We have found the first evidence of neuronal degeneration in narcolepsy.
After hearing the punch line of the joke, the teenager falls to the floor, almost as if actually punched. She remains there, completely unable to move. She hears her parents reassure her friends that they need not worry about her because she will be all right in a few minutes. She is embarrassed and frustrated as the episode continues, and her friends begin to leave. They bid her goodbye, but she is unable to respond. Although she cannot talk or move, she is otherwise in a state of high alertness, feeling, hearing and remembering everything that is going on around her. The episode lasts for five minutes, longer than her typical cataplexieswhich often last only secondsbut shorter than her longest episode, which lasted 25 minutes. Then it ends, almost as abruptly as it began. She gets up from the floor, and her everyday life resumes. Cataplexy, the loss of skeletal muscle tone without loss of consciousness, is one of the defining symptoms of a puzzling neurological disorder called narcolepsy. The cataplectic attacks of narcolepsy are frequently prompted by laughter; other times, embarrassment, social interactions with strangers, sudden anger, athletic exertion or sexual intercourse may trigger an episode.

NARCOLEPSY Complaint of sleepiness or sudden muscle weakness Recurrent daytime naps or lapses into sleep that occur almost daily Overview Male = female Prevalence: about 1 in 4000 in North America and Europe (Hublin et al 1994) Onset: 20 to 40 years old, rarely before age 5 or after age 60. Polysomnography: short sleep latency, short REM sleep latency. Clinical symptoms Excessive sleepiness: Most common initial symptom Sleepiness is similar to those caused by sleep deprivation: most apparent in boring, sedentary situations and is partially relieved by movement. No amount of nighttime or daytime sleep produces full alertness. Cataplexy: Episodic weakness without altered consciousness lasting seconds to minutes. Precipitated by excitement or emotion: may occur several times daily or less than once per month. Laughter is the most common precipitant. Anger, embarrassment, excitement, and other forms of emotion also can induce it. Severe attacks produce complete paralysis of striated muscles, but respiratory muscles are spared.

Symptoms Diagnosis Treatment ... Complications Narcolepsy is a sleeping disorder characterized by periods of irresistible sleepiness. These "sleep attacks" happen without warning and can occur even after a good night's rest. They normally last about 20 minutes. After waking up, the person feels refreshed, only to feel sleepy again a few hours later. There is no known cause of this chronic sleep disorder, but it affects the central nervous system-the part of your brain that regulates sleep. People with narcolepsy seem to have problems routing signals that keep us awake. These messages seem to get blocked or take a detour. In other words, the signals sent by the brain do not always reach the body. It is estimated that over 200,000 Americans have narcolepsy. It is usually diagnosed in teenagers and young adults and is rarely discovered after age 40. Narcolepsy is normally a lifelong condition, but people can manage it and continue to be productive. There are no known medical consequences of this disorder, but it can cause traffic accidents and can affect a person's social and professional life.

What is narcolepsy?
Narcolepsy is a sleep disorder. The principal symptoms are excessive daytime sleepiness (EDS), cataplexy (loss of muscle tone), hallucinations, sleep paralysis, and disrupted nighttime sleep. Doctors also diagnose narcolepsy by measuring how quickly the patient falls asleep and how often rapid eye movements are present at or near the onset of sleep. Excessive daytime sleepiness occurs every day, regardless of the amount of sleep obtained at night. EDS is usually experienced as a heightened sensitivity (sometimes an almost irresistible susceptibility) to becoming sleepy or falling asleep, especially in sleep-inducing situations. Patients describe the problem as a sleepiness, tiredness, lack of energy, exhaustion, or a combination of these feelings, either continuously or at various times throughout the day. Sometimes sleepiness occurs so suddenly and with such overwhelming power that it is referred to as a "sleep attack." Some patients have several "attacks" each day. When the attack occurs during the day, sleep usually last for less than 30 minutes, but sometimes the patient stays asleep for several hours. bodyOffer(4355) Cataplexy is an abrupt loss of voluntary muscle tone, usually triggered by emotional arousal. Attacks can range in severity from a brief sensation of weakness to total physical collapse lasting several minutes. Hallucinations are intense, vivid, sometimes accompanied by frightening auditory, visual, and tactile sensations, and these hallucinations occur just when awakening or falling asleep. Occasionally, they are extremely difficult to distinguish from reality. Sleep paralysis is a momentary inability to move when waking up or falling asleep. This condition can be terrifying, especially if it occurs with a frightening hallucination.

