What is the mitral valve?
The heart has four chambers. The upper chambers are called atria (singular: atrium), and the lower chambers are called ventricles. The heart is also divided into right and left sides.
Blood flows from the body into the right atrium. It is stored there briefly then pumped into the right ventricle. The right ventricle pumps blood into the lungs where it receives oxygen. It flows from the lungs into the left atrium where it is held a few seconds before going into the left ventricle. The left ventricle is surrounded by the largest and strongest of the heart muscles. This large muscle is necessary to pump blood to all parts of the body.

*Oddzia³ Internistyczno-Kardiologiczny Szpitala w ¦wiebodzicach A case of 41-year-old patient with moderate combined mitral valve disease and severe left ventricular insufficiency is presented. Despite of only moderate mitral valve stenosis the patient suffered from recurrent pulmonary oedema. Angiography showed normal coronary arteries, and the patient underwent mitral valve replacement with subsequent significant clinical improvement. The issues concerning referral for surgery and decision-making in valvular disease are discussed.

    Myxomatous Mitral Valve Project
    Myxomatous mitral valve disease is a frequent disorder that is characterized by enlarged, thickened, floppy, gelatinous leaflets, and elongated chordae tendineae. The myxomatous valve as a whole is often moderately to severely regurgitant, which means that blood may leak back through the valve into the left atrium. The optimal treatment to correct this disease is a surgical repair procedure that removes the excess valve tissue and restores a more normal size to the valve. Some doctors, however, have concerns about the durability of the surgical repair since the remaining tissue is still diseased and therefore is possibly unfit to function normally, even after the repair. Normal Mitral Valve Myxomatous Mitral Valve Microscopic examinations of myxomatous tissues have shown that the mitral leaflets and chordae have disorganized, fragmented collagen and elastic fibers, as well as an abnormal accumulation of glycosaminoglycans (GAGs). GAGs are particular types of carbohydrates that have diverse structural and regulatory roles in connective tissues. In heart valves, for example, it is thought that certain GAGs provide lubrication between the valve layers and allow for recovery from compressive stresses, among other functions. Our biochemical research is focused on the identification of different GAG types, structures, and quantities in normal and myxomatous mitral valves. To accomplish these goals, we have chosen to using innovative fluorophore-assisted carbohydrate electrophoresis (FACE) and immunohistochemical methods. These findings will then be correlated with the mechanical properties of these tissues to determine if the accumulation of specific GAGs is associated with functional tissue changes. We are also growing interstitial cells from myxomatous and normal mitral valve organ cultures, in order to identify any phenotypic differences that would suggest a cell-mediated pathoetiology.

    Mitral valve repair

    Mitral repair procedures that preserve the native valve tissue are popular for mitral valve disease. As compared to prosthetic replacement, repairs may have fewer complications, and long-term anticoagulation can be avoided. This is especially attractive in children, as well as adults (especially women of childbearing age and patients with bleeding tendencies) who are noted to have early stages of mitral valve disease. However, due to degeneration of biological or synthetic material used during the repair, re-operation may be required at a later date. The terms annuloplasty and valvuloplasty are often used to categorize the type of repair. An annuloplasty describes procedures performed on the valve annulus or ring that holds the valve leaflets. Valvuloplasty is a broad category that refers to reconstruction of the valve leaflets, chordae and/or papillary muscles (that connect the valve to the heart). A careful evaluation of the leaflets and surrounding tissue is performed before the best operative technique is selected.

