Pulmonary hypertension is rare in patients with systemic lupus erythematosus (SLE) although pulmonary involvement occurs in about 50%. Pulmonary hypertension without obvious pulmonary infiltration is even rarer, the mechanism of which is unclear. We report here a case of SLE who presented with severe pulmonary hypertension and cor pulmonale without any evidence of parenchymal lung disease.
Case Report
The patient was a 19-year-old Saudi female who was admitted to King Khalid University Hospital, Riyadh with a two-year history of dyspnea, easy fatigability, palpitations, anorexia, loss of weight, dry cough and episodic attacks of fever every three to four months with night sweats for the last two years. Five days prior to admission she developed pain in various large and small joints with swelling in interphalangeal and right wrist joints and described features of Raynaud's phenomenon. Chest roentgenogram showed cardiomegaly with prominent pulmonary conus. No lung infiltrate was noticed. High resolution computed tomography (CT) scan was not done. Ultrasound of the abdomen revealed an enlarged liver with mild ascites. Pulmonary function test revealed a restrictive pattern with decreased diffusion lung capacity. Ventilation/perfusion scan of the lungs did not reveal any evidence of pulmonary embolism. Cross-sectional echocardiography and Doppler studies revealed significant enlargement of the right atrium and right ventricle with dilated pulmonary artery and moderate degree of pericardial effusion. Moderate tricuspid incompetence was also detected.

WHAT IS LUPUS?
Lupus is a chronic, autoimmune disease which causes inflammation of various parts of the body, especially the skin, joints, blood and kidneys. The body's immune system normally makes proteins called antibodies to protect the body against viruses, bacteria and other foreign materials. These foreign materials are called ANTIGENS. In an autoimmune disorder such as Lupus, the immune system loses its ability to tell the difference between foreign substances (antigens) and its own cells and tissues. The immune system then makes antibodies directed against "self." These antibodies, called "auto-antibodies" react with the "self" antigens to form immune complexes. The immune complexes build up in the tissues and cause inflammation, injury to tissues, and pain.
More people have Lupus than AIDS, cerebral palsy, multiple sclerosis, sickle-cell anemia, and cycsic fibrosis combined. Lupus Foundation of America market research data show that between 1.4 and 2 MILLION people reported to have been diagnosed with Lupus. For most people, Lupus in a mild disease affecting only a few organs, but for others, it can be serious, or even life threatening. For more about what Lupus is, click here:

THE HISTORY OF LUPUS
"Lupus" is Latin for the word "wolf," and it is common medical lore that the "butterfly rash" seen on the cheeks of many Lupus patients is so similar to the markings of a wolf. The medical name for the disease that we know of as Lupus-Lupus Erythmatosus-was first applied to a skin disorder by a Frencham, Pierre Cazenave, in 1851, though descriptive articles detailing the condition date back to Hippocrates in ancient greece

    LUPUS means wolf ERYTHEMATOSUS means redness The name was coined in 1851 because the facial rash looked like the bite of the wolf . Many people call the face rash, the marlar butterfly rash. See for yourself.
    Lupus, the disease, is believed to have been described by Hippocrates around 400 BC. Lupus is not a new disease but has been described more in the last 150 years. Approximately 25 years ago, lupus was considered a terminal disease. Today, lupus can be controlled with proper medical treatment. There is no cure for lupus. Lupus is a chronic, inflammatory disease in which the body's immune system, instead of serving its normal protective function, forms antibodies that attack healthy tissues and organs. For most people, lupus is a mild disease affecting only a few body organs; for others, it may cause serious and even life - threatening problems. The number of Americans diagnosed with lupus is estimated at 500,000. Lupus is more common that muscular dystrophy, multiple sclerosis, leukemia and Addison's Disease. LE usually is divided into 3 groups: Discoid LE, Systemic LE and Drug-induced Lupus.

WHAT IS LUPUS?

The immune system normally protects the body against viruses, bacteria and other foreign materials. In an autoimmune system disorder like lupus, the immune system loses its ability to tell the difference between foreign substances and its own cells and tissues. The immune system then makes antibodies directed against itself. These antibodies, called "auto-antibodies" react with the "self" antigens to form immune complexes. The immune complexes build up in the tissues and can cause inflammation, injury to tissues, and pain. For most people lupus is a mild disease affecting only a few organs. For others, it may cause serous and even life threatening conditions. More than 16,000 Americans develop lupus each year. It is estimated that up to 2,000,000 Americans have been diagnosed with lupus. Lupus can be difficult to diagnose as the symptoms come and go and mimic other illnesses. The symptoms include transient joint and muscle pain, a rash caused by or worsened by sunlight, sores in the nose and mouth, low grade fevers, hair loss, pleurisy, fatigue, appetite loss and/or painful sensitivity of the fingers to cold. While medical science has not yet found a cure for lupus, new research brings unexpected findings and increased hope each year.
