Diagnosis Your physician will conduct a thorough physical exam and review your medical history. The doctor may also look inside the larynx using indirect laryngoscopy or direct laryngoscopy. Indirect larynogoscopy enables the doctor to look down the throat with a small, long mirror to check for abnormal areas and to see whether the vocal cords vibrate as they should. Direct larynogoscopy allows a doctor to insert a lighted tube through the patient's nose or mouth to look at areas that cannot be seen with a mirror. If the physician notices abnormal areas, he or she may recommend that you have a biopsy to remove tissue samples and study them under a microscope. The physician may also recommend X-rays, a CT scan or MRI scan. A CT enables many X-rays to be taken at once, and a computer puts them together to create a detailed picture of the inside of the body. It is more common in men, although the number of women developing cancer of the larynx is increasing. Cancer of the larynx usually occurs between the ages of 50 and 75 years (check the Canadian Cancer Society web site for the most up to date statistics ). As with other forms of cancer, the cause of larynx cancer is not yet clear, but smoking and heavy use of alcohol are felt to be major contributing factors. Research continues on many fronts to find the key to why cells in various organs and tissues of the body begin a pattern of uncontrolled growth (i.e. become cancerous). Early detection and treatment are extremely important for the current forms of treatment to be most effective. Where is the larynx located? The larynx, or voice box, is a tube that connects the back of the throat to the wind-pipe. A flap of cartilage (epiglottis) at the top of the larynx prevents food and liquids from entering the larynx and wind-pipe. Sound is produced when air from the lungs passes through the vocal cords (glottis) and causes them to vibrate. The vocal cords are located just below the epiglottis at the top of the larynx. Where does the cancer occur?

Treatment of Throat Cancer
Head and neck cancers makes up about 5 percent of cancers in the United States. The American Cancer Society estimates that about 10,000 new cases of laryngeal cancer will be diagnosed in 2002, and 3,900 people will die of the disease. Throat cancers usually develop around age 60, and men are 10 times more likely to develop them than women. Of the new cases this year, 8,100 will be men. The most common cause of cancer of the larynx and pharynx is smoking cigarettes, cigars or a pipe. Drinking large quantities of alcohol also increases the risk. The combination of drinking and smoking is especially conducive to developing throat cancer. Throat cancer is most often a squamous-cell carcinoma, which usually begins on the mucosal surfaces of the larynx, the pharynx or the mouth. About 10 percent of patients newly diagnosed with larynx cancer will have another cancer in the mouth, esophagus or lung. Another 10 to 20 percent will develop a cancer in one of these organs later. That is why it is important for patients with these cancers to continue regular follow-up examinations and to stop smoking and drinking alcohol. Throat cancers are treated by physicians in the Department of Otorhinolaryngology working with other specialists, such as plastic surgeons, oncologists and radiation therapists, as needed.

Symptoms One of the commonest symptoms of laryngeal cancer is huskiness or hoarseness and therefore any long-term change in one’s voice should be investigated. Other symptoms are a change in the voice-pitch, the feeling of a lump in the throat, coughing, difficulty in breathing or swallowing and sometimes even ear-ache. Diagnosis The diagnosis is readily made by using a laryngeal mirror similar to that used by a dentist. At the same time, the presence of enlarged lymph nodes in the neck is looked for by the doctor. The malignant nature of the growth is confirmed by removing a sample of tissue for a biopsy. Treatment The treatment for a localised laryngeal growth is radiation therapy for which a six weeks’ course is usually given. During the latter half of the course, the patient will probably lose his or her voice, have a very sore throat and will find it difficult to swallow. But the voice returns and swallowing becomes easier within three to six weeks of completion of treatment. The results of radiation therapy for early carcinoma of the larynx are excellent.

Print this Page General Information / Anatomy / Function / Statistics The larynx, or voice box, is a complex anatomic structure located below the oropharynx and above the trachea. Most of it lies just behind the Adam's apple. The hypopharynx lies on each side of the larynx and behind it. It is divided into three distinct and important subsites:
  • The Subglottis is the area between the vocal cords and the top of the windpipe or trachea
Nearly all cancers arise from the lining of the larynx and are squamous cell carcinomas, but they differ widely in their behavior depending on their site of origin. Cancers of the glottis or true vocal cords tend to be low grade tumours which grow slowly and spread late. Cancers of the supraglottic larynx region tend to be higher grade tumours which are often more aggressive with more rapid growth and a tendency to early spread to the lymph nodes. Cancers of the subglottic region are rare, but they also have a tendency for early spread. Early cancer of all sites within the larynx is highly curable.
