Scottish Parliament Public Petitions Committee Tuesday 2 October 2001 Myalgic Encephalomyelitis (PE398) The Convener: Helen McDade: I have with me Linda McLean, the mother of a severely affected 15-year-old girl who has been ill for three years, and Alan Stroud, a 17-year-old sufferer of ME, who has been ill for three years. I hope that the committee will appreciate that Alan had to make a lot of effort to be here this morning. I am the mother of an 11-year-old girl who has been ill with ME for five and a half years. She has attended school for about 25 per cent of that period. When she became ill at five and a half, she wanted to be a ballet dancer and join the rainbow guides and the brownies. She wanted to go swimming and take up gymnastics and horse riding. She is now 11 and has not done any of those activities. Her childhood ended when she was six. Many children are like her and Linda McLean represents another such child. We are here today because ME sufferers and their families are desperate. Members of the committee are one of our last shots. It is estimated that 15,000 Scots in Scotland have ME. That is more than the number of people with multiple sclerosis. Of that number, about 2,000 are children and of those children, 25 per cent are severely affected, which means that they are housebound or bedridden. Unable to stand noise, they may live in a darkened, soundproof room because of hypersensitivity and they may not receive support from anyone except, hopefully, their families. Many people live with such suffering.

: Through its history, Myalgic Encephalomyelitis (Chronic Fatigue Syndrome) was perceived to be psychosomatic illness with no organic causes. This has only begun to change in recent years, and many professionals still outspokenly advocate or subtly treat M.E. as a psychiatric disorder. The main research and treatment directions involve anti-depressants, psychotherapy, and exercise. Alternative Challenge : Evidence that M.E. is an organic disorder is overwhelming, but obtusely dismissed by the medical establishment. Exercise often triggers relapses and is inappropriate as a universal treatment. Psychotherapy is useful inasmuch as it is helpful to any other chronic illness or disability. MYALGIC ENCEPHALOMYELITIS IN S. W. SCOTLAND: A CASE-CONTROL STUDY MEASURING ELF EM FIELD EXPOSURE Summary In the ME case homes mean magnetic field strengths (87nT; SD 46) did not differ significantly from controls (99nT; SD 265). Mean electric fields however were almost twice as high (22.7V/m; SD 39.2) compared with controls (13.0V/m; SD 15.5), and this difference was significant (p=0.035). Unusually, 57 percent of cases (35/61) lived in homes on or next to corners, near the ends of cul-de-sac streets, or in end-terraces where the electric cable feed entered the block. These were also the sites of highest electric fields. The significance of this finding is not clear. These results, though from a relatively small sample, suggest that ME may be associated with weak chronic exposure to ELF electric fields. The study gives support to recent occupational studies also finding over-representation of adult cancer subtypes where occupational electric field exposure was elevated. The curious correlation between corner homes and cul de sac homes with ME cases in this sample, (which could not have arisen through selection bias), needs further investigation, and a larger study is needed to confirm these results. INTRODUCTION In Ayrshire an outbreak in West Kilbride was described (Fegan, Behan et al., 1983) affecting 22 patients between January 1980 and June 1983, mainly female. The most characteristic symptom was extreme exhaustion, particularly after exercise. No clear aetiology or diagnostic marker for ME has yet been isolated. Though according to Ramsay (1989) the disorder is often associated with Coxsackie B viral infection (which was found in 18 of the 22 West Kilbride patients), it is not clear whether the viral insult is post-opportunistic, since no clear infectious pathway has been isolated.

Chronic Fatigue Syndrome
This condition is also referred to as ME (Myalgic Encephalomyelitis), Post Viral Syndrome, Fibromyalgia, or Royal Free Disease and a variety of opinions have been put forward suggesting possible co-factors that may be responsible for its development. These may include:
  • A stressful way of life that does not give a chance for relaxation and recovery to occur.
  • Dependence on stimulants in order to keep the pace. These may include coffee, recreational or prescription drugs, cigarettes or alcohol.
  • Reliance on 'junk foods' rather than eating a good quality, varied diet.
  • Recurrent bouts of thrush.
  • A history of recurrent infectious illness with repeated treatment by antibiotic drugs. This is suspected to be the case because the latter are regarded by alternative therapists to have a compromising effect on the efficient functioning of our immune systems.
  • Contracting a viral illness that is not given a chance to clear up, but keeps relapsing.
  • It has also been suggested that a sensitivity to vaccinations may also create a vulnerability to developing chronic fatigue syndrome, especially if other co-factors are present.

