In the United States, at least 13 million people have problems holding urine until they can get to a toilet. This loss of urinary control is called "urinary incontinence" or just "incontinence." Although it affects many young people, it usually disappears naturally over time, which suggests that incontinence, for some people, may be a normal part of growing up. No matter when it happens or how often it happens, incontinence causes great distress. It may get in the way of a good night's sleep and is embarrassing when it happens during the day. That's why it is important to understand that occasional incontinence is a normal part of growing up and that treatment is available for most children who have difficulty controlling their bladders.
How Does the Urinary System Work?
Urination, or voiding, is a complex activity. The bladder is a balloonlike muscle that lies in the lowest part of the abdomen. The bladder stores urine, then releases it through the urethra, the canal that carries urine to the outside of the body. Controlling this activity involves nerves, muscles, the spinal cord, and the brain. The bladder is made of two types of muscles: the detrusor, a muscular sac that stores urine and squeezes to empty, and the sphincter, a circular group of muscles at the bottom or neck of the bladder that automatically stay contracted to hold the urine in and automatically relax when the detrusor contracts to let the urine into the urethra. A third group of muscles below the bladder (pelvic floor muscles) can contract to keep urine back.

Female urinary incontinence - studies of epidemiology and management in general practice

Female urinary incontinence (UI) is a condition with severe economic and psychosocial impact . I have studied the epidemiology of UI in the general female population and the prevailing management of UI by Norwegian general practitioners (GPs). Those who are interested in more details, may take a look at the synopsis below. The main conclusions that may be drawn from these studies are that UI is very prevalent among adult women, mostly in the form of stress incontinence. At least 6% may be regarded as potential patients. Although complicated by methodological difficulties, studies of GPs' management indicate that available therapeutic tools are not used to their full potential. A literature review demonstrates that the history is the most important diagnostic tool in general practice, and that the value of clinical investigations is poorly documented.
The general purpose of the present investigations was to describe the epidemiology of UI in the adult female population, and to describe the management offered these patients by Norwegian GPs. Secondly, the validity of some of the employed methods was investigated. Studies were undertaken with the following specific aims:
  • to validate a severity index and diagnostic questions for different types of female UI
  • to describe the prevalence of female UI in the general adult population, according to different types and severity

What is incontinence?
Incontinence is the impaired ability to control gas or stool. Its severity ranges from mild difficulty with gas control to severe loss of control over liquid and formed stools. Incontinence to stool is a common problem, but often it is not discussed due to embarassment. Both bladder and bowel incontinence are problems that tend to increase with age.
What causes incontinence?
There are many causes of incontinence. Injury during childbirth is one of the most common causes. These injuries may cause a separation in the anal muscles and decrease in muscle strength. The nerves supplying the anal muscles may also be injured. While some injuries may be recognized immediately following childbirth, many others may go unnoticed and not become a problem until later in life. In these situations, past childbirth may not be recognized as the cause of incontinence. Anal operations or injury to the tissue surrounding the anal region similarly can damage the anal muscles and hinder bowel control. Infections around the anal area may destroy muscle tissue leading to problems of incontinence. In addition, as people age, they experience loss of strength in the anal muscles. As a result, a minor problem in a younger person may become more significant later in life. Diarrhea may be associated with a feeling of urgency or stool leakage due to the frequent liquid stools passing through the anal opening. If bleeding accompanies lack of bowel control, consult your physician. These symptoms may indicate inflammation within the colon (colitis), a rectal tumor, or rectal prolapse - all conditions that require prompt evaluation by a physician.

This is a disorder that people feel they must hide from others and talk about only in whispers." Many otherwise healthy, active individuals suffer from incontinence. Fecal incontinence (also called bowel incontinence or anal incontinence) affects people of all ages. We are not only here for people who would call themselves "incontinent". Leakage from the bladder is all too common, especially if you laugh, sneeze or exercise ("stress incontinence"). Less common is leakage from the bowel. But other problems are:
  • needing often to hurry to the toilet to empty your bladder needing to empty your bladder very many times a day having to get up several times a night to go wetting the bed in your sleep - can happen to people of all ages, not only children your bladder does not empty completely you need adaptations to your home or your clothes to make going to the toilet easier you have a learning difficulty or memory problems that affect your ability to use the toilet
