by RAMZI KYSIA UNICEF just released statistics showing a significant improvement in the nutritional status of children in Iraq. According to the figures, over the last two years chronic malnutrition has declined by 23%, and acute malnutrition has declined by almost 50%. The improvement is visible. At the hospitals IOve visited, particularly in Central and Northern Iraq, wasting diseases such as kwashiorkor and marasmus are no longer pandemic. And while doctors throughout Iraq continue to report shortages in essential medicines and equipment, pediatric cancers have replaced malnutrition as their chief complaint. Despite these improvementsUNICEF figures show that over 1 in 5 Iraqi children remain malnourished. Our work isnOt over yet. There are several reasons why malnutrition has declinedalmost all due to busting sanctions. One reason is, fairly obviously, because more food is available. In December 1999, the UN lifted the limit it had placed on Iraqi oil sales through the Oil-for-Food program, and in early 2000 exempted food from the security review process. This allowed Iraq to import more food, more quickly, and distribute it to families in need. Of the $24.2 billion in supplies Iraq has been allowed to import under the Oil-for-Food program to date, almost $10 billion has arrived in just the last yearallowing the Iraqi government to increase the food ration they provide to everyone in Iraq.

According to the Health Care Financing Administration, dehydration was ranked one of the ten most frequent admitting diagnoses in a study on Medicare hospitalizations. Increasing age is one of the major risk factors for dehydration. In fact, those persons between the ages of 85 and 99 years are six times more likely to be hospitalized for dehydration. It is very important that healthcare providers in nursing homes recognize that elderly are at risk for developing dehydration. When a person is recognized as being at risk for dehydration, preventative measures should be taken from the offset to avoid dehydration.

November 12, 1998 During this holiday season, as you visit with friends and family, be sure to check on your elderly relatives and neighbors. If they've looked thin to you, there's a good chance your hunches are right: They're not eating enough. Malnutrition among the elderly can occur rapidly in response to a serious illness, or it can develop insidiously over time as the individual gradually eats less and less food. The good news is that, for many people, early prevention can ward off more serious problems. So go ahead: Bring in food, take them out to eat once a week, send over "care packages," or encourage them to attend the nutrition meal sites or sign up for home-delivered meals through programs offered by the county. Today the "old old" segment of our population, or those over age 85, are increasing at the rate of 150% per year. With advancing age come nutrition concerns centered around adequate food and fluid intake.

For the first time in human history, the number of overweight people rivals the number of underweight people, according to a forthcoming report from the Worldwatch Institute, a Washington, DC-based research organization. While the world's underfed population has declined slightly since 1980 to 1.1 billion, the number of overweight people has surged to 1.1 billion. Both the overweight and the underweight suffer from malnutrition, a deficiency or excess in a person's intake of nutrients and other dietary elements needed for healthy living. "The hungry and the overweight share high levels of sickness and disability, shortened life expectancies, and lower levels of productivity-each of which is a drag on a country's development," said Gary Gardner, co-author with Brian Halweil of Underfed and Overfed: The Global Epidemic of Malnutrition. The public health impact is enormous: more than half of the world's disease burden-measured in "years of healthy life lost"-is attributable to hunger, overeating, and widespread vitamin and mineral deficiencies. Although this information refers to people with Alzheimer's disease, many older people can suffer from malnutrition. So, please read on! Though no scientific evidence links nutritional factors to the cause of Alzheimer's disease (AD), malnutrition and weight loss frequently accompany the illness.
Ironically, people with AD tend to eat less just when they require high-quality nutrition for general good health and to meet their energy needs, which often increases as the disease advances.
People with AD become malnourished for a number of reasons: In the early stages, impaired memory and judgment can interfere with shopping for, storing and cooking food. Family members don't often recognize these subtle changes in behavior and may be unaware the person with AD is not eating regularly or healthfully. Many people with AD report they have difficulty smelling or tasting food. It's sometimes hard for them to distinguish one food from another. They may not enjoy foods that were their favorites, so they eat less. As the disease progresses, people with AD often become agitated and restless. They may wander about for extended periods of time, even at night. This increased activity can raise their energy needs by as much as 1600 calories a day over and above the calories required to maintain their body weight. Yet sitting through an entire meal can be an ordeal for an agitated person. He/she is very likely to fidget or leave the table.
