Arthritis means "joint inflammation" and refers to a group of diseases that cause pain, swelling, stiffness, and loss of motion in the joints. "Arthritis" is often used as a more general term to refer to the more than 100 rheumatic diseases that may affect the joints but can also cause pain, swelling, and stiffness in other supporting structures of the body such as muscles, tendons, ligaments, and bones. Some rheumatic diseases can affect other parts of the body, including various internal organs. JUVENILE RHEUMATOID ARTHRITIS (Systemic Onset) Juvenile rheumatoid arthritis is a chronic inflammatory disease that always involves the joints, but may produce extensive connective tissue and visceral lesions. Despite extensive research the etiology of JRA remains unclear. JRA is arbitrarily defined as beginning before the age of 16 years. Approximately 20% have acute systemic JRA which is characterized by febrile onset, variable joint manifestations, rash, generalized lymphadenopathy, splenomegaly, liver disease, and rarely by GI tract disease. Other children with JRA primarily have arthritis that is present at the onset of illness. In 30% only a single joint, usually the knee, is involved; in 50% multiple joints are involved. The disease is termed polyarticular if more than 4 joints are involved and pauciarticular if 4 or fewer joints are affected. The acute systemic onset of JRA is often referred to as Still's disease because of Still's classic description of the disease in 1897. The systemic symptoms of JRA may precede the onset of overt arthritis for a variable periods of time ranging from months to years. The most characteristic features of systemic JRA are a high-spiking fever and the rheumatoid rash. The temperature peaks once or twice daily, often in the late afternoon or evening, to a level of 39

    What Is It? If your question is not answered here, please contact one of our information specialists. JRA is the most common form of arthritis in children. It may be a mild condition that causes few problems over time, but it can be much more persistent and cause joint and tissue damage in other children. JRA can produce serious complications in more severe cases. Arthritis is best described by four major changes in the joints that may develop. The most common features of JRA are: joint inflammation, joint contracture (stiff, bent joint), joint damage and/or alteration or change in growth. Other symptoms include joint stiffness following rest or decreased activity level (also referred to morning stiffness or gelling), and weakness in muscles and other soft tissues around involved joints. However, because JRA affects each child differently, your child may not experience all of these changes. Children also vary in the degree to which they are affected by any particular symptom.

    about Juvenile Rheumatoid Arthritis~ Our daughter Anna was diagnosed with a mild case of JRA in July 1999. Anna has pauci-articular JRA, which means that she has four or fewer joints affected. For Anna, the JRA is limited to her left knee.
    As Anna's Mom, I have gone through all kinds of emotions since she was diagnosed (even with a mild case!). I have spent hours at the computer researching every page I could find with some sort of information on JRA. That's why I wanted to do this page. I want to include many links to the varied sites available to make research less time-consuming for other parents and families.
    For any parent whose child has been diagnosed with a chronic illness, I think it is crucial for you to surround yourself with friends and family who will listen to you and encourage you and keep you from feeling totally overwhelmed by everything. For me, it was my quest for information which helped me feel a little more in control, and my friends and family who helped me to regain my emotional balance. I found a message board where other JRA moms (and a few dads occasionally) talk and ask questions and share information and give advice. It has been an additional great system of support, hope, and encouragement.

If you know a child who has Juvenile Rheumatoid Arthritis (JRA) or if you want to learn more about this condition, you may find it interesting to read about our personal experience. I'm not a medical professional ... and I don't play one on the web ... but I felt that others might benefit from the sort of information that I can share. You can also get information here about a popular email-based support and information group for people involved with Juvenile Arthritis, called The JRA List
    At the very least, I hope that my collection of related internet links may be useful to you in helping to sort through the millions of arthritis-related websites in existence, while you search for more information and the answers to your questions about JRA. One of the main reasons for putting together this website is my personal commitment to helping spread awareness of Juvenile Rheumatoid Arthritis (JRA). Until 1995, I didn't even know what those initials stood for. Unfortunately, most people still don't. Despite increasing numbers of cases, JRA has still not received a lot of public recognition. For many, it remains a more personal struggle. Hopefully, this will change. When there's greater public awareness of a significant health problem it's more likely there will be funding made available for its' study. With continued research comes the development of new, safer, and more effective medications. It is my hope that one day ... in the not so distant future ... a cure will be discovered. Possibly even a vaccine that will help prevent JRA in future generations of children.

    Juvenile Rheumatoid Arthritis
    Arthritis beginning at or before age 16. Juvenile rheumatoid arthritis (JRA) is similar to adult RA (see Ch. 50 ). The disease tends to affect large and small joints and may interfere with growth and development. Micrognathia (receded chin) due to impaired mandible growth may occur.
    Symptoms and Signs
    Systemic onset (Still's disease) occurs in about 20% of patients. High fever, rash, splenomegaly, generalized adenopathy, serositis, and a striking neutrophilic leukocytosis and thrombocytosis occur often. At times these manifestations precede the appearance of the arthritis. Rheumatoid factor and antinuclear antibodies are absent. Pauciarticular onset occurs in about 40% of patients, usually young girls. Antinuclear antibodies are present in up to 75% of affected children. Chronic iridocyclitis occurs in nearly 20% of these patients; it is often asymptomatic and is detected only with periodic slit-lamp examinations. A subgroup of affected older boys includes many who have HLA-B27 antigen. Most of these boys subsequently develop classic features of one of the seronegative spondyloarthropathies: ankylosing spondylitis, inflammatory bowel disease, psoriatic arthropathy, or Reiter's syndrome.

    Ness Motley filed in August one of the first cases against the manufacture of the biological product, Enbrel (etanercept). Enbrel (etanercept) was approved by the U.S. Food and Drug Administration on November 2, 1998, as a second-line agent for the treatment of juvenile rheumatoid arthritis. At the time Enbrel was co-marketed by Immunex, Corp. (in 2001 Amgen, Inc. bought Immunex Crop. of Seattle and Enbrel for some $16 billion, the largest biotech buyout ever) and Wyeth-Ayerst Pharmaceuticals. Although Enbrel has been touted in the press and through marketing as a wonder "drug", it is a potent product whose long term effects and mechanism of action are not known or fully understood. The adverse effects that are known are serious and fatal. Patients have neither been warned adequately of these risks nor their severity.

    My Story
    Hi my name is Danielle!
    I am 9 years old and I have had systemic onset juvenile rhuematoid arthritis since I was 3years old!!
    I had a state of remission when I was 6, it lasted for almost 3 years and returned again in Nov. 1999.
    I have had a hard time with the disease this time, during my remission I did anything I wanted to do and now I have a hard time doing anything. I love to play soccer but it was difficult for me this spring, I never played with active arthritis until then, and it was painful sometimes especially at the end of the day.
    I am going into the 4th grade this year, I like school, I have to miss alot when my disease is really active which was alot this year...
    I have a sister and 2 brothers, and we fight like all siblings do, but we have fun together also.
    I am interested in meeting others like me or anyone who wants to chat about or learn about arthritis so drop me a line!!!!!

    • Juvenile rheumatoid arthritis is also known as JRA. It is arthritis that affects children age 16 or younger. In JRA, the body's immune system starts to attack instead of protect the body's tissues. The attack can cause redness, swelling, and pain in the joints. There are three main types of JRA. They include pauciarticular, polyarticular, and systemic. Ask your doctor.
    What causes it?
    • The cause is not known. It may be due to both genetic factors (from parents) and environmental factors (for example, from infections).
    Who can get it?
    • Children age 16 and under can get JRA. It is more common in females.
    What are the signs and symptoms?
    Symptoms of JRA usually show up around the age of 4 or 5. To be diagnosed for JRA, symptoms must last at least 6 weeks. They may include:

    PAUCIARTICULAR JUVENILE RHEUMATOID ARTHRITIS Juvenile rheumatoid arthritis is a chronic inflammatory disease that always involves the joints, but may produce extensive connective tissue and visceral lesions. Despite extensive research the etiology of JRA remains unclear. No autoantibody specific for the disease has been documented. Much evidence points to a major role for immunologic reactivity in perpetuating rheumatoid inflammation. Unidentified initiating factors probably stimulate a synovitis that triggers the local synthesis if IgG and rheumatoid factor (IgM anti- IgG) by plasma cells within the joint. Activation of the complement system within the joint space generates several materials that attract PMN's into the joint cavity, where they phagocytize the immune complex, thus leading to the formation of the rheumatoid arthritis cells. The neutrophils, following phagocytosis, are stimulated to discharge hydrolase from lysosomal granules. These enzymes together with fibrinogen may initiate the destructive changes characteristic of rheumatoid joint disease. JRA is arbitrarily defined as beginning before the age of 16 years. It is slightly more common in females than in males and is said to affect as many as 250,000 children in the United States. Although JRA may begin at any age, an increased incidence of onset generally occurs from 1- 3 years of age and later from 8- 12 years of age. Approximately 20% have acute systemic JRA which is characterized by febrile onset, variable joint manifestations, rash, generalized lymphadenopathy, splenomegaly, liver disease, and rarely by GI tract disease.
