Imperforate Anus Imperforate anus is an abnormality of the anus (the opening through which feces pass out of the body). The anus may not properly connect with the bowel (large intestine), or it may be located in an abnormal position. Some babies with imperforate anus also have an abnormal connection (called a fistula) that allows feces to flow from the bowel to the bladder, urethra or vagina, or to leave the body through an opening near the scrotum. Depending on the location of the defect, an imperforate anus may be classified as a "high lesion" or a "low lesion." A high lesion is more likely to be associated with a fistula. If there is no way for feces to be expelled from the intestine, then the baby is said to have an intestinal obstruction. This can cause serious and even life-threatening complications. As feces build up within the bowel, the intestine may be severely stretched, sometimes so much that it develops a weakened area in the wall, and then a hole (intestinal perforation). Such a perforation allows stool to leak out into the abdomen, which can cause a severe infection Imperforate anus is a relatively common birth defect, occurring in about one out of every 5,000 newborns. It is more common in boys than in girls. Babies with imperforate anus also have a high likelihood of other associated birth defects, often of the esophagus, duodenum (first section of the intestine), kidneys, heart or central nervous system (brain and spinal cord). Introduction - How to Make the Best Use of This
This document came into being from a combination of factors. First, the pullthrough list archives prior to Jan. 2000 are no longer accessible. I had saved many notes on various topics relating to Stephen’s problems that I thought could help other parents. Second, there are parents who have no computer access who could benefit from the information on the PTL list serv and on the web. Finally, and most importantly, my husband and I felt strongly that God was calling me to organize the information I had to help other parents. So, I dug out all of my saved notes, articles, PTN newsletters, etc. and attempted to summarize, assemble, and organize the information.

Medical Encyclopedia Encyclopedia Surgery I -> Imperforate anus repair Imperforate anus repair Alternate Names: Anorectal malformation repair; Perineal anoplasty Description: Surgical repair involves creating an opening for passage of stool. Complete absence of an anal opening requires emergency surgery for the newborn.
Surgical repairs are done while the baby is deep asleep and pain-free (using general anesthesia).
Surgery for a high type imperforate anus defect usually involves creation of a temporary opening of the large intestine (colon) onto the abdomen to allow passage of stool (this is called a colostomy ). The baby is allowed to grow for several months before attempting the more complex anal repair.
The anal repair involves an abdominal incision, loosening the colon from its attachments in the abdomen to allow it to be repositioned. Through an anal incision, the rectal pouch is pulled down into place, and the anal opening is completed. The colostomy may be closed during this stage or may be left in place for a few more months while the tissues heal, and closed at a later stage.
Surgery for the low type imperforate anus (which frequently includes a fistula ) involves closure of the fistula, creation of an anal opening, and repositioning the rectal pouch into the anal opening.

  • Epidemiology -
    • 1 in 4500 live births
  • Classification
    • LOW ABNORMALITIES
      • Easy to identify, easy to treat
      • The gut ends blindly at level below the pelvic diaphragm
      • There are 4 lesions in the low type -
      • Covered anus
      • Anus covered with a skin tag
      • The opening is situated anteriorly in the midline depending on the length of the skin tag
      • Ectopic anus
      • The anal opening is not present in the normal place
      • The anal opening is commonly in he perineum in boys or into the vulva in girls.
      • Anal stenosis
