to increase the availability of new treatment information and its use in treating patients. Information and references from the most recently published literature are included after review by pediatric oncology specialists. Cancer in children and adolescents is rare. Children and adolescents with cancer should be referred to medical centers that have a multidisciplinary team of cancer specialists with experience treating the cancers that occur during childhood and adolescence. This multidisciplinary team approach incorporates the skills of the primary care physician, pediatric surgical subspecialists, radiation oncologists, pediatric medical oncologists/hematologists, rehabilitation specialists, pediatric nurse specialists, social workers, and others in order to ensure that children receive treatment, supportive care, and rehabilitation that will achieve optimal survival and quality of life. What causes this condition? What are treatment options? Hemihypertrophy (say: hem-ee-hi-per-troh-fee) is a rare condition in which one side of the body grows faster than the other. It can run in families, or occur by chance. Most people with hemihypertrophy look fairly normal, but might have to get different size shoes or gloves. Some people with hemihypertrophy have different leg lengths, and will need surgery or shoe lifts to correct this when they reach adulthood.

It could be an isolated finding in an normal individual or occur in several syndromes such as Beckwith-Wiedemann syndrome. Hemihypertrophy usually occurs sporadically and 1:86,000 in incidence. The clinical feature of hemihypertrophy is associated with neoplasm, most frequently with Wilms tumor, adrenocortical carcinoma, hepatoblastoma and rarely with rhabdomyosarcoma, neuroblastoma, pheochromocytoma or undifferentiated sarcoma of lung. The overall incidence of abdominal neoplasm associated with hemihypertrophy is 5.9% and average annual incidence in the first 4 years of life is 0.027 per person-year. In this article, we describe 2 patients with congenital asymmetry of limbs who presented with hepatoblastoma and were underwent surgical intervention. The postoperative recovery were smooth and the patients were doing well at present follow up.
