DEFINITION:
This condition is characterized by an ectopic urethral opening which may be located ventrally on the glans, penile shaft, scrotum or perineum. The hypospadiac meatus may be either normal in diameter, wide, very wide or stenotic (usually in distal forms of hypospadias). Often, there is a blind ending periurethral duct with its orifice placed distally from the hypospadiac meatus. The glans may be flattened or conical in shape, and the glanular groove can be well or poorly developed...

Clinical Overview
Introduction
Hypospadias is a congenital defect of the penis resulting in incomplete development of the penile urethra. The abnormal urethral opening may be any place along the shaft of the penis or may open onto the scrotum or the perineum. As the position of the urethral meatus becomes more proximal, ventral shortening and curvature during penile erection are more likely. In patients with severe hypospadias, the genitalia may look ambiguous at birth, resulting in emotional and psychological stress for parents in that the gender assignment of their baby immediately comes into question. Left uncorrected, patients with hypospadias may need to sit down to void and tend to shun intimate relationships because of the fears related to normal sexuality.
Incidence
Hypospadias is one of the most common congenital anomalies, occurring in approximately 1 in 250 to 1 in 300 live births. In Europe, the prevalence of hypospadias in the 1970's and 1980's has been increasing with no obvious explanation. In the United States, data from two birth defects surveillance systems has also shown an unexplained doubling in the incidence of hypospadias. (Paulozzi, Erickson et al. 1997) The U.S. study from the Centers for Disease Control is particularly intriguing in that the incidence of severe hypospadias, not just mild forms, is increasing, implying that the increase in hypospadias is not due to an increase in surveillance or reporting.

Elle n'est pas rare car elle touche 8 garçons sur 1000 naissances masculines. Cette affection peut être génétique, donc avec hérédité possible, mais également due à certains médicaments (hormones) pris pendant la grossesse. Le méat urinaire, l'orifice par lequel les urines peuvent s'écouler, se trouve normalement à l'extrémité du pénis, dans l'axe de celui-ci.
En cas d'hypospadias, le méat urinaire se trouve sur la face ventrale, le dessous, du pénis, entre l'extrémité du pénis et les bourses, quelques fois même dans les bourses.
En cas d'épispadias, le méat se trouve sur la face dorsale, le dessus, du pénis. Le pénis est souvent court, et peut ne pas être rectiligne. De plus, le fait que le méat s'abouche de manière anormale peut entraîner des troubles de l'érection. Plus l'hypospadias est sévère (plus le méat est situé loin de l'extrémité du pénis), plus il est important de réaliser au plus tôt une étude du sexe génétique de l'enfant (risque d'ambiguité sexuelle).
La conséquence de ces malformations est une gêne à la miction (uriner), et surtout, plus tard, une diminution du pouvoir fécondant (déposer le sperme au fond du vagin lors des rapports sexuels) pouvant aller jusqu'à la stérilité.

An eight-month-old, crossbreed dog, weighing 9 kg, was admitted to the Veterinary Hospital with congenital abnormalities of the urogenital tract. It was the only animal of the litter with deformity. On physical examination, underdevelopment of the penis, fusion failure of the urethra, prepuce and scrotum (Figure 1a), and an external urethral orifice located in the perineum about 1.2 cm below the anus (Figure 1b), were observed. Testicles could be palpated in the scrotal area. A groove covered by mucosa extended cranially from the cleft scrotum to the glans penis area. The dog did not display dysuria or urinary incontinence, but he squatted to urinate. Alteration of the urinary bladder shape with dilatation of its neck and presence of ventral diverticulum were observed on ultrasonographic examination (Figure 2a) and retrograde cystourethrography (Figure 2b). Chromosome analysis was performed. Peripheral blood samples and tissue were cultured as described (1, 2). Sixty cells were cytogenetically analyzed and the chromosomes were identified and classified according to Mellink and Bosma (1989) (3). Normal karyotype (78, XY) was found in all mitotic metaphases cells analyzed by Giemsa staining (Figure 3). The diagnosis was perineal hypospadias.

What is hypospadias?
Hypospadias is the medical word for a condition in which the urinary opening (meatus) does not form as usual while a baby is growing before birth. It is one of the most common birth defects in boys, with approximately 8 out of 1,000 male newborns being affected. Rarely, it can occur in girls too, but is not as obvious. This brochure will discuss hypospadias in boys. Some cases of hypospadias are very minor and others are more complex. In a normal male, the urinary opening is usually exactly in the middle of the penis at the tip. The pictures below show the normal location of the opening and the different places where the opening might occur in boys with hypospadias.

My 21 year old son had a mild hypospadias repaired as a toddler. He seems to be the only occurence of this defect in the family. We have been told that he has a slightly higher than average chance of passing this defect on to his own offspring. Is there a relationship between this congentital defect and others such as intersex, cryptorchidism, other midline defects? Is he at any greater risk of fathering a child with these problems? Dear Harold,
The incidence of hypospadias has been calculated at 1 in 300 live male births. In the US, about 6000 boys are born each year. Familial tendencies indicate some genetic factors due to the fact that fathers with hypospadias have an 8% chance of passing the defect on to each male child he fathers. Also, 14% of male siblings are affected. This condition is more common in whites than blacks.
The most common associated anomalies would include undescended testes and inguinal hernia.

Hypospadias is a condition of varying severity in which the opening of the urethra ( the urethral meatus) is not at the tip of the penis, where it usually is, but instead is variably located along the head and shaft below, on the bottom of the penis. Not infrequently, chordee is also present along with hypospadias. This is a band of fibrous tissue which connects the tip of the penis to the frenulum in the bottom, thereby, causing a bowing/ bending of the penile tip which is a characteristic appearance of the chordee. The two conditions may also be present independently. As far as two openings at the tip of the penis is a separate entity referred to as urethral duplication. The two openings may be one on top of the other, or side by side, the former being the more common one. There are different types of urethral duplication in terms of their internal anatomy varing from both openings being functional and connected to the bladder, to one being only a “blind pit” or just an impression of an opening. In any case this requires close and careful examination. Given your history of encounters as you described above, you may seek another urologist’s opinion to settle the controversy, since without an examination, I could not make a definitive statement.

Hypospadias is a congenital defect that affects up to 3 in 1,000 newborn boys. The condition varies in severity. In most cases, the opening of the urethra is located near the tip of the penis on the glans. More severe forms of hypospadias occur when the opening is at the midshaft or the base of the penis.Occasionally the opening is located in the scrotum or the perineum (beneath the scrotum). This anomaly is often associated with chordee, a tight fibrous band that results in a downward curvature of the penis seen with erection. (Erections are common with infant boys.) Some cases are inherited; others result from unknown causes.
