Miller Fisher Syndrome
Botulism and Miller Fisher syndrome can present in a similar fashion with cranial nerve involvement and pupillary changes. If food-borne, botulism typically occurs as a descending paralysis. The patient is often sick prior to the onset of symptoms and tends to eat canned goods. Having a low vital capacity, pupillary changes, and a normal spinal fluid following symptoms that occurred two to three days before, botulism was a strong consideration. EMG can be useful for the differential and usually shows reduced CMAP amplitudes, normal nerve conductions, and post-tetanic facilitation. Stool or serum will be positive for botulinum toxin in adults; adults tend to ingest the preformed toxin while infants ingest the spores of Clostridium botulinum Imaging studies and CSF should be normal. Adult botulinum poisoning is often fatal if untreated. Other considerations:
Myasthenia gravis may present with rapid-onset of bulbar and respiratory weakness but without autonomic dysfunction. Patients often complain of diplopia, but rarely have complete ophthalmoplegia. There is usually limb weakness as well. Tensilon and EMG testing were not compatible with this diagnosis.

Thank you for providing this forum.
I was recently hospitalized and diagnosed with Miller Fisher syndrome. It began one day with distorted vision and by the next morning I could not control my motor functions and my vision was double. The neurologist ordered every tetst imaginable, blood, CT, tensilon, cranial and spinal MRIs, lumbar puncture, EMG etc.
My dilemmas is, nothing unusual has shown up in the diagnostic work. All of the tests were withing the normal range. There was a very small brain lesion on the MRI but I was told, not in an area likely to cause the type of motor problems I was having. They initially began treating me with steroids for MS, but based on the presence of ophthalmoplegia and lack of reflexes, changed the diagnosis to Miller Fisher. I was given 5 days of IVIg and released. I am still weak - but can walk - although the double vision persists. During a followup appointment last week, my neuro told me that a blood antibody test for Miller Fisher had just come back "negative" but they are standing by the diagnosis based on my symptoms and the response to IVIg.

Miller Fisher Syndrome Medline NLM definition: A variant of the GUILLAIN-BARRE SYNDROME characterized by the acute onset of oculomotor dysfunction, ataxia, and loss of deep tendon reflexes with relative sparing of strength in the extremities and trunk. Antibodies that recognize the disialosyl residue of ganglioside GD1b are specifically associated with sensory ataxic neuropathy. Experimental sensory ataxic neuropathy is induced by sensitization of rabbits with GD1b, which is localized in the primary sensory neurons of dorsal root ganglia.
Antibodies against several other glycoconjugates, including myelin-associated glycoprotein, GM1, GD1a, GalNAc-GD1a and galactocerebroside, are known to be present in patients with some immune-mediated neuropathies. Some of the antibodies may bind to the glycoconjugate(s) with unique localization and determine the distribution of the damage in the pathogenesis of autoimmune neuropathies.

It is often bloody.1 Typical symptoms of C. jejuni infection also include fever, nausea, vomiting, abdominal pain, headache, and muscle pain. A majority of cases are mild and do not require hospitalization and may be self-limited. However, C. jejuni infection can be severe and life threatening. Death is more common when other diseases (e.g., cancer, liver disease, and immuno-deficiency diseases) are present. Children under the age of five and young adults aged 15-29 are the age groups most frequently affected. The incubation period (the time between exposure on onset of the first symptom) is typically two to five days, but onset may occur in as few as 2 days or as long as 10 days after ingestion.1 The illness usually lasts no more than one week; however, severe cases may persist for up to three weeks.
