This disorder is one common cause of nephrotic syndrome in children affecting 2 to 3 children per 100,000 population under age 16 in the US. Minimal change disease is also seen rarely in adults. The cause is unknown but may be related to an autoimmune illness. Risk factors include a history for a immune disorder, recent immunization, or a bee sting. Diagnosis is made by renal biopsy . Treatment include systemic corticosteroids which are usually quite effective in curing this disease. Other medications include chlorambucil and cyclophosphamide. In most cases, a moderate protein diet (1 gram protein per Kg body weight per day) will be recommended. Salt (sodium) restriction can be helpful to reduce swelling and vitamin D is usually supplemented.

It refers to a histopathologic lesion in the glomerulus that almost always is associated with nephrotic syndrome. It typically is a disease of childhood, but it also can occur in adults. Pathophysiology: It is postulated that MCD is a disorder of T cells, which release a cytokine that injures the glomerular epithelial foot processes. This, in turn, leads to a decreased synthesis of polyanions. The polyanions constitute the normal charge barrier to the filtration of macromolecules such as albumin. When the polyanions are damaged, leakage of albumin follows. The identity of this circulating permeability factor is uncertain, although it is postulated that it may be hemopexin. Some of the cytokines that have been studied in MCD are interleukin-12 (IL-12) and interleukin-4 (IL-4). IL-12 levels have been found to be elevated in peripheral blood monocytes during the active phase and normalized during remission. Interleukin-18 (IL-18) can synergize with IL-12 to selectively increase the production of vascular permeability factor from T cells. In addition, levels of IL-4 and CD23 (a receptor for immunoglobulin E) have been found to be elevated in peripheral blood lymphocytes.

Bacterial fruit blotch of watermelon In Queensland, the fruit blotch disease of watermelon has occurred sporadically over the last 25 years. We have regarded it as a minor disease but recently fruit blotch has shown that, given suitable conditions, it can cause severe damage. Since mid 1995, blotch has been noticed in the Ayr, Bowen, Chinchilla and Emerald districts. The severity of the disease is greatly modified by weather conditions and outbreaks are often associated with periods of wet, windy weather. Hail damage can greatly increase disease severity. In 1996, a second strain which also infects rockmelon and honey dew was identified and these crops must now be considered at risk from this disease. Figure 2.
