It is a common skin condition but is frequently misdiagnosed. Hidradenitis suppurativa does not occur before puberty. Most people are between 20 to 40 years old when they develop hidradenitis suppurativa. It can be a disabling and distressing skin condition. Cause of Hidradenitis Suppurativa The exact cause of hidradenitis suppurativa is disputed. Because it looks similar to acne, some investigators believe that it is caused by the formation of keratin plugs in the follicles. Because hidradenitis suppurativa occurs in areas that contain apocrine, or sweat, glands, other investigators believe that it is caused by a blockage of the apocrine gland. In either case, the plugged gland or follicle becomes larger, ruptures, and becomes infected. Obesity and cigarette smoking may be triggering factors.

This section is dedicated to alternative (not antibiotic) treatments suggested by discussions with other patients like me on the web and which, OF COURSE, none of us has used since the FDA has not approved them for the treatment of hidradenitis suppurativa.
Those of us who have had this disease for over twenty years are generally immune to most antibiotics (I think that I use Keflex and Cipro as nutrients) or have become sensitive to them (Tetracycline makes me break out in an itchy rash which, when scratched, causes huge black and blue marks which also itch). Some of us have been beaten up by this disease badly enough so that our immune systems for staph and/or strep are partially or totally compromised ( My body starts singing "Its a Wonderful Day in the Neighborhood" when it sees staph and invites it in to tunnel and waste tissue and gives it a map so that it can tunnel through sinus cavities in what parts of my tissue are not scarred by prior tunneling).
The number of massive plastic surgeries many of us have had (mine are well into double digits) serve not to completely rid us of lesions in that area but merely to remove the worst areas of scarring and tunneling to avoid the worst of the complications of long term HS.

    Hidradenitis Suppurativa
    Hidradenitis is a chronic disease of the apocrine glands (a form of sweat gland found on certain parts of the body). For unknown reasons, people with hidradenitis develop plugging or clogging of their apocrine glands. It causes chronic scarring and pus formation of the underarms (axilla) and groin/inner thigh areas. In women it can also occur under the breasts. It is similar to acne, which is also a disease of the sebaceous glands. Hidradenitis is more common in people who have had acne. It may be an unusual type of adult acne.
    This condition is slightly more common in women and African-Americans. Hidradenitis usually starts as one or more red, tender, swellings in the groin or armpits. Over a period of hours to days the lesions enlarge and often open to the skin surface draining clear to yellow fluid. The involved area then heals with scarring. The condition usually continues for years with periods of flare and remission.
    Bacterial infection produces the pain and odor. Hidradenitis is made worse by being overweight, however this condition is not caused by obesity and weight loss will improve but not cure hidradenitis. Hidradenitis may become worse under stress. Hidradenitis is not caused by poor hygiene.

    Summary Hidradenitis suppurativa is a chronic and often disabling disease characterized by intermittent periods of inflammation and abscess formation in apocrine sweat gland-containing skin. Various therapies have been implemented in its treatment with variable results. Pathogenesis, histopathology and therapy of this disease are reviewed.
    Introduction
    Hidradenitis suppurativa (HS) is a chronic disease of unknown etiology that mainly affects the genitofemoral, perianal and axillary regions (1-4). The disease is believed to be follicular in origin (1). HS has a higher prevalence in women than men and is estimated to affect 4 percent of women in the general population (5).
    Etiology
    HS often occurs in multiple members within families, suggesting a genetic predisposition (5). Various etiological agents have been associated. The increased incidence of HS in obese women with acne led to the theory of HS being associated with an hyperandrogenic endocrine disorder (6,7). One study did not find evidence for biochemical hyperandrogenism in women affected with HS (8). Thus, the role of androgens in HS is still not clear. Smoking (9), lithium (10) and oral contraceptives (11) may also be associated with HS, possibly as triggering factors. Nonetheless, the etiology of the disease is unknown.
    Pathogenesis
    Historically, apocrinitis was believed to be the defining histologic feature and pathogenetic mechanism of HS (12,13). Characteristic lobular abscesses in the apocrine gland, demonstrated histologically, seemed to further implicate apocrine gland involvement in the manifestation of disease (12-14). Recently, histological evidence points to HS being a follicular disease (1,15-19). Histological examination in the majority of specimens reveals follicular involvement, including poral occlusion and folliculitis (1). Apocrinitis as the dominant histological feature is found in only a small number of specimens (1,17). Furthermore, a paucity of apocrine glands was demonstrated in the genitofemoral region, one area commonly affected by HS (1). This finding supports the theory that apocrine gland inflammation is not the etiological and pathogenetic mechanism of HS, but rather a secondary manifestation of follicular involvement (1).

    Hidradenitis can get worse over a period of time; many of our members have multiple lesions on different parts of the body at the same time. On the other hand, there are members who flare only occasionally and/or don't progress. If HS does progress, fistulas and sinus tracking (under the skin) may form, causing even more problems. The pain and inflammation can be disabling. Experiences with HS are varied - some people go through experimental treatments (radiation), radical surgery (requiring skin grafts), the gamut of medications (long-term antibiotics, accutane), and/or frequent lancings. The sores are not typically infected; in studies, more than 50% of lesions cultured do not show any bacteria. But, it is important to note that cellulitis can take over areas, and this requires immediate medical attention.
    As you can imagine, many people who have HS deal with depression and physical pain on a daily basis. There are different theories about what causes HS - there appears to be a strong genetic link, and the cycles of the disease indicate hormonal imbalance(s). HS is often seen in conjunction with thyroid disease, polycystic ovarian syndrome, diabetes, chrohn's disease, and several autoimmune disorders. We are always looking for reasons WHY and HOW the disease affects us - some people with HS have literally dedicated their lives going research and outreach about HS, hoping to find answers.

    What is Hidradenitis Suppurativa? Hidradenitis Suppurativa (HS) is also known as 'Acne Inversa' and is a non-contagious, recurrent skin disease usually found in inverse areas, or those places where there is skin-to-skin contact (armpits, groin, breasts, etc.) and around hair follicles where apocrine sweat glands are located. It is characterized by a progression from boils, or abscesses, to non-inflamed, hard lumps, painful, rounded deep-seated inflamed lesions with subsequent scarring and chronic seepage (suppuration - hence the name). These hard lumps occur under the skin and may be as large as baseballs in some people, are very painful to the touch and may persist for years with occasional to frequent periods of inflammation. Inflamed lumps lead to extensive draining and the development of sinus tracts, or tunnels under the skin, which heal slowly or not at all and lead to further inflammation and lumps; bacterial infections are possible at these sites. Drainage of the lumps provides some relief from severe pain; however, pain occurs 24 hours a day, 7 days a week for HS sufferers during flare ups. Because of the drainage, which may have a foul odor, and the pain, people often do not work or even go out in public.

    Background This is a chronic and relapsing condition occurring within apocrine sweat glands, most commonly in women. Deep seated inflammatory nodules form in the axilla, groin, pubic region, and perineum. It is an androgen-dependent disorder and results from keratinous material occluding the sweat pores. There is an association with lithium therapy and Crohn's disease. The skin shows a deep seated mixed inflammatory cell infiltrate with scarring, granulation tissue, and foreign body giant cells. Chronic abscesses and sinus tracts leading to the surface may develop. Early and new lesions show a suppurative folliculitis. Outline
