Lymphangioma : Often apparent in the first decade of life, this relatively uncommon tumor consists of large serum filled spaces lined by endothelial cells. The lesions can be located deep within the orbit or in the eyelids. With valsalva or a upper respiratory tract infection, lymphangiomas may increase in size and be more noticeable. However, patients may not be aware of the lesion until there is an abrupt onset of proptosis. This is due to hemorrhage from capillaries into the serum filled spaces, often described as a "chocolate cyst". In younger children, if the lesion is large, strabismus and ambylopia can be a problem. CT scans can vary in presentation. This patient has a mass in the lateral cone that is compressing the globe. Since the lesion is vascular, not encapsulated and often infiltrates around normal orbital structures, complete resection is usually not successful and debulking is only performed in severe cases. Histopathologically, the endothelial-lined spaces have no pericytes or smooth muscle in the in their wall. There are scattered follicles of lymphoid tissue with interstitial capillaries throughout. Aim: To shown the clinical and histopathological features of a cystic mesenteric lymphangioma. Material and methods: A 17 year old male with a past history of 17 days of right lower abdominal pain, palpable mass, fever, nausea and vomitus. The initial clinical diagnosis was appendicitis, during the appendectomy a mesenteric tumor was found. The patient was then sent to a third level cancer center for further diagnosis and treatment. In an abdominal CT scan a multicystic retroperitoneal tumor was found and diagnosed as a cystic pancreatic tumor. At midline exploratory celiotomy a cystic tumor infiltrating the right colon mesenterium, duodenum, proximal jejunum, and pancreas head was found. A pancreatoduodenectomy in bloc with right hemicolectomy was performed. During the postoperative period the patient developed cholestasis due to endovenous parenteral nutritional support that improved when it was suspended. The patient was discharged without complications at the 15 th postoperative day. The histopathologic diagnosis was cystic mesenteric lymphangioma. At 12 months of follow

Patients can present with acute proptosis (bulging eye) after minor head trauma, as a gradual proptosis, or after an upper respiratory infection. The tumor tends to start in the superior and nasal orbital quadrants. More than 50% affect anterior (conjunctival and adnexal) structures. Typically, the tumor bleeds into itself causing cysts of blood (called chocolate-cysts) within the tumor. If the cyst forms behind the eye, it pushes the eye forward. If the tumor forms in the eyelid or structures around the eye "adnexa" blood filled lymphatic channels called, "lymphangiectasias" beneath the conjunctiva. This small lymphangioma can be seen in the closed eyelid.

SOLIS, MD, Macon, GA ABSTRACT: Mesenteric lymphangioma was manifested as a bowel obstruction in a child with cerebral palsy. Diagnosis was not confirmed until the time of laparotomy. Mesenteric lymphangioma is rare but should be considered in a child with bowel obstruction and no other known abdominal disease. Lymphangioma is a benign cystic tumor in infants and children.1 Lymphangioma of the mesentery is rare and often diagnosed incidentally.2 When symptoms occur, they may include pain, nausea, vomiting, palpable mass, peritonitis, and bowel obstruction.2-4 Lymphangioma is often confused with mesenteric cysts both clinically and in the literature, but distinct histologic criteria distinguish the two entities.2,4-6 We report a case of mesenteric lymphangioma causing small bowel obstruction in a child with cerebral palsy. CASE REPORT Fig 1 A diagnosis of severe dehydration and bowel obstruction was made, and the patient was admitted for fluid resuscitation and nasogastric suction. Differential diagnosis included proximal stool impaction, intussusception, midgut volvulus, and internal hernia. Six hours later, the patient's temperature spiked to 103degreesF and the abdomen became acutely tender, which prompted emergency laparotomy. A 7 cm multiloculated cystic mass was found on the mesentery of the jejunum, causing compression and obstruction ( Fig 2 ). The tumor, mesentery, and affected bowel were resected. The remaining bowel was reanastomosed primarily. Histologic analysis confirmed the mass to be a mesenteric lymphangioma. SUPERFICIAL LYMPHANGIOMA (LYMPHANGIOMA CIRCUMSCRIPTUM) Multiple dilated lymphatic vessels that occupy the papillary dermis are characteristic of lymphangioma circumscriptum. These are associated with delicate fibrous stroma and, therefore, do not simply represent lymphangiectasis. A deep component may or may not be apparent in a given case. Hyperkeratosis is very uncommon though the location of the vessels is the same as that found in angiokeratomas. This is from an area within which there were closely grouped, light amber papules that looked almost like blisters. The patient is a 45 year old female. Scanning power view. A low power view.
