THE MOLECULAR NEUROPATHOLOGY OF MUSCULAR DYSTROPHIES Rashna S Dastur, Daya K Manghani Muscular dystrophy covers a diverse group of inherited disorders in which the main clinical defects in the skeletal muscle i.e. weakness and wasting are mainly similar in all patients, but show genetically different diseases. This suggests that the various gene products participate or work together as a common functional structure or "biochemical cascade". In striated muscle cells, dystrophin - glycoprotein complex (DGC) serves as a link between the extracellular matrix and the subsarcolemmal cytoskeleton. The absence or dysfunction of one gene product (protein) in the DGC affects the functioning of the others causing the muscle cell to breakdown. Variability of clinical severity and the age of onset found in different types of dystrophies therefore depend on the functional importance of their gene products to the complex. Cohen’s technique for reimplantation was used in 40 patients (73 ureters), Leadbetter-Politano Technique in 31 patients (41 ureters), Lich-Gregoir Technique in 11 patients (15 ureters), and Paquin’s Technique in 9 patients (11 ureters). Ureteric tailoring was performed in 27 ureters while ureteric plication was done in 11 ureters. Ureteric stents were placed post-operatively in 94 ureters.

CASE REPORT A 46-year-old woman experienced an abrupt onset of severe occipito-cervical and upper back pain while eating dinner. The pain aggravated while standing and alleviated while lying flat. Tracing her medical history, she had bronchiectasis with regular treatment for 6 years, but vigorous cough with blood-tinged sputum occurred unremittingly. DISCUSSION In conclusion, there have been only few reports on SIH that mentioned the associated spinal MR findings as well as the epidural fluid enhancement.(7,9) However, the enhancement of the epidural fluid was usually attributed to fenestrated neovascularization provoked by an inflammatory component, such as blood, of the fluid collection. In our case, we presented another cause for the enhancement - the contrast extravasation from a torn meningeal vessel.

Norman L. Foster, M.D. Senior Research Scientist, IoG Professor of Neurology, Medical School Norman Foster studies the clinical features and pathophysiology of Alzheimer's disease, frontotemporal dementia and related neurodegenerative disorders. Using positron emission tomography (PET), he has found that these age-related illnesses exhibit different patterns of selective neuronal vulnerability that correspond to symptoms that patients experience. In Alzheimer's disease for example, there is a decline in cerebral glucose metabolism particularly in the posterior temporoparietal cortex with relative sparing of the primary motor and sensory cortex. This accounts for the development of profound memory, language and visuospatial deficits at a time when patients are without any physical disability. His recent work with investigators in the Division of Nuclear Medicine has found that the posterior cingulate gyrus is affected early in Alzheimer's disease which may aid in diagnosis. He and his collaborators have examined cholinergic and dopaminergic PET markers in dementia and studied the effects of drug treatment on measures of cholinesterase in the brain. He is also interested in clinicopathological correlation in genetic forms of dementia. Foster is the principal investigator at the University of Michigan for the Alzheimer’s Disease Cooperative Study that evaluates drug treatments for dementia. He is also evaluating the measurement of cerebral glucose metabolism as a biomarker for frontotemporal dementia.

Chronic Subdural Haematoma, A Stroke Mimic Condition MUMTAZ ALI, SHAHID AYUB, TARIQ KHAN Department of Neurosurgery, Postgraduate Medical Institute Hayatabad Medical Complex, Peshawar ABSTRACT: The objective of this study was to evaluate the surgical outcome of two burr holes craniostomy without drainage systems in our setup . This was a Prospective study performed at the department of Neurosurgery Hayatabad Medical Complex, Peshawar from December 1997 to October 2001.Eighty six patients of chronic subdural haematoma were operated with two burrhole craniostomies without drainage system. Majority of these patients presented to the medical unit Hayatabad Medical Complex, Peshawar and Khyber Teaching Hospital Peshawar with stroke like condition . These patients were referred in emergency to our department. Rest of cases was admitted through casuality and from private clinics. Detailed history and examination along with CT findings were documented on arrival. All these patients were operated in emergency. There were 54(63.1%) male and 32 (27%) female with a ratio of 1.5:1. The age ranged from 1-80 years with an average of 56 years. At the time of admission 53(61.8%) patients were having GCS from 3-8, 26 (20.2%) patients were having GCS from 9-12 and only 7 (8.1%) patients were having GCS from 13-15. Predisposing factors in these patients were head injury in 69 (80.2%) cases, brain atrophy and ventricular peritoneal shunt in 5 (5.8%) cases each, bleeding disorder, anticoagulant therapy and lumbar puncture in one (1.2%) case

Closure of the Air-Bone gap in 20 dB was 98% resp. 90% in the two groups with a more pronounced high tone hearing loss in the Erbium-Yag-laser treated group.
