Posters Moyamoya disease associated with Alagille syndrome. Evidence against a common vascular pathological basis. Bosma JJD, Golash A, Mallucci CL and May PL (Liverpool) Objectives To show that the association between moyamoya disease and Alagille syndrome is coincidental.
Materials and Methods We report the fourth case in the world with the association of moyamoya disease and Alagille syndrome. The data available on these four patients are shown. The hypothesis of other authors that both conditions share the same vascular pathological basis is checked through a survey of the world literature.
Discussion The literature shows that the disease gene for Alagille syndrome has been mapped to chromosome 20 [2]. Evidence suggests that moyamoya disease is a result of an upregulation of the elastin gene expression [3]. This gene is located on chromosome 7 [1].
Conclusion It is unlikely that a single pathological phenomenon is the basis of both Alagille syndrome and moyamoya disease. Its association might be coincidental.

Summary Background . To investigate whether nitric oxide (NO) contributes to formation of abnormal collateral circulation in patients with moyamoya disease. Methods . Cerebrospinal fluid (CSF) samples were obtained from the subarachnoid space of the Sylvian fissure during combined bypass surgery for moyamoya disease and kept frozen until NO metabolites, nitrate and nitrite, were measured using a Griess method. Results Interpretation. Nitric oxide concentrations in CSF are chronically elevated in moyamoya disease, probably reflecting development of abnormal collateral circulation. i.e. moyamoya vessels. Vascular bypass surgery can reduce abnormal collateral circulation with reduced production of nitric oxide. Article in PDF format (235 KB) Online publication: December 1, 2000

= Abstract = Recent Clinical and Basic Research in Moyamoya Disease Dal Soo Kim, M.D. Department of Neurosurgery, Medical College, Catholic University, Seoul, Kore Moyamoya disease is an unusual form of chronic cerebrovascular occlusive disease characterized by bilateral stenosis and/or occlusion of distal internal caroid arteries and their vicinity, by a hazy network of collateral circulation at the base of brain called moyamoya vessels and clinically by recurrent ischemic attacks in children and by intracerebral hemorrhage in adult. This review included the trends of recent clinical and basic research in moyamoya disease, mainly selecting from recent annual reports published by the research committee on spontaneous occlusion of the circle of Willis(moyamoya disease) of the Ministry of Health and Welfare, Japan.

Case

A 35-year-old female was admitted to the hospital because of speech disturbance and right side paresthesia of the body, which gradually worsened. The patient had been well until one month earlier. There was no important finding of family history, physical examination including neurological tests, laboratory data and chest X-ray except the electrocardiographic findings which showed ST segment depression and T wave inversion in lead , aVL, V3-6 and Q wave in V1-2 (Figure 1). Magnetic resonance imaging of the brain showed infracted area of low signal intensity on the T1-weighted image and high signal intensity on the T2-weighted image in the left superior parietal lobe (Figure 2). A angiography of the both cerebral arteries showed obliterated anterior cerebral arteries and middle cerebral arteries with characteristics "puff of smoke", that is the typical finding of moyamoya disease (Figure 3). Though the patient had no angina history before, a coronary angiography which was performed due to abnormal electrocardiographic findings and hypokinetic wall motion of the anterior myocardium on 2 dimensional echocardiography, showed the left coronary artery with ostial total obstruction and collateral blood flow from normal right coronary artery (Figure 4). The final diagnosis was moyamoya disease with total occlusion of left main coronary artery.

Traboulsi The Center for Genetic Eye Diseases, Department of Pediatric Ophthalmology, Cleveland, United States of America PURPOSE: To report a patient with congenital constrictive malformations of the internal carotid arteries and a morning glory disc anomaly (MGDA). METHODS: Descriptive case report. RESULTS: A 14-year-old girl underwent serial ophthalmologic examinations since the age of seven years because of blurred vision and floaters. A right optic disk anomaly was present that was most compatible with an MGDA accompanied by an inferior choroidal coloboma. Because of the association of such disc malformations with moyamoya disease, the patient underwent cerebral MRI and MRA that showed narrowing of both internal carotid arteries, a finding consistent with early-stage moyamoya disease. CONCLUSION: This case illustrates the importance of recognizing the association of MGDA with moyamoya disease, a treatable condition that may have devastating neurological consequences.
