Q : Can you give a brief description of intermittent explosive disorder? When is uncontrolled anger considered an illness? A : Intermittent explosive disorder (IED) is a rare impulse control disorder. It's characterized by repeated episodes of aggressive, violent behavior. IED occurs more often in men than in women. Signs and symptoms usually appear in young adulthood.
In IED, the aggressive behavior during an episode is greatly out of proportion to the event or trigger leading up to it. A person with IED may describe aggressive episodes as "spells" or "attacks." Later, the person may be upset or feel remorse, regret or embarrassment about the aggression.
No one test can confirm a diagnosis of IED. A doctor may diagnose IED based on:
  • Signs and symptoms
  • Psychiatric or psychological evaluation
Treatment of IED may include:
  • Psychotherapy
  • Medication
Not all aggressive behavior is due to IED. Some common causes of aggression not related to IED include:
  • Use of alcohol or drugs, especially stimulants such as phencyclidine (PCP) and cocaine, barbiturates, and inhalants

Definition
Intermittent explosive disorder (IED) is a mental disturbance that is characterized by specific episodes of violent and aggressive behavior that may involve harm to others or destruction of property. Usually, these episodes follow minor incidents and are out of proportion to the trigger.
Description
The DSM-IV decribes intermittent explosive disorder as one of several impulse-control disorders, including kleptomania (impulsive stealing), pathological gambling, and pyromania (setting fires). There must be several instances of failure to resist aggressive or violent behaviors that result in harm to others or destruction of property. Spurred by a minor incident, these acts are grossly out of proportion to the stressor. To be IED, these behaviors are not caused by another mental disorder (e.g. antisocial personality disorder, bipolar disorder , borderline personality disorder, or attention-deficit/hyperactivity disorder ). I have been treated in about every way imaginable for intermitent explosive disorder, bi-polar, depression, and anger. Neurontin seem to work the best after everything from Lithium to Zoloft seemed to build tolerance or resistence. I have been trying to follow the use of testoterone therapy out of some clinical trials at UCLA, and would be very interested in participating in one of these. Trying to find someone in Omaha, NE is proving to be very difficult. Is there such a thing as long distance trials? I am a male 43yo and have been living with this illness since puberty. Anything to help point me in a right direction would be appreciated.

Intermittent Explosive Disorder
This disorder is commonly seen in children of divorce and remarriage. Other causes are quite prevalent, however. Among other causes to look for are, strict discipline, emotional distance on the part of the parent, and the “push-pull” characterization, where the parent effectively says, “I love you, go away.”
To compensate for these mistreatments, the child will stuff his or her feelings until they cannot cope any further, then explode by assaulting individuals or destroying property or both. This disturbance may be preceded by a triggering mechanism or may seem to occur spontaneously.
This self-test is designed to alert individuals to the possibility that they may want to seek professional help in dealing with trauma, past or present. Please keep in mind that this is not an official, standardized or approved test for intermittent explosive disorder. These are simply symptoms seen in those who have come to counseling and have subsequently been diagnosed as having this disorder. If your child has some of these symptoms, you will want to contact a professional for further testing.

Q. I am a graduate student researching the history of Impulse Disorders, specifically Intermittent Explosive Disorder. I need to find out when Intermittent Explosive Disorder became a diagnosable illness. Was it in the DSM-III? When did it first appear as a bonafide mental illness? A. As you know, Intermittent Explosive Disorder (IED) is now classified in DSM-IV among the "Impulse Control Disorders Not Elsewhere Classified". This has remained essentially unchanged since DSM-III, which was published in 1980. Prior to thatin DSM-IIthe nearest thing to IED was so-called Explosive Personality, which was regarded as a personality disorder. In DSM-III, and ever since, it was recognized that some individuals have bouts of explosive behavior that are NOT part of their underlying character structure, and, indeed, are experienced as ego-alien or ego-dystonic. But, when you describe it as a bonafide mental illness, you may be going a bit further than our understanding permits.

Current Psychiatry Reports Abstract Intermittent explosive disorder (IED) may best be thought of as a categoric expression of recurrent, problematic impulsive aggressive behavior. Although diagnostic criteria issues have made systematic research in IED difficult, recent work with new research criteria may allow for important empiric work to take place. Given that previous research in the area of impulsive aggression has been highly informative concerning the genetics, biology, and pharmacologic treatment of this behavior, application and extension of this work to IED is crucial. This paper reviews several important aspects of IED including its history in the Diagnostic and Statistical Manual of Mental Disorders, the formation of new research criteria, and the pheno-menologic, epidemiologic, genetic, biologic, and treatment correlates of this disorder. The reaction is out of proportion to the event that triggers or provokes the outburst. What is going on in the body? The exact cause of intermittent explosive disorder is not known. Some evidence suggests there may be a link between this disorder and mild neurological problems similar to those associated with some learning disabilities What are the signs and symptoms of the condition? A person who has intermittent explosive disorder: destroys property has repeated acts of sudden verbal or physical aggression is unable to stop or control the impulsive, aggressive actions A person who has this disorder is not violent or aggressive most of the time. The outbreaks of severe rage and anger are usually isolated. The amount of aggression is out of proportion with the incident that triggered the outburst. What are the causes and risks of the condition? The cause of intermittent explosive disorder is not known. It may be caused by an abnormality in the nervous system. Things that increase a person's risk for the disorder include the following:
