The stretching of the cornea tends to progress but the rate varies. Sometimes one eye may be badly affected while the other eye may show very little sign of the condition. The cause of keratoconus is unknown. It may be associated with allergy or could have a genetic component. Keratoconus rarely appears in an individual until puberty or beyond. Although no one can be sure how far keratoconus will develop in an individual, the condition does not cause blindness. With the current treatment available, most people should be able to lead a normal lifestyle despite this condition. What is Keratoconus?
Keratoconus is a disorder that causes progressive thinning and distortion of the cornea - the clear dome at the front of the eye. The cornea normally has a rounded shape, but in keratoconus the thinned area bulges forward to produce a cone like protrusion. This results in progressive distortion and reduced vision. Many keratoconus patients experience blurred distance vision, glare, light sensitivity, disturbed night vision, headaches and reading problems, Keratoconus may affect only one eye but more commonly occurs in both eyes. It affects people of all races and both sexes. Most patients develop keratoconus in their late teens to early twenties although it can begin at any age. Pellucid marginal degeneration (PMD) is a corneal thinning disorder related to keratoconus that primarily affects the inferior cornea.

Keratoconus "Keratoconus is a progressive eye condition that sometimes is difficult to diagnose," says Michael Ward, FCLSA, director of Emory Eye Center's contact lens service. "Keratoconus usually affects both eyes and requires careful contact lens management." Typically appearing during the teens to early 20s, keratoconus causes the cornea to thin and form scar tissue, until the eye is cone shaped. Since the cornea is responsible for most of the eyes' refraction, any disruption can cause considerable visual distortion that may not be corrected with spectacles. Individuals with keratoconus require specially designed contact lenses to
manage vision. Common misconception: That contact lenses halt the progress of keratoconus. In fact, the lenses simply improve vision while the condition is developing. Last Revision Date: April 2002 Keratoconus is an irregular protrusion of the cornea, the clear surface over the colored part of the eye. The cornea acts like the windowpane to the eye. If this windowpane is not smooth, the light will not bend evenly and an irregular image will be formed. Like looking through a bumpy piece of glass. During the early stages of this disease, vision may still be correctable to 20/20 with glasses. The only clue to a keratoconus diagnosis may be from corneal measurements or corneal map. A topographical map of the cornea will show the high and low spots on the cornea, much like a topographical map of the earth will show the mountains and oceans. As this disease progresses, vision will no longer be correctable with glasses. Iron will deposit around the base of the protrusion. The tissue will thin and a scar may develop. In severe cases, a temporary painful condition called acute hydrops may occur when water rushes into the thinned area of the cornea. This will lead to severe scarring. About one in 2000 people will develop keratoconus. Most people will have a mild or moderate form of the disease. Only 10% of keratoconics will develop the most severe form. It typically is diagnosed in the late teens or twenties. However, many people have been diagnosed in their mid to late thirties; this is usually a more mild form of the disease. One eye will precede the other and they will progress at uneven rates. The eyes may go for long periods of time without any change and then may change dramatically over a period of months.

Purpose

Background
Keratoconus is a bilateral, asymmetric, chronic, progressive ectasia of the cornea characterized by steepening and distortion of the cornea, thinning of the apical cornea, corneal scarring, and treatment-related sequelae, such as abrasions from contact lenses and surgical complications. Patients experience distorted vision that worsens with disease progression. Their vision is typically corrected with spectacles early in the disease and, later, with rigid contact lenses. Some patients eventually undergo corneal transplantation in one or both eyes. Keratoconus affects people in their prime earning years and profoundly affects their lives.
Previous large-scale studies of keratoconus have focused on incidence and prevalence, etiologies, or the clinical management of keratoconus. Few have characterized the course of the disease and risk factors for its progression in large samples of keratoconus patients. The incidence of vision-threatening corneal scarring in keratoconus is unknown. Patients’ most frequent questionshow rapidly their keratoconus will progress, how bad their vision will become, whether they will need corneal surgery, how successful their contact lenses will becannot be answered on the basis of the current body of knowledge.

What Is Keratoconus?
Keratoconus is a progressive non-inflammatory disorder that causes a characteristic thinning and cone-like steepening of the cornea. This steepening results in distortion of vision, increased sensitivity to glare and light and an associated reduction in visual acuity. These symptoms usually appear in the late teens and early twenties. Keratoconus may progress for 10-20 years and then can slow or even stabilize. Each eye can be affected differently. This can result in a dramatic decrease in the ability to see clearly even with corrective lenses. Eyeglasses or soft contact lenses may be used to correct the mild nearsightedness and astigmatism in the early stages of keratoconus. As the disorder progresses and the cornea continues to thin and change shape, rigid gas permeable (RGP) contact lenses can be prescribed. The contact lenses must be carefully fitted, and frequent checkups and lens changes may be needed to achieve and maintain adequate vision.

What is Keratoconus?
The cornea is the clear window at the front of the eye. Keratoconus is a condition where the cornea develops a distorted shape producing blurred vision. The eye is normally shaped round like a basketball. With nearsightedness, the cornea is too large and with farsightedness, the eye is too short. With astigmatism, the eye is shaped more like a football rather than a basketball. In other words, it is curved more in one direction than in the direction 90 degrees away. This "football shaped" cornea is what produces the distortion with astigmatism. Associated with this change in corneal shape, is a thinning of the central part of the cornea which eventually produces a "bulge" in the center of the cornea (see photo below). Thus, the name "keratoconus". "Kerato" means cornea and "conus" means cone shaped. The conical shape produces distortion of vision. The football shape analogy produces "regular" astigmatism. This is where the "football" or cornea is shaped differently but is a smooth curvature. In keratoconus, the cornea develops "irregular" astigmatism. This is where each direction of the football shaped cornea has a non-smooth surface much like the peaks and valleys of a mountain range. This produces severe distortion of vision.
