*For Medical Professionals only, registration required Non-neurogenic neurogenic bladder, also called Hinman's syndrome, characterized by clinical and urodynamic evidence of involuntary bladder outlet obstruction at the level of the external sphincter, in the absence of a definable structural or neurological cause. The bladder becomes large and trabeculated, in response to repeated contraction against a closed sphincter. Children present with incontinence, infrequent difficult voiding, faecal soiling and constipation. Urinary tract infections may also occur. Some believe the condition is psychogenic in origin, because excessive punishment for bedwetting results in a vicious cycle of sphincter tightness, poor bladder emptying and further enuresis. Defaecation is similarly affected, with resulting constipation, faecal soiling and encopresis. Radiological findings include hydroureteronephrosis , renal scarring, vesicoureteric reflux , large volume trabeculated bladder, conical dilatation of the posterior urethra, and persistent or intermittent narrowing in the region of the external sphincter ( Fig.1

Causes and symptoms Diagnosis ... Resources Definition Neurogenic bladder is a dysfunction that results from interference with the normal nerve pathways associated with urination. Description Normal bladder function is dependent on the nerves that sense the fullness of the bladder (sensory nerves) and on those that trigger the muscle movements that either empty it or retain urine (motor nerves). The reflex to urinate is triggered when the bladder fills to 300-500 ml. The bladder is then emptied when the contraction of the bladder wall muscles forces urine out through the urethra. The bladder, internal sphincters, and external sphincters may all be affected by nerve disorders that create abnormalities in bladder function. There are two categories of neurogenic bladder dysfunction: overactive (spastic or hyper-reflexive) and underactive (flaccid or hypotonic). An overactive neurogenic bladder is characterized by uncontrolled, frequent expulsion of urine from the bladder. There is reduced bladder capacity and incomplete emptying of urine. An underactive neurogenic bladder has a capacity that is extremely large (up to 2000 ml). Due to a loss of the sensation of bladder filling, the bladder does not contract forcefully, and small amounts of urine dribble from the urethra as the bladder pressure reaches a breakthrough point.

What is a neurogenic bladder?
Neurogenic bladder may also be called neuropathic bladder. The muscles and nerves of the urinary system work together to hold urine in the bladder and then release it at the appropriate time. Nerves carry messages from the bladder to the brain and from the brain to the muscles of the bladder telling them either to tighten or release. In a neurogenic bladder, the nerves that are supposed to carry these messages do not work properly, essentially paralyzing the bladder.
What causes neurogenic bladder?
In children a neurogenic bladder may be secondary to a birth defect or it may be acquired as the result of a different problem. The following are some of the most common causes of neurogenic bladder:
  • spina bifida - a defect that occurs during early fetal development. The defect consists of incomplete bony closure of the spinal cord through which the spinal cord may or may not protrude.
  • spinal cord trauma
  • central nervous system tumors

This case deals with the Chiropractic Management of a 12 year-old girl who showed a marked improvement in a chronic Autonomous Neurogenic Bladder (ANB) secondary to a surgically re-positioned myelomeningocele caused by Spina Bifida Manifesta while being treated for a functional scoliosis. This was one of the most satisfying cases I have handled. I believe it exemplifies what we, as Chiropractors, can accomplish with interprofessional cooperation for the benefit of our patients. ANAMNESIS: On August 20, 1991, Valentina C. and her father, presented themselves to my chiropractic office for a consultation for scoliosis. They were referred to me by Dr. Antonio M., a staff neurosurgeon at a local hospital. Valentina had recently had an orthopedic consultation for the spinal complaint and pain with weakness in dorsiflexion of the right foot when walking. When the orthopedist requested an EMG of the sciatic nerve and prescribed a L'NARD boot, Dr. M. raised an objection, intervened and requested they try a chiropractic approach to the problem (for complications which will become obvious later). REMOTE ANAMNESIS: Valentina, a twelve year-old student, was born with Spina Bifida Manifesta (L5-S3) and a myelomeningocele. At the age of four years, she underwent neurosurgery (by the same Dr. M.) to re-position the escaped contents of the spinal canal. The surgery was successful, but she developed a "clubfoot" and a spastic urethral sphincter, necessitating two surgical catheterizations following enteric bacterial infections of the urinary bladder with vesiculoureteral reflux.

Such cases of neurogenic bladder for example, in patients with diabetes, Parkinson's disease, or myelomeningocele (bulging out of the spinal cord through a defect in the spine) may not be associated with the severe irritative symptoms seen in bladder infections. Instead, patients may have severe, total incontinence, a rigid bladder and a nonfunctional sphincter mechanism. On the other hand, people with a neurogenic bladder may have a functional sphincter mechanism, but a rigid bladder that allows high pressures to build in the bladder and prevents urine drainage. In such instances, hydronephrosis (urine-caused swelling of the upper ureter) and renal insufficiency (inadequate kidney function) may result. Risk Factors Below are the two main risk factors for neurogenic urinary incontinence.
  • Pelvic Surgery
    Like pregnancy and childbirth, pelvic surgery can weaken and damage the pelvic floor muscles. As a result, the pelvic floor muscles may no longer be able to provide the necessary support to the bladder neck and urethra, and these structures may drop freely when downward pressure is applied. This condition, which is known as hypermobility, causes incontinence during physical activity, when the urethra cannot close tightly enough to resist increased abdominal pressure on the bladder.

I'm coming to you from Japan. My urologist finally came to the conclusion that I am suffering from neurgenic bladder. I didn't have a good night's sleep for longer than I can remember. Like I was awakened every hour or two. Talk about feeling miserable. Well, two days ago he prescribed a very mild muscle relaxant and an alpha blocker by the name of Halenol. I got up to urinate two times the night before last, and only one time last night. My doctor, wife and I regretfully believe that I will eventually need spinal surgery. Yeah, I hurt it leg pressing - believe it or not - 840 pounds. What an idiotic thing to do! But, that was way back in 1954 when I was 21 years old, and the superman syndrome had me firmly by the "you-know- whats". I've had trouble on and off since then finally culminating in the misery I'm experiencing now. Anyway, two days of relief is God sent. Maybe, it'll continue. I thought I'd tell you about it. OVERACTIVE BLADDER According to a two-year study undertaken at the University of North Carolina School of Nursing, Chapel Hill, older rural women can improve their urinary incontinence problem through behavioral management intervention. Molly Dougherty, PhD, RN and a professor at the university led the study group. The National Institute of Nursing Research, part of the NIH, funded the research. There were a total of 178 subjects involved in the study group, all of whom resided in Florida and were 55 years of age or older. The behavioral management intervention involved the women setting goals for their continence. Three behavioral phases in sequence were also established. The first sequence involved emptying the bladder at regular timed intervals. These timed intervals were gradually increased in timing, until it reached 2 1/2 hours or more. The women kept a bladder diary that included such information as descriptions of their liquid intake and intervals between urination. Another intervention that was used sought to strengthen the muscles to prevent leaking urine. This involved pelvic muscle (kegel) exercises.
