Congenital nystagmus occurs at birth or in the first few months of life. It is usually associated with an underlying visual disorder, such as albinism in which there is poor pigmentation of the eyes, skin and hair. Congenital nystagmus may occur, however, in association with a wide range of infantile visual disorders. Accurate diagnosis for nystagmus is necessary to ensure the child has adequate help to deal with the disorder. Occasionally no visual disorder can be detected and the nystagmus is called congenital idiopathic nystagmus or congenital motor nystagmus Recent research shows that nystagmus occurs in approximately 1 in 2,000 births. Some of these children will, however, have multiple disabilities. Inheritance patterns
These depend upon the underlying visual disorder. Congenital idiopathic nystagmus may also be inherited. Genetic counselling may be sought in cases of nystagmus. Pre-natal diagnosis

nystagmus. DESCRIPTION. nystagmus is the involuntary, rhythmical, oscillationsof one or both eyes, in any or all fields of gaze.
http://www.aniridia-network.net/conditions/conditions/nystagmus.html
NYSTAGMUS DESCRIPTION Nystagmus is the involuntary, rhythmical, oscillations of one or both eyes, in any or all fields of gaze. It can be
pendular: undulating movements of equal speed, amplitude, and duration, in each direction
or
jerky: with slower movements in one direction, followed by a faster return to the original position. Movements may be horizontal: from ulternating between, left and right
Vertical: ulternating between up and down
oblique: moving in a diagonal plain
In a circle. or any combination of these: Generally, the faster the rate, the smaller the amplitude (and vice versa). The defect is classified according to the position of the eyes when it occurs. Grade I occurs only when the eyes are directed toward the fast component; grade II occurs when the eyes are also in their primary position; grade III occurs even when the eyes are directed toward the slow component. CAUSE The cause of nystagmus is unknown. Reduced acuity is caused by the inability to maintain steady fixation. Head-tilting may decrease the nystagmus and is usually involuntary (toward the fast component in jerky nystagmus, or in such a position to minimize pendular nystagmus). Head nodding often accompanies congenital nystagmus. Congenital nystagmus of the pendular type usually accompanies congenital visual impairment (e.g., corneal opacity, cataract, albinism, aniridia, optic atrophy, chorioretinitis).

An additional test is the horizontal gaze nystagmus ("HGN"). When used in combination with divided attention tests, HGN helps police officers correctly distinguish suspects who are under the influence of alcohol from those who are not. The test is based on the fact that alcohol affects the automatic tracking mechanisms of the eyes. Nystagmus is defined as "an involuntary rapid movement of the eyeball, which may be horizontal, vertical, rotatory, or mixed." Alcohol slows down the eyes' ability to rapidly track objects and causes to eyes to oscillate, or "jerk", before they normally would in a sober person. Alcohol stimulates the nerve endings, making nystagmus more pronounced in intoxicated persons. As a person's blood alcohol concentration increases, the eyes will "jerk" sooner as they move to the side. The HGN test claims to gauge intoxication by measuring the involuntary oscillation of the eyes. Nystagmus may be caused by many other factors. The court, in

Nystagmus What is nystagmus?
Nystagmus means that your eyes move without you controlling them and often without you knowing it. They usually move from side to side but it can also be up and down or even sometimes in a circular movement. This means that you may not be able to see as clearly as other people. Some babies are born with nystagmus or it can develop when they are only a few months old. People can also have nystagmus when they are older.

What is Nystagmus?
Nystagmus is generally described as an involuntary movement of the eyes, which reduces vision. The movement is usually side to side (but can be up and down or circular motion) and can be either jerk or pendular. (See this site for animations that shows the difference www.geocities.com/HotSprings/Spa/3699/Appear.html ). Nystagmus is neither contagious or infectious, but it is thought to sometimes be hereditary. Normal (physiological) nystagmus occurs for example when a passenger of a train watches as telegraph poles pass the window. His/her eyes will travel one way, and then jerk in the opposite direction to begin watching the next pole. There are over 40 different types of nystagmus ( www.theberries.ns.ca/BOTW_archives/nystagmus ) but the main division is between Congenital and Acquired Nystagmus.
  • Congenital nystagmus is thought to be present at birth, but is usually not apparent until the baby is a few months old.
  • Acquired nystagmus occurs later than 6 months of age, and can be caused by a stroke, disease such as multiple sclerosis, or even a heavy blow to the head.

Causes of Nystagmus
Albinism is considered to be the most common condition accompanying nystagmus, but often the cause of congenital nystagmus is unknown. Albinism is a condition in which there may be a lack of colour in all of, or any one of the skin, hair and eyes. Nystagmus can run in families, and it has been known to be passed down to the next generation, but can also skip generations. The lack of understanding of the causes of nystagmus can only reinforce the complexity of the disease. [Return to top] [Home] Last Updated Nov 2001

Nystagmus is a symptom of many different disorders. Your health care provider will take a careful history and perform a thorough physical examination, which will emphasize the nervous system and inner ear. Questions asked in a medical history may cover the following areas:
  • Time pattern
    • When was it first noticed? How often does it occur? Has it ever happened before? Is it getting better, worse, or staying the same?
    Quality
    • Are there side-to-side eye movements?

Abstract
Background
Pendular Nystagmus commonly occurs in congenital and acquired disorders of Myelin. Objective
To characterize the Nystagmus in 3 siblings with an infantile form of an autosomal recessive peroxisomal assembly disorder causing LeukoDystrophy. Design
We examined visual function and measured Eye movements using infrared Oculography. We noted changes in eye speed and frequency before and after the administration of Gabapentin ( Neurontin ) to 1 patient. Results
All 3 siblings showed Optic Atrophy and Pendular Nystagmus that was predominantly horizontal, at a frequency of 3 to 6 Hz, with phase shifts of 45 degrees to 80 degrees between the oscillations of each eye. Gabapentin administered to 1 child caused a modest improvement of Vision and the reduction of the velocity and frequency of oscillations in the Eye with worse Nystagmus. Conclusion
The Pendular Nystagmus in these patients was due to their LeukoDystrophy and may have a similar PathoGenesis to the oscillations seen in other disorders affecting Central Myelin
