As a teacher of persons with visual impairment, I have recently received a request to provide an assessment for functional vision for an 8th grade student who experiences Juvenile Rheumatoid Arthritis and associated Iritis. I have found little information describing the possible visual implications regarding Iritis and need additional information. A majority of the staff which work with this student feel she is faking it. Can you provide me with information and possible resources which will help enlighten these individuals? Next Article

iritis. iritis can occur at any age. It is rare in childhood. Children withrheumatoid arthritis may have iritis without having its usual symptoms.

IRITIS
Iritis can occur at any age. It is rare in childhood. Children with rheumatoid arthritis may have iritis without having its usual symptoms. Therefore their eyes are checked regularly. Iritis causes usually pain and redness of the eye, the eye may be very light sensitive, too. As one looks at near with the inflamed eye, it may hurt badly. When we look at near distance the small muscles of the iris contract. Contraction of the inflammed muscle causes the pain. Iritis should be treated as soon as possible. It is best to go to the emergency clinic or to the eye doctor's office if you can go there without delay. It is important not to make an appointment which usually means several weeks of waiting. The treatment of iritis is based on rest and steroids , cortison-like medications. Rest is instituted by using dilatation of the pupil , then the small muscles cannot contract. The eye may become quite comfortable in a few days but the treatment is continued for weeks and often months until the interior of the eye looks normal. If the treatment is discontinued too early, the symptoms may reappear. Iritis tends to be reactivated by viral infections now and then. If one has had the symptoms once, they are easily recognized the next time. The treatment is always the same.

Wednesday, December 04, 2002 Iritis ... as I write this. I've been diagnosed with a painful condition of the eye, called iritis. I thought it was "pinkeye," ostensibly the result of a cold virus I had been fighting but it's much worse. Because my eyes erupt with pain while looking at bright light sources such as computer screens I'm going to take some quality time offline. I do notice one thing now, as a result of this sudden malady: websites designed with black text on white backgrounds are exceedingly hard on these (relatively young) eyes. I know most people use the black-on-white look because it's the closest thing to ink on paper, but maybe that's not such a bright idea. A computer screen isn't paper, Sherlock. Do us all a favor, you black-on-whites, find some softer tones for your site background. Think about it: a white background plus a computer screen: you might as well stare directly into a lightbulb. Anyway, I hope to be back this weekend, or next week sometime. There are plenty of great weblogs in by blogroll scroll down, on the bottom right.

WHAT YOU SHOULD KNOW
Iritis (eye-RYE-tis) is an irritation of the iris in your eye. It may appear suddenly without warning or may develop over a period of time. The problem usually affects only one eye and, with treatment, should clear up in a week or two.
Causes
Iritis usually starts as an infection in another part of the body that moves to the eye. It sometimes occurs as a result of a disease or an eye injury. Often the cause is unknown.
Signs/Symptoms
Typical symptoms include eye pain, redness, and blurred vision. Your eyes may tear more than usual, and you may develop an increased sensitivity to bright light.
Care
Your doctor may prescribe medicine to relieve the pain and irritation and treat any infection.
WHAT YOU SHOULD DO
  • To ease the pain, apply a clean, warm or cool washcloth to your eye several times a day for 10 to 20 minutes.

Sieben Jahre nach der Erstdiagnose " Morbus Bechterew " ereilte mich meine erste Iritis - völlig unerwartet, aus heiterem Himmel, also ziemlich genau nach Schulbuch. Über Stress, einem oft erwähnten Auslöser, konnte ich mich nach einer Woche Ferien nicht beklagen. Eines Samstags im Oktober begann ich um etwa 15 Uhr zu bemerken, dass ich auf dem rechten Auge unscharf sehe - das hatte ich zwar noch nie, dachte mir aber nichts Böses dabei.
Der junge Assistenzarzt sagte zwar etwas von "vielen Zellen", aber als Laie, der die Krankheit zum ersten Mal hat, kann man sich natürlich kaum etwas darunter vorstellen. Da kann das Web helfen: Gute bebilderte Beschreibungen einer Iritis (auf Englisch) finden sich zum Beispiel hier oder hier bzw. (in Deutsch, speziell beim Morbus Bechterew)

Presentation
What to do:
  • Perform a complete eye exam, including topical anesthesia if necessary; visual acuity, pupillary reflexes, funduscopy, slit lamp examination of the anterior chamber (including pinhole illumination to bring out cells and flare) and fluorescein staining to detect any corneal lesion.
  • Attempt to ascertain the cause of the iritis (is it generalized from a corneal insult or conjunctivitis, a late sequela of blunt trauma, infectious, or autoimmune?)
  • Explain to the patient the potential severity of the problem: this is no routine conjunctivitis, but a process which can develop into blindness.
  • Arrange for ophthalmologic consultation or followup, and, if acceptable to the consulting ophthalmologist . . .
  • Dilate the pupil and paralyze ciliary accommodation with 1% cyclopentolate (Cyclogyl) drops once, which will not only relieve the pain of the muscle spasm, but will keep the iris away from the lens, where meiosis and inflammation might cause adhesions (posterior synechiae). For a prolonged effect, instill 1 drop of homatropine 5% before discharge.
  • Suppress the inflammation with topical steroids, like 1% prednisolone (Inflamase) drops once;

Iritis Inflammation of the iris of one or both eyes; if acute, may give rise to Glaucoma , either because white cells produced in response to inflammation build up and block channel which drains fluid from front chamber of eye, or because iris sticks to front of lens, which also prevents drainage of fluid; if long-standing, may give rise to Cataract . Because of the risk of glaucoma, an eye specialist should be seen as soon as possible; steroid drops may be necessary to reduce inflammation and prevent iris sticking to lens, but they do not get to the root of the problem. Iritis tends to be a recurrent constitutional problem, so constitutional treatment from an expert homeopath should be sought. Iritis is a form of uveitis, specifically anterior uveitis. a wonderful site for informatiion on the treatment of uveitis can be found at Harvard University Medical School and MEEI the clinc associated with both the School and MGH. http://www.uveitis.org
the above link also has a wonderful support group with individuals who are also in Arizona. I don't know of an Uveitis specialist offhand in Arizona however but I do know that an excellent ocular immunologist/uveitis specialist will be In Salt Lake City in the near future with whom you can consult.
