CONGENITAL MYOPATHIES include ocular muscle disorders presenting at birth or early childhood and are part of more systemic conditions affecting skeletal and, rarely, cardiac muscle. Infants are usually ‘floppy’ and may be observe to have limited ocular excursions and ptosis. Diagnosis is afforded by skeletal muscle biopsy with electron microscopy, histochemical staining, biochemical testing for genetic analysis. C HRONIC P ROGRESSIVE E XTERNAL O PHTHALMOPLEGIA describes a variety of conditions presenting at any age characterized by gradual limitation of eye movement function. While CPEO may be limited to just the extraocular muscles and lids these conditions are usually accompanied by systemic involvement of other muscles and tissues, especially those with high energy requirements such as brain and kidneys when the basic defect is a mitochondrial DNA abnormality (mtDNA disorders - see prior Grand Rounds presentation for more on these conditions). DYSTROPHIES are primary muscle diseases genetically determined, both recessive and dominant. Some effect eye muscle as well, as in Oculopharyngeal and Myotonic dystrophy DERMATOMYOSITIS reportable cases - most often no EOM involvement Return to Slide

Muscle Disorders     A necropsy is the only way to find out if this is a problem.  This is caused by a Vitamin E deficiency.  When a combination of Vitamin E and sulfur containing amino acids occur, chicks show symptoms of muscular dystrophy, especially in the breast muscle.  Bundles of affected muscle cause light colored streaks in the breast.

Project Duration : Contribution BBW/OFES : CHF 50'000. Project-Number : BBW 97.0047-2 Project Partners : Abstract Many people suffer physically from working with computers. Common problems are aches and pain in the neck and shoulder region and disorders in the hands or the wrists. The European project PROCID aimed at providing a scientific base for prevention measures against such problems. A key element in medical and physiological studies of such issues is the analysis of electromyograms (EMG), i.e., the recordings of the electrical signals that are associated with all muscle activity. Ideally, the medical researcher would like to see the activation pattern of each individual motor unit (muscle fibre group). However, the measured electrical signal is the superposition of the signals from a large number of such of motor units, the 'signature' of which is not known a priori and may even change over time. The desired decomposition of the measured signal is a challenging signal processing task. Further information: http://www.isi.ee.ethz.ch/research/research_topics/biomedsp/index.en.html

BS397 LECTURES 20-24 Neuromuscular Disorders The following topics are covered to give a feel for how the application of new methods has advanced our knowledge and understanding of muscle diseases, and to correlate pathological information with clinical data at the individual patient level. HISTORY Our understanding of neuromuscular disorders usually begins with clinical descriptions and attempts to classify the disease and then on to a description and understanding of its pathology (often helped by animal and experimental studies involving physiology, biochemistry and pharmacology). New breakthroughs in molecular genetics have now allowed us to accurately classify and describe the inheritance, aetiology and pathological mechanisms involved in many of these disorders and are bringing us to the verge of possible new treatments. Clinical descriptions of neuromuscular disorders in sporadic and familial groups exhibiting proximal or distal wasting and weakness and variable progression. Some attempts at relating pathology of nerve or muscle to the disease.

The mission of the Kentucky Arthritis Foundation is to improve lives through leadership in the prevention, control and cure of arthritis and related diseases. Kentucky Osteopathic Medical Association (KOMA) The Kentucky Osteopathic Medical Association knows that you are more than just the sum of your body parts. That’s why doctors of osteopathic medicine (D.O.s) practice a "whole person" approach to health care. Instead of just treating specific symptoms, osteopathic physicians concentrate on treating you as a whole. Osteopathic physicians understand how all the body’s systems are interconnected and how each one affects the others. They focus special attention on the musculoskeletal system, which reflects and influences the condition of all other body systems.

Patient Notes Patient Notes are handouts that physicians can give patients to help answer their questions. Each one describes a common medical problem, in language patients will appreciate, and typically includes clear and simple advice as well. One (and sometimes more) appears in each issue of Postgraduate Medicine , a peer-reviewed clinical journal for primary care physicians. Collected here and categorized by topic are selected Notes from past issues. Keep checking, as we will make new postings every month. If you are a physician and have an idea for a topic or would like to write a Patient Note for our consideration, please send us an e-mail Find more patient handouts on The Physician and Sportsmedicine's Personal Health Web page.

Update on Recent Research in Muscle Disorders

GENE THERAPY As you will be aware, teams of scientists all over the world are working to convert the large amount of knowledge we have gained about the Duchenne muscular dystrophy gene into practical treatments that will help cure or lessen the effects of the disease. Many different approaches are being taken, and several groups reported important progress in 1999. More details of these publications can be found by clicking on the American Muscular Dystrophy Association web site. The four main strategies in use are: Replacing the defective DMD gene with a working copy. Because of the enormous size of the DMD gene - it is the largest gene known - this approach has proven very difficult, and little progress has been reported in the literature recently. Increasing production of a related protein which may be able to substitute for the dystrophin protein. The discovery of the "Utrophin" gene, closely related to the DMD gene, by Professor Kay Davies in Oxford has given rise to this avenue of research. Good progress has been made on ways to increase the production of Utrophin in muscle, but the study of patients with naturally high Utrophin levels indicates that they may have problems with blood clots, and another study showed that natural over-expression of Utrophin was not sufficient to prevent DMD-associated heart disease. Nevertheless, mice with a form of DMD did show improvement when Utrophin gene therapy was used.

Order Form Reviews IASP Temporomandibular disorders and chronic orofacial pain conditions are common sources of frustration to patients and clinicians. This book reviews current research on the mechanisms of temporomandibular and other orofacial pain conditions of muscle and joint origin, considers the biologic responses associated with temporomandibular disorders, and provides scientifically based recommendations on diagnosis and treatment. Useful to clinicians who treat patients with these conditions and researchers interested in the pathophysiology of muscle and joint pain.

Creatine supplements help muscle disorders
March 09, 1999 NEW YORK (Reuters Health)Creatine, an over-the-counter amino acid supplement popular among athletes, may help to build muscle and increase muscle strength in patients with muscular dystrophy and other neuromuscular disorders, a study suggests. People with these muscle disorders had a small increase in handgrip, ankle, and knee strength after taking creatine monohydrate daily for 10 to 12 days, according to the report in the March 10th issue of the journal Neurology. The study included 21 people with various types of neuromuscular disorders including muscular dystrophy, which cause muscle atrophy (wasting) and loss of strength. "The treatment appears to be well tolerated in the short term," report Dr. Mark Tarnopolsky and Joan Martin, of McMaster University Medical Center in Ontario, Canada. But the researchers caution that more studies need to be conducted to determine if creatine continues to have a benefit over a longer period of time. The subjects consumed either a placebo ("dummy" pill) or 10 grams of creatine monohydrate daily for five days followed by five grams of creatine monohydrate daily for five to seven days.
