MITRAL VALVE DISEASE AND HEART FAILURE IN DOGS
What is the mitral valve?
The heart has four chambers. The upper chambers are called atria (singular: atrium), and the lower chambers are called ventricles. The heart is also divided into right and left sides.
Blood flows from the body into the right atrium. It is stored there briefly then pumped into the right ventricle. The right ventricle pumps blood into the lungs where it receives oxygen. It flows from the lungs into the left atrium where it is held a few seconds before going into the left ventricle. The left ventricle is surrounded by the largest and strongest of the heart muscles. This large muscle is necessary to pump blood to all parts of the body.
Each side of the heart has a valve to keep blood from going backward from the ventricles to the atria. The valve between the left atrium and left ventricle is called the mitral valve. Because of the very large pressure created when the left ventricle contracts, the mitral valve wears out in many dogs. This wearing out process begins with a small leak that gradually gets more severe.

Mitral Valve Clinic
The diagnosis and treatment of mitral valve disease is an integral part of the delivery of heart care services. The University of Michigan Medical Center is a leader in the diagnosis and treatment of mitral valve disease. Mitral valve disease and its complications, mitral prolapse syndrome, mitral murmurs, congestive heart failure, shortness of breath, embolism, atrial fibrillation, etc. affect tens of thousands of patients each year. This interdisciplinary program was founded to provide these patients with services that their primary care physicians and cardiologists can not provide, including:
  • provision of identified patients with a multi-disciplinary approach to the diagnosis and treatment of mitral valve disease evaluation of the etiology of mitral valve disease generation of research literature regarding mitral disease provision of patients and referring physicians with mitral valve disease education in the areas of effective diagnostic methods and treatment options available identification and targeting of adult patients at risk for and with mitral valve disease
  • Comprehensive evaluation of patients with mitral valve disease is a frustrating problem in the practice of cardiology, internal medicine and family practice, as many physicians are not familiar with the scope of this disease. The establishment of this program is in keeping with current trends in medical care, which are focusing on timely, effective interdisciplinary approaches to the treatment of systemic diseases.

The Coughing Dog - mitral valve disease May be due to endocardiosis, bacterial endocarditis or rupture of chordae tendineae (usually secondary to endocardiosis). Also congenital mitral valve disease. Eventual outcome is likely to be congestive cardiac failure.
Common in Cavalier King Charles, Chihuahua, Miniature Poodles, Minaiture Schnauzer and Pekingese. Clinical Signs systolic heart murmur
    dyspnoea
    cough
exercise intolerance / collapse etc. Diagnosis Not every small breed with a heart murmur that coughs is in heart failure! It is important to look for (or at least consider other causes eg collapsing trachea). Chest radiographs should be obtained and a proper clinical assessment carried out before assuming the diagnosis. Lateral and DV radiographs should be obtained to look in particular for left atrial enlargement, pulmonary oedema or pleural effusion. Treatment Ace inhibitors (enalapril, benazapril, ramipril)

Valvular heart disease describes structural or functional abnormalities of single or multiple cardiac valves that results in alteration in blood flow across the valve.
According to the causes it can be classified as: Rheumatic
    Rheumatic origin:
    Preceded by rheumatic fever
    Rheumatic fever
Is an inflammatory disease, which occurs as a delayed sequel to pharyngeal infection with Beta hemolytic streptococci (Bacteria).
This may lead to rheumatic heart disease if not treated properly on time. Rheumatic endocarditis Primarily affects the valve leading to fibrous thickening of the valve leaflets or even calcification leads to stenosis. Mitral and aortic valve commonly involved. Management: Antibiotics-Penicillin is the drug of choice. Salicylates and corticosteroids are also used. Non-Rheumatic Non-rheumatic origin: Caused by congenital malformations, bacterial endocarditis, syphilis etc.

