Announcing a New Education Program From NOTES NAASO's Obesity Treatment Education System (NOTES) is pleased to announce the availability of up to 2 hours of Continuing Medical Education on the Office Management of Obesity for primary care physicians and others treating obesity. This course provides knowledge pertinent to the standard management of obese patients and addresses questions such as:

  • Who is a good candidate for surgery?
  • Which medications work in which patients?
  • How can I get reimbursed for obesity treatment?
NOTES was launched in 2001 to bring together NAASO's leadership in science with its expertise in the treatment of obesity. TODAY’S CHALLENGE in health care is to prevent or diagnose, treat, and manage acute or chronic diseases that affect many people while controlling cost, maintaining standards of care, and improving quality of life.1 Obesity is one such treatable disease because of its association with physical, psychosocial, and disability costs that affect quality of life.2-4 In the United States, obesity reaches epidemic proportions, with an estimated 300,000 deaths per year attributed to it.5,6 For the severely obese, it is a chronic, heterogeneous, multisystemic disease that has many equivocal causes and consequences.4 As with non–insulin-dependent diabetes,7 obesity treatment is not considered or managed as a serious problem and presents difficulties in conforming to the current health care system of treatment and reimbursement.8 Yet, it is expensive to society.9 When compared to our total yearly health care bill of approximately $884 billion (in 1993 dollars),10 the 1990 direct and indirect costs associated with the disease burden, death, disability, products, and services associated with obesity totaled approximately $100 billion.11,12 The prevalence of adults participating in some kind of weight-loss program is 25% of men and 45% of women, or one in three adults.12-14 Purchasers of obesity treatments are increasingly scrutinizing the associated costs, quality, and outcomes of programs.15,16 A proposal has been made to accredit weight-loss treatment programs in a manner similar to that used in accrediting hospital programs.17

Childhood Obesity Obesity in children and adolescents is a serious issue with many health and social consequences that often continue into adulthood. Implementing prevention programs and getting a better understanding of treatment for youngsters is important to controlling the obesity epidemic. Many parents are rightly concerned about their child's weight and how it affects them. They look for specific answers for prevention and treatment options. Unfortunately, the state of the science is a lot less precise than we would like. Are kids too concerned about their weight? What are the best strategies for prevention? What treatments work over a long time? Researchers are trying to answer those and many other questions. In many cases, common sense works well. In situations where there are serious health, psychological or social problems, parents should seek out the best possible advice. Severe obesity is a long term condition that is very difficult to treat. Surgery to promote weight loss by restricting food intake or interrupting the digestive process is an option for severely obese people. A body mass index (BMI) above 40which means about 100 pounds of overweight for men and about 80 pounds for womenindicates that a person is severely obese and therefore a candidate for surgery. Surgery also may be an option for people with a BMI between 35 and 40 who suffer from life-threatening problems (for example, severe sleep apnea or obesity-related heart disease) or diabetes.
