Leukopenia
Leukopenia is usually characterized by a reduced number of blood neutrophils, although a reduced number of lymphocytes, monocytes, eosinophils, or basophils may also contribute to the decreased total cell count. Neutropenia accompanied by monocytopenia and lymphocytopenia is often a more serious disorder than neutropenia alone.
NEUTROPENIA
(Granulocytopenia; Agranulocytosis)
A reduction in the blood neutrophil (granulocyte) count, often leading to increased susceptibility to bacterial and fungal infections. Ch. 151
Etiology
Acute neutropenia (occurring over a few days) often develops when neutrophil use is rapid and production is impaired. Chronic neutropenia (lasting months or years) usually arises from reduced production or excessive splenic sequestration of neutrophils. Neutropenia may be classified by whether it arises secondary to factors extrinsic to marrow myeloid cells or whether an intrinsic defect appears to be present in the myeloid progenitors (see Table 135-1 Secondary neutropenia: Drug-induced neutropenia has several underlying mechanisms (immune, toxic, idiosyncratic, or hypersensitivity reactions) and should be differentiated from the severe neutropenia that predictably occurs after large doses of cytoreductive cancer drugs or radiotherapy (see

    Febrile neutropenia remains a major complication of cancer chemotherapy administration and its frequency is likely to increase as the numbers of cancer patients are growing.
    Our strategy to cope with febrile neutropenic patients is definitely influenced by the availability of new agents that can be used for preventive or therapeutic purposes.
    In that respect, new antifungals and novel granulocyte stimulating agents will probably influence our clinical behaviour. On the other hand, the approach of febrile neutropenia is influenced by the characteristics of the patients who present it. It is now clear that the population of febrile neutropenic patients is an heterogenous one and that all these patients do not require the same attitude. Thus, exploring our therapeutic armamentarium and defining the factors which safely predict the course of febrile neutropenia is definitely needed for an optimal management of that condition. Furthermore, as new techniques continuously appear in cancer medicine and supportive care, the issue related to febrile neutropenia need to be periodically evaluated.

    Definition: A low number of neutrophils (type of white blood cell) in the blood. This can happen after a chemotherapy treatment, when the cells in the bone marrow can be damaged by the drug(s). Since these cells are very important in protecting the body from infection, a person receiving chemotherapy must be alert to the possible signs of low white count, as well as taking protective measures against infection. Management and evaluation of neutropenia should depend on the severity of the neutropenia and on the presence of symptoms suggesting infection. Patients with severe neutropenia with fever or signs of infection and those with evidence of malignant disease should be hospitalized. The cause of neutropenia should be determined, potential sources of infection evaluated and antibiotic therapy started. The diagnostic evaluation includes history of prior infection, a review of prior blood counts and exploration for evidence of infection or rheumatologic conditions. Patients with mild neutropenia and no evidence of infection may be monitored with blood cell counts for six to eight weeks. Neutropenia in children is likely to be benign and often resolves without intervention. Laboratory tests include microscopic examination of a peripheral blood smear, appropriate cultures and specific chemical, bacteriologic and viral studies, and serologic testing for collagen vascular disease. Bone marrow aspiration is needed when the cause of neutropenia is not clear, when the neutropenia does not resolve or if there is evidence of multiple cytopenias.

    Previous Next Management of the Febrile Child with Neutropenia and a diagnosis of a solid tumor. Aquino and colleagues examined the efficacy of using ciprofloxacin in the outpatient management of selected pediatric patients with fever during an episode of neutropenia. Patients were eligible for enrollment if they were one to 21 years of age, their malignancy was in remission, it had been seven or more days since their last chemotherapy course and they had an absolute phagocyte count of greater than 100 mm . In addition, the parents had to be reliable. After appropriate laboratory tests, the children received a single dose of intravenous ceftazidime (50 mg per kg). If the child was clinically well after being observed from two to 23 hours, enrollment was offered. Treatment began with ciprofloxacin therapy (20 mg per kg per day in two divided doses). The children were maintained on the ciprofloxacin therapy until they were afebrile for 24 hours, had negative blood cultures and evidence of bone marrow recovery. If the children appeared toxic, had positive blood cultures or were febrile for five or more days, they were admitted for inpatient treatment. Eighty-nine percent of the children were successfully treated as outpatients. Four percent of the children were admitted to a hospital because of positive blood cultures. Two of 45 children had to be admitted to the hospital because of noncompliance with the outpatient regimen. All those admitted had uncomplicated hospitalizations and recovered fully.