What is narcolepsy?
John couldn’t hold down a job. It wasn’t that he wasn’t a hard, willing worker. He was. But John suffered from a strange, mystifying condition that caused him to suddenly and involuntarily lapse into a sleep. It didn’t matter where he wason the factory floor, driving to work, conversing in the lunch roombecause the condition would strike without warning. Other symptoms that affected John were sudden muscle weakness, sleep paralysis, and hallucinations. For his employers, John was too much of a liability. bodyOffer(5540) John is just one of thousands of people who suffer from narcolepsy. Long seen as a psychological illness, researchers have recently concluded that narcolepsy is, in fact, physical in nature. According to the American Journal of Psychiatry , “At the present time, narcolepsy is considered primarily to be an organic neurologic (brain) disorder rather than a psychogenic (originating in the mind) disorder.” So, just what causes narcolepsy? After 75 minutes of falling asleep, the average person will fall into the REM stage of sleep, which is also known as the dream state. During REM, a person’s muscles go perfectly limp. A person suffering from narcolepsy, however, plunges into the dream state immediately when he/she falls asleep. During the day, he/she will get uncontrollable urges to sleep and will again be transported directly into the REM stage of sleep. Narcolepsy can also manipulate the normal functioning of REM to the degree that upon waking, the REM physical changes are still operative. Therefore, the patient's body is still in a limp state. He/she may not even be able to move any of his/her muscles. The REM state can also be triggered by emotional stimulus: laughing, becoming angry, stress. Any of these things can cause a sufferer to collapse in a heap, as the muscles suddenly become weak and limp.

  • "I'm not tired...": At this point, there must be a second piano recording because I'm sure even Ben's hand can't stretch the chords of the treble clef. You can take the bottom tone of each chord up an octave and it sounds similiar, or leave out the repeating G's of the bass clef and use two hands for the treble.
  • "I don't remember all last year", etc.: A second piano can be heard faintly at this point, playing extremely fast in a high register. I ignored it and transcribed the more prominent part.
  • There are a few occassions throughout the song where there is significant banging in the lower register or the piano, but lacking the notation for such an effect, I left it out of the transcription.

INTRODUCTION
Narcolepsy is a lifelong disorder which may entail serious disablement. It is estimated to afflict more than 20,000 people in the United Kingdom many of whom are undiagnosed. No cure for Narcolepsy is yet known and no wholly adequate treatment is available. The cause is suspected to be a central nervous system defect but the exact mechanism is not clearly understood. There is no evidence of a psychological cause. Narcolepsy (or a predisposition to develop it) may be an inheritable characteristic passed down in families. The symptoms usually appear independently of one another. Typically, they are rather mild at the start and increase in severity at a gradual rate over a period of years. However, the development and the severity of symptoms are very variable in different individuals. The two primary symptoms of narcolepsy are excessive daytime sleepiness and cataplexy . In severe cases either can result in complete disability. The other symptoms appear less frequently than the primary symptoms and usually present no serious problems. They are sometimes experienced by people who are not narcoleptic.
EXCESSIVE DAYTIME SLEEPINESS (EDS) is usually the first symptom to be noticed. It may take the form of sleepiness, tiredness, lack of energy, or irresistable bouts of sleep. Although this pathological sleepiness is evident every day, its intensity varies through the day and the person suffering from it may find only the more severe episodes troublesome. Over twenty four hours the total time sleeping is usually no more than what is normal. The abnormality lies in the continuous susceptibility to sleepiness or to falling asleep and the circumstances in which this occurs.
