Clinical Hypothyrodism
Hypothyroidism usually is caused by an abnormality of the immune system that results in damage and destruction of the thyroid gland. This process results either in loss of thyroid tissue or an enlargement of the thyroid. The gland has the shape of a bow tie or butterfly, and is located just below the larynx, or "Adams Apple", and in front of the trachea, or windpipe. In most instances there is no pain or tenderness associated with thyroid diseases, although patients occasionally complain of difficulty in swallowing as if there were a lump in their throat.
Often the only sign of hypothyroidism during childhood is an abnormal rate of linear growth. Actually the child may not be short compared to other children of the same age if he or she were above average height before the disease occurred. Therefore, the most important feature of hypothyroidism is a decrease in the rate or velocity of growth in height. If the disease is recognized early and adequately treated, the child will grow at an accelerated rate until reaching the same growth percentile where the child measured prior to the onset of hypothyroidism.

Primary Hypothyroid
Primary hypothyroidism occurs when disease of the thyroid gland prevents it from producing adequate amounts of thyroid hormone. Symptoms may vary from mild to severe and from nonspecific to very specific. In general, all metabolic processes slow down. Patients are often fatigued, and may also have depression, decreased intellectual function, and, rarely, overt psychosis. Patients frequently complain of cold intolerance, dry skin, and constipation. The heart rate is often slowed, as is the relaxation phase of deep tendon reflexes. There may be high cholesterol, mild anemia, and high serum enzymes, including CPK. The T4 and Free T4 is low and the TSH is always high (usually greater than 20 mU/L) . T3 may be normal.

43 y/o patient with the chief complaint of "decreased energy." She complains of fatigue, inability to finish tasks, sleeping more, yet always being tired. She has also noticed a decreased cold tolerance, constipation, and dryness of the skin. She comments that she is always cold, even in the summer. Seconday Hypothyroid (Hypopituitarism)
Secondary hypothyroidism is defined as the failure of the thyroid gland due to the inadequate production of Thyroid Stimulating Hormone (TSH). This can occur due to primary failure of the pituitary or as the result of hypothalamic dysfunction (tertiary hypothyroidism). Pituitary failure can result from destruction by pituitary and non-pituitary tumors, head trauma, radiation, postpartum infarction (Sheehan’s syndrome) or unknown causes (idiopathic). Generally the symptoms of hypothyroidism are less severe than in primary hypothyroidism and are often complicated by deficiencies of other pituitary hormones, as well as the mass effects of the intracranial tumor. Patients with secondary hypothyroidism must be evaluated for pituitary tumors and other hormone deficiencies.

Nutritional Therapy for Thyroid Health and Hypothyroid Support
This article discusses clinical and sub-clinical hypothyroid conditions and applicable nutritional support measures. It is not the intent of this article to diagnose, instruct individuals in self diagnosis or instruct individuals in the treatment of thyroid disease. The thyroid is a small butterfly-shaped gland, located in the neck, wrapped around the windpipe, and is located behind and below the Adam's Apple area. The thyroid produces several hormones, of which two are key: triiodothyronine (T3) and thyroxine (T4). These hormones help oxygen get into cells, and regulate metabolism. The hypothalamus in the brain releases Thyrotropin-releasing Hormone (TRH) which tells the pituitary gland to release Thyroid Stimulating Hormone (TSH). The circulating TSH in your bloodstream is what tells the thyroid to make thyroid hormones and release them into your bloodstream. This feedback loop is affected by emotions, stress and the state of health of the entire nervous and endocrine system. Diminished thyroid activity, or hypothyroidism, decreases metabolic rate, makes it impossible to assimilate food properly, depresses the activity of other glands in the body and ultimately adversely affects every cell and tissue in the body.

    HELP! My TSH Is "Normal" But I Think I'm Hypothyroid

    Many people write to me, asking if they could be hypothyroid, despite having had their TSH tested, and it comes back in the "normal" range. These people often describe a list of standard hypothyroid symptoms (see my article "Symptoms, Symptoms, Symptoms" ) and have shown low basal body temperature tests, but standard TSH testing has shown them to be in the "normal range." Here are some things to pursue in getting a diagnosis. What is your lab's "normal" range, and how does your doctor define "normal?"
    Have you been tested for thyroid antibodies?

    Have you had a TRH test?

    Could You Have an Underlying Additional Problem that Requires More than Just Standard Thyroid Hormone Replacement such as Levothyroxine Sodium/Synthroid?
