Hydrocephalus Hydrocephalus is that state in which something has occurred to prevent this orderly procession of events. If this occurs, there is a relative build up of spinal fluid in the brain which can lead to injury or even death if not treated. It is very important to realize that hydrocephalus is the result of some event on the nervous system and that a child's prognosis is not so much based on the hydrocephalus as the cause of the hydrocephalus. While many cases have no clear cause, the following have been associated with the development of hydrocephalus; bleeding, infection, trauma, tumors, vascular problems, and structural problems. Some occur during pregnancy and others after birth. In addition, a small number can be transmitted genetically. It is important to realize that while hydrocephalus is very treatable, the underlying cause may not be or may have caused irreversible damage to the brain. It is important for the doctors taking care of a child with hydrocephalus to determine if the problem is ongoing or if it has resolved but left enlarged but stable ventricles. This can sometimes be difficult to tell and often the decision will be based on many factors including symptoms, eye exams, changes in level of activity or school performance. In addition to this often the child will have repeated (serial) radiographic exams to look for changes in the ventricles.

Hydrocephalus is most often treated with the surgical placement of a shunt system. This system diverts the flow of CSF from a site within the central nervous system to another area of the body where it can be absorbed as part of the circulatory process. A shunt is a flexible but study silastic tube. A limited number of patients can be treated with an alternative procedure called third ventriculostomy. In this procedure, a small hole is made in the floor of the third ventricle, allowing the CSF to bypass the obstruction and flow toward the site of resorption around the surface of the brain. The term hydrocephalus is derived from the Greek words "hydro" meaning water and "cephalus" meaning head. As its name implies, it is a condition in which the primary characteristic is excessive accumulation of fluid in the brain. Although hydrocephalus was once known as "water on the brain," the "water" is actually cerebrospinal fluid (CSF) - a clear fluid surrounding the brain and spinal cord. The excessive accumulation of CSF results in an abnormal dilation of the spaces in the brain called ventricles. This dilation causes potentially harmful pressure on the tissues of the brain. The ventricular system is made up of four ventricles connected by narrow pathways. Normally, CSF flows through the ventricles, exits into cisterns (closed spaces that serve as reservoirs) at the base of the brain, bathes the surfaces of the brain and spinal cord, and then is absorbed into the bloodstream.

HYDROCEPHALUS Hydrocephalus is excessive accumulation of cerebrospinal fluid within the cranium. If persistent and progressive ,it is treated by shunt insertion. Recently other methods of treatment were introduced and these will be discussed in the appropriate section. About 30 years ago before shunts were available children with hydrocephalus, either died or left with severe disability. With the introduction of shunts procedures, hydrocephalic child can lead a reasonable normal life and normal life expectancy. Hydrocephalus is described either as obstructive or noncommunicating and non-obstructive or communicating. This relates to the level of obstruction and whether CSF exits from the foramina in the roof of the 4th ventricle or is fully trapped within the ventricular system. Hydrocephalus is also classified as acquired when it is caused by acquired causes or congenital. In congenital hydrocephalus it is caused by either excessive production of CSF, blockage to the CSF drainage or reduced excretion into the venous channels. The CSF is clear colourless fluid which has minimal content of protein. It is contained in the ventricular system as well as the subarachnoid spaces surrounding the brain and the spinal cord. The CSF is in hydrostatic equilibrium with the interstitial tissue of the brain and can permeate across the brain tissue in both directions. It is expected that the brain tissue and the CSF would have the same hydrostatic pressure in any part of the brain. As much as the brain tissue is protected by a blood brain barrier from changes outside the central nervous system, the CSF has the same protection and does not change biochemically as a result of changes in the systemic circulation. These barriers are at the level of the endothelium of brain capillaries, at the level of the epithelium of the choroid plexuses and the outer layers of arachnoid matter. These barriers protect the brain and the subarachnoid spaces from damaging influences outside the brain.

What Is Hydrocephalus? Hydrocephalus comes from the Greek: "hydro" means water, "cephalus" means head. Hydrocephalus is an abnormal accumulation of cerebrospinal fluid (CSF) within cavities called ventricles inside the brain. CSF is produced in the ventricles, circulates through the ventricular system and is absorbed into the bloodstream. CSF is in constant circulation and has many important functions. It surrounds the brain and spinal cord and acts as a protective cushion against injury. CSF contains nutrients and proteins necessary for the nourishment and normal function of the brain. It also carries waste products away from surrounding tissues. Hydrocephalus occurs when there is an imbalance between the amount of CSF that is produced and the rate at which it is absorbed. As the CSF builds up, it causes the ventricles to enlarge and the pressure inside the head to increase. Hydrocephalus that is congenital (present at birth) is thought to be caused by a complex interaction of environmental and perhaps genetic factors. Aqueductal stenosis and spina bifida are two examples. Acquired hydrocephalus may result from intraventricular hemorrhage, meningitis, head trauma, tumors and cysts. Hydrocephalus is believed to occur in about 2 out of 1,000 births. The incidences of adult-onset hydrocephalus and acquired hydrocephalus are not known. How Is Hydrocephalus Treated?
