Bethan Lang and Angela Vincent discuss these findings and their potential relevance to other areas of medicine. M yasthenia gravis (MG) is an autoimmune disease in which antibodies to nicotinic acetylcholine receptors (AChR) cause loss of function and muscle weakness. The antibodies mainly act via complement-mediated lysis and increased AChR degradation. However, recent studies suggest that antibodies can affect ion channel functions in different ways; for instance, in some patients with myasthenia gravis, antibodies do not bind directly to the AChR, but appear to reduce AChR function via perturbing signalling pathways. Moreover, maternal antibodies that specifically inhibit the function of fetal AChR can lead to fetal paralysis and severe developmental abnormalities, even when the mother is asymptomatic. By contrast, in Lambert-Eaton myasthenic syndrome (LEMS), antibodies mainly cause down-regulation of voltage-gated calcium channels, and new data shows that these antibodies bind to the
