MULTIPLE SYSTEM ATROPHY Philip A. Hanna, M.D. Parkinson's Disease and Movement Disorders Center New Jersey Neuroscience Institute Edison, New Jersey Multiple system atrophy (MSA) is a sporadic, progressive disorder encompassing three previously-described neurodegenerative disorders: Shy-Drager syndrome (SDS), striatonigral degeneration (SND), and olivopontocerebellar atrophy (OPCA). All three types of MSA are characterized by parkinsonism, particularly rigidity, slowed and decreased movement, and impaired balance, though rest tremor is not a main feature. Other symptoms may include weakness, spasticity and disturbance of the autonomic nervous system resulting in lightheadedness or fainting spells upon standing (due to a marked fall in blood pressure), incontinence of bowel and bladder, sleep apnea, and sexual dysfunction (impotence) in males. Shy-Drager Syndrome (SDS) has a predominance of autonomic disturbances, as listed above including postural lightheadedness/fainting, impotence and bowel and bladder dysfunction. Patients may also have dry mouth and skin, abnormal sweating and difficulty regulating body temperature, sleep apnea and problems with chewing, swallowing and breathing. The treatment of SDS is difficult as levodopa can further lower blood pressure. Means of reducing postural blood pressure changes include increasing dietary salt, use of Jobst stockings, and medications such as fludrocortisone or midodrine. Since these methods can increase blood pressure when supine (lying down), the head of the patient's bed should be elevated at least 30 degrees.

Using positron emission tomography with [ C dihydrotetrabenazine , I am studying patients who have dementia with Lewy striatal presynaptic monoaminergic terminals. I am using the same ligand with PET and also [ C raclopride to study both presynaptic dopaminergic terminals and D 2 dopaminergic receptors in patients with sporadic cerebellar degeneration in comparison to patients with multiple system atrophy of both the parkinsonian and cerebellar types. I am also using PET with [ F fluorodeoxyglucose to study the hereditary spinocerebellar atrophies. In all of these studies, the PET evaluations of neurotransmitters and receptors are correlated with the results of clinical examinations, anatomical imaging with MRI and neuropsychological testing. Idiopathic Orthostatic Hypotension And Shy-Drager Syndrome Cerebellar And Spinocerebellar Disorders Disorders of the cerebellum and its inflow or outflow pathways produce deficits in the rate, range, and force of movement. Anatomically, the cerebellum has three subdivisions. The archicerebellum (vestibulocerebellum) comprises the flocculonodular lobe, helps maintain equilibrium and coordinate eye-head-neck movements, and is closely interconnected with the vestibular nuclei. The midline vermis (paleocerebellum) helps coordinate movement of the trunk and legs. Vermis lesions result in abnormalities of stance and gait. The lateral hemispheres, which make up the neocerebellum, control ballistic and finely coordinated limb movements, predominantly of the arms. Signs of cerebellar disease are listed in Table 179-3.

Lang Abstract: Objectives: The majority of patients with diffuse Lewy body disease have cognitive or psychiatric manifestations as part of their initial presentation. A sizable minority present with parkinsonian features alone. Autonomic features may also occur, typically after the development of cognitive changes. We aim to demonstrate that diffuse Lewy body disease may rarely also present with parkinsonism accompanied by marked autonomic dysfunction in the absence of significant cognitive or psychiatric abnormalities. Methods: Case report based on a retrospective chart review and neuropathological examination. Results: Conclusions: In addition to cognitive, psychiatric, and parkinsonian presentations, diffuse Lewy body disease may present with parkinsonism and prominent autonomic dysfunction, fulfilling proposed criteria for the striatonigral form of MSA.