In the spring and summer of 1975, there was a major, severe epidemic of a communical, apparent viral disease at the Mercy San Juan Hospital in Carmicheal, a suburb of Sacramento, California. It occurred in February, and the bulk of the disease happened between July and November of 1975. Cases continued to occur, although few, until 1978 The epidemic involved all departments of the hospital. It was equally severe in all departments. I was appointed chairmen of a committee to investigate the outbreak. At the time, I feared that there might be fatalities and so Asked that the CDC (Communicable Disease Center, Atlanta, Georgia) become involved. An epidemiologist fro there spent a week at the hospital. Another epidemiologist from the State came for the day. Cultures were obtained for all known viruses, bacteria, and rickettsiae, and all were negative. The disease was apparently due to a new agent of disease. At the time, we did a literature search and found three reports of outbreaks that were called EPIDERMIC PHLEBODYNIA (meaning painful veins). While the disease at Mercy San Juan was somewhat similar, it included many more features that were subscribed in epidermic phlebodynia and so at the time, I believed it was not the same disease. Later , additional literature search showed that the disease was very similar to epidermic neuromyasthenia/ myalgic encephalomyelitis. But toublingly, very few if any, vascular features were mentioned.

He smiles at me while I remove my coat and sit down. A cursor flashes on his computer screen next to my name. I see that his coffee break follows my consultation. I see that I have seven minutes. Seven whole minutes. There is a list of other patients underneath me, some with asterisks by their surnames. "Post-Viral Syndrome?" I ask. There are no pictures on the walls, no personal objects on the desk. Nothing to rest my eyes upon. Maybe he likes it this way, cool, impersonal... institutional. Why should his patients - or are they customers now? - see his life, see his loved ones in little heart-shaped frames? "But... that's the same as M.E. isn't it?" I say, "Or... or Chronic Fatigue Syndrome?" unsure of the new terms I am using. Myalgic Encephalomyelitis. Myalgic Encephalomyelitis. It gets louder in my head, beats out a rhythm of panic. "There's a fine line between the two," he replies slowly, quietly, obliquely.

What is Chronic Fatigue Syndrome?
Generaly Known as CFS- Chronic Fatigue Syndrome or CFIDS - Chronic Fatigue and Immune Dysfunction Syndrome or ME - Myalgic Encephalomyelitis A complex and debilitating chronic illness that affects the brain and multiple body systems. Although its name focuses only on one aspect of the illness, chronic fatigue and immune dysfunction syndrome (CFIDS) is a serious and complex illness marked by numerous symptoms including disabling fatigue, problems with information processing and memory, flu-like symptoms, pain in the joints and muscles, dizziness, nausea, sleep disorders and headache. Strong support and understanding from family and friends is very important to people with this debilitating and poorly understood illness as they work to integrate the challenges adn lifstyle changes caused from CFIDS into their lives. The keys to helping a person with CFIDS are education, communication and emotional support. Most importantly is beleive them when they say they are too tired to go for a drive, or to visit family, or that they need to take a nap from doing something as simple as bathing the baby.
When we tell people that My husband Suffers from Myalgic Encephalomyelitis(CFS), and they hear that there is no known test or cause for Chronic Fatigue Syndrome, they immeditately take that as "this illness is not real". This is just not the case and so hard to hear over and over.

    Myalgic Encephalomyelitis
    Mr. Llew Smith : To ask the Secretary of State for Health (1) what research is currently funded by her Department into myalgic encephalomyelitis ; and what research has been completed by her Department into myalgic encephalomyelitis since 1979 ; (2) what attempts she has made to ensure that all GPs are aware of the research and diagnostic criteria developed to ensure the proper health care for sufferers of myalgic encephalomyelitis ; (3) what representations she has received from the Myalgic Encephalomyelitis Association in regard to counselling and support offered by the national health service to myalgic encephalomyelitis sufferers ; (4) what information she has on the occurrence of myalgic encephalomyelitis in teenagers.
