This is the clear message given by Adrain Millen, Director of Dental Services of the Northern Health and Social Services Board. Mouth Cancer Awareness Week starts on Monday 12 November with the aim of informing more people about how to help prevent the disease. Around 100 people in Northern Ireland develop mouth cancer every year but if symptoms are spotted early and treated rapidly, patients have a good chance of making a full recovery. Mouth cancer can appear in deferent forms and can affect all parts of the mouth, tongue, and lips. Most often, cancer appears as a painless ulcer that does not clear up but sometimes a white or red patch in the mouth may develop into cancer. It is important to visit your dentist if these areas do not heal within two weeks. If your dentist suspects signs of cancer, you will be referred to a consultant who will carry out the necessary examinations. The vast majority of tobacco users and smokers are hooked when they are children. During this time period they are easily influenced by peer pressure and advertising. Once hooked, the majority of tobacco users become hopelessly addicted. More than 5 million children living today will die prematurely because of a decision they will make as adolescents-the decision to use tobacco and smoke cigarettes. Studies indicate that nonsmokers are also adversely affected by environmental tobacco smoke. There are 4000 chemicals in tobacco with 100 identified poisons and 63 known drugs which cause cancer. The most talked about drug in tobacco is nicotine. Nicotine is vasoconstrictor and a nerve toxin, it has been classified as a class I insecticide. The Hopkins scientists report that squamous cell carcinoma in patients over age 75 "more commonly results from accumulated spontaneous mutations rather than through heavy exposure to known carcinogens, and less commonly involves mutation of the p53 gene." Mutation of the p53 gene, a tumor suppressor gene that normally helps to prevent the formation and growth of cancers, is the most common cancer-related genetic mutation. Many independent mutations in different genes are required for a cell to become a cancer cell. Results published in the March 1995 issue of Archives of Otolaryngology Head and Neck Surgery suggest older people may develop cancer because they accumulate genetic mutations that usually occur only in people who have been exposed to carcinogens. The aging process itself may increase the tendency for certain older people to develop cancer, the researchers say. In support of this view, the study found that older patients who develop cancers besides those in the head and neck were more likely than their younger counterparts to get skin and prostate cancers as well. Both of these cancers are commonly associated with old age. In contrast, when middle-aged people developed other cancers, they were more likely to have lung and colon cancers. These cancers are strongly associated with smoking, drinking and mutations in the p53 gene.
