According to Reuter's Press, Los Angeles (2001, December 13), "Irritable Bowel Syndrome (is) a chronic condition believed to plague 20% of the adult (US) population".
IBS is a very common disorder that affects the functioning of the bowel. Although the severity of the condition (also occasionally referred to as spastic colon ) can vary considerably from one patient to another, typical characteristics include changes in bowel habits ranging from diarrhea to constipation, cramp-like pains, bloating and a feeling of "gassiness."
For some sufferers, IBS is little more than a minor inconvenience; for others it can be totally disabling. In extreme cases, the patient is unable to travel, function in the workplace or enjoy socializing.
What It is Not: Some Common Misconceptions

Irritable bowel syndrome, also known as spastic colon, is a digestive system disorder that presents as abdominal pain, bloating, cramping, excess mucus in stools, diarrhea, and constipation. It occurs mainly in the daytime and not during sleep, although in some patients the need to defecate may awaken them early in the morning. Irritable bowel syndrome usually begins in the first three decades of life and may affect up to one-quarter of the population in industrialized countries. The exact etiology of irritable bowel syndrome is unknown, but stress, depression, hypersensitivity to certain hormones, and pressure from gas, diet, and medications may be contributing factors. The syndrome is characterized by variable changes in the frequency and strength of muscular contractions in the intestines, which are responsible for moving digested food and, ultimately, feces through the gastrointestinal tract. Changes in frequency and contraction, and a patient’s sensitivity to them, result in the classic symptoms of pain, diarrhea, or constipation. In most patients, irritable bowel syndrome shows a tendency toward diarrhea, constipation, or an alternation between the two. Diagnosis usually is made after these symptoms have occurred for at least 3 months. Patients with these symptoms should have a complete examination to rule out diseases with similar symptoms, such as bacterial overgrowth, endometriosis, lactose intolerance, and celiac disease.

  • General Features
  • History . Patients often describe a long history of chronic or intermittent diarrhea, which usually starts before age 50 and is exacerbated by anxiety or stress. Diarrhea is often worse in the morning and after meals. These patients often complain of a sensation of incomplete evacuation, distention, passage of mucus, or associated abdominal, pelvic, and back pain. Pain is relieved by evacuation. Alternating constipation, diarrhea, and bloating is typical. Diarrhea is not bloody unless accompanied by an anorectal lesion such as hemorrhoids or a fissure. Systemic symptoms or weight loss are not present. Nutritional status is not compromised. Proposed criteria for diagnosis of IBS are found in Box 5-1 Etiology . The following factors are believed to play a mechanistic role in IBS and more than one may operate in a given individual: (1) abnormal motility, (2) abnormal visceral perception, (3) psychologic factors, (4) luminal compounds (lactose, short chain fatty acids, food allergens, and bile acids), which may irritate the bowel.

    What Are the Cures for IBS?
    Of all intestinal disorders diagnosed by physicians throughout the United States, Irritable Bowel Syndrome is by far the most common. Many of those afflicted, however, remain reluctant to seek help from their physician, in the mistaken belief that nothing can be done about it. Wrong! This most prevalent of intestinal disorders is controllable.
    Although no known cures for IBS exist thus far, various treatments can control this distressing functional disorder of the bowel. No single specific treatment, however, has been established for managing all the various symptoms of IBS.
    What Happens After the Condition is Diagnosed?
    First, the physician discusses the severity of the condition with the patient. Individual and multiple symptoms are noted, as well as their intensity and frequency. When all factors and possibilities have been explored, the physician draws up an appropriate treatment plan.
    During the first meeting with the physician, the IBS patient must provide information about lifestyle and habits. The patient may also be asked to keep a diary of potential "triggers" and a record of food eaten.
    Above all, the physician will dispel any misgivings that the patient may have about seeking help. The unfortunate myth that Irritable Bowel Syndrome is "all in the mind" is one of the most frequent reasons given by patients for delaying or failing to obtain treatment. Your physician can and will reassure you that IBS is one of the most common intestinal disorders seen in gastroenterology today and, more importantly, that it
