    Hypotension
    Baisse de la pression artérielle systolique au-dessous de la normale, c'est-à-dire de 100 mm de mercure. Ce terme recouvre également la tension intérieure d'un organe en tant que pression, par exemple l'hypotension oculaire.

    Il existe plusieurs formes d'hypotension :
    • L'évolution de la maladie se fait quelques années plus tard vers des troubles articulaires et tendineux associés à des problèmes de marche, avec tremblements, rigidité dans les mouvements, ainsi que des troubles de la parole. La maladie de Shy et Drager, qui évolue vers la mort après une période très longue, est secondaire à une atteinte anatomique à type de dégénérescence (destruction progressive) des noyaux gris du cerveau (amas de substance grise noyés dans la substance blanche) mais également des noyaux gris situés dans la moelle épinière et à d'autres niveau de l'encéphale (système nerveux à l'intérieur du crâne). Ces noyaux gris portent le nom de locus niger, noyau dorsal du vague, locus caeruleus, putamen. Ces localisations nerveuses permettent, quand leur état est normal, d'assurer le bon fonctionnement automatique de certains organes (régulation de la tension artérielle, régulation de la sueur, adaptation de l'ouverture et de la fermeture des vaisseaux, etc…).
    • · L'hypotension contrôlée (en anglais induced hypotension) se caractérise par l'abaissement de la tension artérielle au cours de certaines interventions chirurgicales nécessitant une diminution de la pression du sang à l'intérieur des vaisseaux. C'est le cas, entre autres, des interventions portant sur les lésions de la face et de la mâchoire (chirurgie maxillo-faciale) ou des tumeurs vasculaires (touchant les vaisseaux) situées dans l'encéphale (à l'intérieur du crâne). L'hypotension contrôlée est mise en place pour éviter les hémorragies susceptibles d'avoir lieu au cours de l'intervention.

    Hypotension Hypotension is the clinical term for low blood pressure, which can mean inadequate blood flow to the heart, brain and other vital organs. Blood pressure is measured in millimeters of mercury (mm HG). For a healthy young adult, normal blood pressure is considered to be less than 140mm Hg over 90mm Hg. The first number in this case, 140 is systolic blood pressure, the pressure in the blood vessels when the heart contracts; the second number in this case, 90 is the pressure between contractions. In most healthy people, blood pressure remains above 90/60 (or, 90 over 60). However, blood pressure varies from person to person; what is considered borderline low for one person may be normal for another. Chronic low blood pressure is almost never serious. But health problems occur when blood pressure drops suddenly and the brain is deprived of an adequate blood supply. This can lead to dizziness or lightheadedness. It most commonly occurs in someone who's rising from a prone or sitting position to a standing position. In such cases, it is known as postural hypotension, orthostatic hypotension or neurally mediated orthostatic hypotension. Clinically, postural hypotension is defined as a 20mm Hg–reduction in systolic blood pressure or a 10mm Hg–reduction in diastolic blood pressure within three minutes of standing.

    Contributing risk factors Recommendations Most physicians in the English-speaking world consider hypotension - low blood pressure - to be a symptom of some other disorder. In other parts of the world, however, hypotension is itself considered to be a disorder that can cause various symptoms, including depression, lethargy and fatigue. This different attitude is probably the result of different methods of medical training. There are different kinds of hypotension and the following is one way of classifying them:
    • Constant Hypotension. The blood pressure is always low, less than 90/60 mmHg. When the blood pressure is too low there is inadequate blood flow to the heart, brain and other vital organs. Borderline low blood pressure for one person may be normal for another. Many people have a chronic problem with low blood pressure that is not particularly serious. Temporary Hypotension. The most important factor is how the blood pressure changes from the baseline or normal condition. A fall of 30 mmHg or more from a person’s baseline is considered low blood pressure. This can be caused by any of the following:
      • Shock Stress/trauma Sudden change in posture (orthostatic hypotension) Allergy Dehydration Drug-induced, including alcohol toxicity

    I read your discussion of vertigo and wonder if that is what I’m experiencing. When I get up, say from a chair, I feel dizzy, start seeing black or black dots, and my legs sometimes feel like they are going to give out. Sometimes I have to lean up against something to keep from falling down. I do have trouble with my balance, although there is no spinning. Is this vertigo?

    Although vertigo can be triggered by standing, it's less likely to be vertigo if there's no sensation of the body, or surroundings, moving or spinning. There is another type of dizziness upon standing that's caused by postural hypotension. This type of dizziness, faintness or light-headedness results from a decrease in blood flow to the brain, due to a drop in blood pressure upon standing up.
    At one time or another, we’ve probably all experienced mild postural hypotension from standing up too quickly. As long as it occurs only occasionally, there’s little cause for alarm.
    But chronic postural hypotension can be a debilitating condition, associated with increased risk of injury from accidents and falls, and obviously is a cause for concern. Persistent light-headedness or faintness upon standing may be a symptom of a serious underlying disorder. Anyone with this problem should be seen by their physician.

    Low Blood Pressures run natrually in my family. Normally my BP is 80/62. I recently became a victim of heat stress, and the Paramedic told me my BP was alright..120/76. My concern is if I am not able to communicate the fact that I have this condition, how will those giving me aid know? It is important to bear in mind the other possible causes of a low blood pressure in the parturient who has a functioning epidural block ( Table 6.2 The incidence of hypotension can be markedly reduced by the employment of preventative measures. A reduction from 79%-100% to 0-10% has been demonstrated as a result of the use of the left lateral position, intravenous preloading, and vasopressor therapy (2, 3).     Many CFS patients experience lightheadedness or worsened fatigue when they stand for prolonged periods or when in warm places, such as in a hot shower. These conditions are also known to trigger NMH. One study observed that 96% of adults with a clinical diagnosis of CFS developed hypotension during tilt table testing, compared with 29% of healthy controls. Tilt table testing also provoked characteristic CFS symptoms in the patients. A study (not placebo-controlled) was conducted to determine whether medications effective for the treatment of NMH would benefit CFS patients. A subset of CFS patients reported a striking improvement in symptoms, but not all patients improved. A placebo-controlled trial of NMH medications for CFS patients is now in progress.