    Treatment for mitral valve prolapse:
Specific treatment will be determined by your physician based on:
    • your age, overall health, and medical history extent of the disease your tolerance for specific medications, procedures, or therapies expectations for the course of the disease your opinion or preference
Most patients have no symptoms of mitral valve prolapse and, therefore, will not require any treatment. Antibiotics may be given to persons who have leaky valves, and who are undergoing dental or surgical procedures. These antibiotics are given to prevent an infection called bacterial endocarditis. http://www.ctvsurgery.org/articles.htm
Articles MITRAL VALVE DISEASE The mitral valve is located on the left side of the heart. It separates the atrium from the left ventricle. The right side of the heart collects blood from the body and directs it to the lungs for oxygenation. The left atrium collects blood from the lungs and then propels the oxygenated blood to the left ventricle. The left ventricle then contracts and pushes the blood through the aortic valve to the brain and the rest of the body. Mitral valve disease can manifest as either mitral stenosis, which is a narrowing of the mitral valve opening, or mitral regurgitation, which results in a valve that allows free flow in both directions. A person may be born with mitral stenosis and in fact, some children require operation at a very early age. Most people develop mitral stenosis later in life die to calcification of the valve. Rheumatic heart disease, which may follow a case of rheumatic fever, often affects the mitral valve and can lead to scarring, calcification, and thickening of the mitral valve leaflets. This may result in mitral stenosis, mitral regurgitation, or even a combination of the two. Although mitral stenosis may be corrected by valve repair in some patients, in the vast majority replacement of the valve is necessary. Mitral regurgitation occurs when the leaflets of the valve fail to meet, which results in backflow through the valve. The end result is that the ventricle (the pumping chamber of the heart) has to work twice as hard to supply the same amount of oxygenated blood to the body. Eventually, due to the increased workload, the ventricle will increase in size and become weakened, and may suffer irreparable harm. Mitral regurgitation may be due to prolapse of one of the valve leaflets. More often it is due to thinning of the leaflet tissue over time, dilatation of the base of the valve

Friday, August 30, 2002 Chihuahuas and human beings don’t appear to have a lot in common. But one thing Chihuahuas and humans do have in common is that they both can suffer from heart disease — specifically mitral valve disease.
Unfortunately, the cure that’s available for people — surgical replacement of defective heart valves — isn’t practical for Chihuahuas. Consequently, treatment of Chihuahuas with mitral valve disease involves trying to control the disease. Meanwhile, without proper care, some Chihuahuas may die within two years of diagnosis.
A researcher at the University of Illinois Urbana-Champaign is giving owners of Chihuahuas with advanced mitral valve disease new hope. Using a medication that’s usually prescribed for human cardiovascular patients, Mark Oyama, D.V.M., assistant professor of small animal specialty medicine at the College of Veterinary Medicine, may have found a way to reduce the effects of mitral valve disease in dogs, literally giving these dogs new leases on life.

What is Mitral Valve Disease (MVD)?
The heart consists of 4 chambers, 2 atria and 2 ventricles, with the atrioventricular valves ensuring that blood flows from the atria to the ventricles when the heart is beating. A defect or weakness in the mitral valve or the left atrioventricular valve allows some blood to move back into the left atrium, known as mitral regurgitation. This means the heart is less efficient at pumping blood through the body.
Mitral valve insuffiency is the most common of the acquired cardiac disesase in older dogs, affecting over 1/3 of dogs older than 10 years. However, in certain breeds, mitral valve insufficiency develops at a younger age, due to an inherited predisposition for the disorder.
Some of the breeds affected with a higher incidence of this condition are the Cavalier King Charles Spaniel, Cairn Terrier, all 3 sizes of Poodles, Bull Terrier, Miniature Pinschers, Whippets, Chihuahua, Pekinese, Dachshund, Beagle, Papillion, Dobermans, Great Dane and German Shepherd. In general, the smaller breeds are most often affected.

    INTRODUCTION
Mitral valve repair is an open heart procedure performed by cardiothoracic surgeons to treat stenosis (narrowing) or regurgitation (leakage) of the mitral valve . The mitral valve is the "inflow valve" for the left side of the heart. Blood flows from the lungs, where it picks up oxygen, and into the heart through the mitral valve. When it opens, the mitral valve allows blood to flow into the heart's main pumping chamber called the left ventricle. It then closes to keep blood from leaking back into the lungs when the ventricle contracts (squeezes) to push blood out to the body. It has two flaps, or leaflets.
The mitral valve is highlighted on this illustration of a heart. The mitral valve is the "inflow valve" for the left side of the heart. Blood flows from the lungs, where it picks up oxygen, and into the heart through the mitral valve. (Click on the illustration to enlarge it.) Occasionally, the mitral valve is abnormal from birth (

    Mitral Valve Prolapse
Mitral valve prolapse (MVP) is the most common condition of the heart valves. Some studies show it affects 6% of all women. Incidence
MVP is extremely interesting since it is so commonly diagnosed in young women and yet, the incidence decreases markedly in elder women. This drop off in incidence is not seen in the male population (refer to graph below). The occurrence of MVP in men is consistent in both young and old populations. Previous to the Framingham study, the incidence of MVP was thought to be much higher in women, however, with this new data, it is now believed that MVP affects equal numbers of men and women. This intrigues researchers searching for the cause(s) of mitral valve disease and attempting to explain why the condition manifests itself so differently in men and women. The cause of MVP is usually unknown but occassionally, MVP is associated with other heart conditions (e.g. atrial septal defect, coronary artery disease, diseases of the heart muscle) but most likely, these diseases would have been present with or without MVP. The outcome from MVP differs for men and women too. Recent studies have indicated that, for men, the incidence of mitral prolapse requiring surgical intervention is higher than for women and increases with age.