    Mitral Valve Prolapse WHAT IS MITRAL VALVE? The mitral valve is one of the four valves of the heart. It is located in the left side of the heart, and it separates the left atrium (or upper chamber) from the left ventricle (or lower pumping chamber). Because the mitral valve lies between the left atrium and left ventricle, it is also known as the left atrioventricular valve. Valves open and close, like doors. The closure of the valves in the heart is what produces the characteristic "lub-dub" heart sounds. They help direct the flow of blood through the heart, normally allowing flow in one direction. The mitral valve leaflets open equally when blood is flowing from the left atrium and filling the left ventricle. The mitral valve leaflets then close, with both leaflets firm and shut, when the left ventricle contracts and pumps the blood out to the body. WHAT IS MITRAL VALVE PROLAPSE? Mitral valve prolapse (MVP) is a condition in which there is a slight variation in shape or structure of the mitral valve, causing one or both of the mitral valve leaflets to "billow" or "bulge" backward into the upper left atrium during the contraction of the lower left ventricle. Remember, when the left ventricle contracts and ejects blood into the aorta and to the body, the mitral valve leaflets should remain firm and shut. Instead, the mitral valve leaflets(s) billow backward causing prolapse Sometimes when the mitral valve leaflet prolapses, it may produce a "click". As mentioned earlier, valves help direct the flow of blood in one direction through the heart. Often, when the mitral valve leaflet billows, or prolapses, it allows for a small backward leak of blood. This may produce a soft murmur. The "click-murmur", if present, can sometimes be heard with a stethoscope. Over the years, MVP has also been called the Click-Murmur Syndrome, the Floppy-Valve Syndrome, and Barlow's Syndrome, after the name of its discoverer.

  • Papillary muscle attached to chords that are attached to the mitral valves anterior and posterior leaflets were illustrated. Rupture of a chord is like the rupture of a shroud of a parachute with similar catastrophic results. The result is a prolapse of the leaflet into the atrium with retrograde blood flow occurring acutely without the opportunity for a compensatory response.
  • Reasons for mitral valve repair: Myxomatous degeneration, ischemia (papillary rupture, regional with global dysfunction), annular dilatation, annular calcification, rheumatic fever complication, endocarditis complication, congenital.
  • "Breakdown" of repair is more apt to occur with anterior leaflet repair.

    Valve Disease
    Every time your heart beats, blood flows into, through, and out of your heart. In fact, your heart moves about 100 gallons (379 liters) of blood through your body every hour. Blood is pumped through your heart in only one direction. Heart valves play key roles in this one-way blood flow, opening and closing with each heartbeat. Pressure changes behind and in front of the valves allow them to open their flap-like "doors" (called cusps or leaflets) at just the right time, then close them tightly to prevent a backflow of blood. There are 4 valves in the heart:
    • Tricuspid valve Pulmonary valve Mitral valve Aortic valve
    Blood without oxygen returns from the body and flows into the heart's upper-right chamber (the right atrium). From there, it is forced through the tricuspid valve into the lower-right chamber (the right ventricle). The right ventricle pumps the blood through the pulmonary valve and into the lungs. While in the lungs, the blood picks up oxygen. As the right ventricle is preparing to push blood through the pulmonary valve, the tricuspid valve closes to stop blood from flowing back into the right atrium.

    Mitral Valve Prolapse and Thyroid Disease

    Recent medical research has found that the prevalence of mitral valve prolapse is substantially greater in patients with autoimmune thyroid disorders such as Graves' Disease and Hashimoto's Thyroiditis. According to the About.com Heart Surgery Forum, MVP is the most common heart valve abnormality, with estimates ranging from 2 million or more Americans diagnosed with this condition, and most are women (about 80%).
    When you have MVP, one or both valve flaps are enlarged. When the heart contracts or pumps, the flaps don't close smoothly or evenly. Instead, part of one or both flaps collapses backward into the left atrium. This sometimes allows a Mitral Valve Prolapse small amount of blood to leak backward through the valve and may cause a heart murmur.
    Major Symptoms of MVP include: - Pounding, fast heartbeat

    Mitral Valve Prolapse
    The picture above shows the valves of a normal heart. In the "systole" picture in the upper right corner notice how the ends of the valve meet perfectly to close off the flow of blood. Then, in the picture below, notice how in Mitral Valve Prolapse, the ends of the valve do not quite come together, allowing a leakage of blood back into the chamber of the heart. DEFINITION OF MVP:
    A fairly common and often benign disorder in which a slight deformity of the mitral valve (situated in the left side of the heart) can produce a degree of leakage (mitral insufficiency). Mitral valve prolapse causes a characteristic heart murmur that may be heard through a stethoscope.
    Often no symptoms are present and the condition may be discovered on a routine examination.
    On the other hand, there are many who experience the following symptoms :
    Chest pain (sharp, dull or pressing).
    Fatigue, shortness of breath.
    Dizziness.
    Anxiety.
    Lightheadedness when getting up from a chair or bed.
    Palpitations.
