Hidradenitis Suppurativa I. Definition: Hidradenitis is a chronic disease of the scent or apocrine glands which causes chronic scarring and pus formation of the axillae and groin areas. It is similar to acne which is a disease of the sebaceous glands. This condition is slightly more common in African-Americans and women. Hidradenitis usually starts as one or more red, tender, swellings in the groin or axilla. Over a period of hours to days the lesions enlarge and often open to the skin surface draining clear to yellow fluid. The involved area then heals with scarring. The condition usually continues for years with periods of flare and remission. II. Causes and Associations: For unknown reasons, people with hidradenitis develop plugging or clogging of their apocrine glands. This leads to bacterial infection which can produce pain and odor. Hidradenitis is made worse by being overweight, however this condition is not caused by obesity. Hidradenitis is more common in people who have had acne. Hidradenitis may become worse under stress.

Hidradenitis Suppurativa is een chronische ziekte waarbij zich, vaak pijnlijke, met pus gevulde abcessen in liezen en/of oksels vormen. Ook kunnen abcessen voorkomen op de billen, in de schaamstreek en in en onder borsten. Minder vaak zijn nek, oren, borst en rug betrokken. De ontstekingen bij hidradenitis onderscheiden zich van 'gewone puisten' door hun afmetingen, vaak vele malen groter, en door hun vorm, vaak afgeplat, ovaal en zonder duidelijk aanwezige 'kop'. Vaak zijn er in de aangedane gebieden ook talgbultjes en kleinere ontstoken puistjes aanwezig. Fistelgangen kunnen zich vormen wanneer een ontsteking geen weg naar buiten vindt en ruimte zoekt om uit te breiden. Er ontstaan daardoor gangen/tunnels en waar de huid wat dunner of weker is geworden komt de inhoud van de ontsteking vaak door minuscule openingen naar buiten (constante drainage). Het beeld is bij iedereen anders en kan zeer wisselend zijn. De ene dag kunnen zich ontstekingen vormen en na enkele dagen kunnen deze spontaan verdwenen zijn. Dit bemoeilijkt natuurlijk het stellen van de juiste diagnose en ook de behandeling.

De mogelijke rol van talgklieren in hidradenitis was voorheen nog niet onderzocht. Acne behandeling, in het bijzonder isotretinoïne (vitamine A-derivaat), is echter niet effectief bij hidradenitis. Er was geen eerdere informatie te vinden over talguitscheiding op bepaalde plekken bij hidradenitis. Doel: het onderzoeken van de talguitscheiding en de kenmerken van huidvirilisatie (vermannelijking) bij hidradenitis patiënten. Methodes: Sebutapes ® en scores van acne, overmatige beharing en haaruitval bij 16 vrouwen met hidradenitis suppurativa werden vergeleken met een controle groep van 16 gezonde vrouwen. Usually S. aureus is initially involved, but gram-negative organisms such as Proteus may predominate in chronic cases. Symptoms, Signs, and Diagnosis The lesions may be confused with furuncles but tend to be more persistent and are diagnosed primarily by their location and clinical course. Clinically, the lesions are typically tender, reddish purple nodules resembling furuncles but occurring in apocrine sweat gland-bearing areas, including (in decreasing frequency) the axillae, the groin, around the nipples, and around the anus. Pain, fluctuation, discharge, and sinus tract formation are characteristic among patients who have had the disorder for years. In other chronic cases, coalescence of inflamed nodules may cause palpable cordlike fibrotic bands in the axilla. The condition may become extensive and disabling; if the pubic and genital areas are involved, walking may be difficult, and odors may be unpleasant.
