Causes, incidence, and risk factors Return to top Primary infertility is the term used to describe a couple that has never been able to conceive a pregnancy , after a minimum of 1 year of attempting to do so through unprotected intercourse. Secondary infertility is the term used to describe couples who have previously been pregnant at least once, but have not been able to achieve another pregnancy. Causes of infertility include a wide range of physical as well as emotional factors. Approximately 30% to 40% of all infertility is due to a "male" factor, such as retrograde ejaculation impotence , hormone deficiency, environmental pollutants, scarring from sexually transmitted disease, or decreased sperm count . Some factors affecting sperm count are heavy marijuana use or prescription drugs, such as cimetidine, spironolactone, and nitrofurantoin.

    Polycystic ovarian syndrome (PCOS) accounts for 90% of women with oligomenorrhoea (infrequent periods) and 30% of women with amenorrhoea (absent of periods) and over 70% of women with anovulation.
    Diagnosis of Polycystic Ovarian Syndrome
    Laparoscopy Laparoscopy allows direct inspection of the ovaries; the ovaries are enlarged and polycystic. However, polycystic ovaries may appear normal at laparoscopy. Vaginal ultrasound scan (better than abdominal) The vaginal ultrasound may show the typical PCOS appearance but reliability varies with expertise. Blood hormone levels of LH, FSH, androgens and SHBG Ideally, these tests should be performed during the first four days of the cycle. If the women has no period, then the test can be performed anytime, and repeated if the results do not provide a clear picture. FSH levels are low or normal, LH levels are often raised. However, a normal level does not exclude diagnosis of polycystic ovarian syndrome (PCOS). The levels of androgens and testosterone may be raised.

    Immunology May Be Key To Pregnancy Loss By Carolyn B. Coulam M.D. and Nancy P. Hemenway Until the last decade, there was little a couple could do if they suffered from recurrent pregnancy losses. Miscarriages that couldn't be attributed to chromosomal defects, hormonal problems or abnormalities of the uterus were labeled "unexplained," and couples would continue to get pregnant, only to suffer time and again as they lost their babies. New research, however, indicates that as many as 80 percent of "unexplained" losses may the attributable to immunological factors-and some new therapies are enabling up to 80 percent of those affected to carry a baby to term. About 15 to 20 percent of all pregnancies result in miscarriage, and the risk of pregnancy loss increases with each successive pregnancy loss. For example, in a first pregnancy the risk of miscarriage is 11 to 13 percent. In a pregnancy immediately following that loss, the risk of miscarriage is 13 to 17 percent. But the risk to a third pregnancy after two successive losses nearly triples to 38 percent. Many doctors do not begin testing for the cause of pregnancy loss until after three successive miscarriages. However, because the risk of loss to a third pregnancy after two successive miscarriages is so high, the American College of Obstetrics and Gynecologists (ACOG) now recommends testing after a second loss-especially for women over the age of 35.

    Premature ovarian failure
    The term premature menopause (premature ovarian failure) is defined as the occurrence of menopause before the age 40. Premature menopause does not usually occur until a woman has reached her late forties or early fifties. Premature ovarian failure can present at varying ages and in many forms. If premature menopause happens before puberty, the woman will present with lack of sexual developments and her periods will not started. If the condition presents after puberty, the woman will suffer from amenorrhoea and infertility. The diagnosis of premature menopause can be a shattering experience to the women and her family. At present, the only chance of a pregnancy for women wih premature menopause lies through egg donation . It may be possible in the future to restore ovarian function in some women with a positive family history through the transplantation of their ovarian tissue , which had been frozen years before thus allowing natural conception or IVF Embryo donation is another option if the male partner is also infertile.
