The Weight-control Information Network (WIN) is a national information service of the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK), National Institutes of Health (NIH). WIN was established in 1994 to provide health professionals and consumers with science-based information on obesity, weight control, and nutrition. WIN has also developed the Sisters Together: Move More, Eat Better Media program that encourages Black women 18 and over to maintain a healthy weight by becoming more physically active and eating healthier foods.

Introduction
The prevalence of obesity is rising to epidemic proportions
For example, the prevalence of obesity has increased by about 10-50% in the majority of European countries in the last 10 years and currently affects 77% of males living in urban areas of Western Samoa in the Pacific.
The health, economic and psycho-social consequences of the increasing incidence of obesity are substantial. Obesity is associated with numerous health complications which range from non-fatal debilitating conditions such as osteoarthritis , to life threatening chronic diseases such as Coronary Heart Disease diabetes , and certain cancers . The psychological consequences of obesity can range from lowered self-esteem to clinical depression . Recent estimates suggest that between 2 to 8% of the total sick care costs in Western countries are attributable to obesity. Despite the high prevalence of obesity and the many advances in our understanding of how it develops, present management strategies have persistently failed to achieve long term success. Table 1. Introduction
Obesity in dogs is a serious medical problem. Fat dogs are more at risk in surgery, more prone to injury, and have more stress on their heart, lungs, liver, kidneys, and joints.
A guide to determine if Pal is malnourished
Overweight dogs have increasing amounts of fat over the ribs and minimal or absent waist and tuck-up. Obese dogs have heavy fat over the ribs, along the spine, and around the tail. Morbidly fat dogs have protruding abdomens and fat deposits throughout their bodies. Owners can examine their own dogs for a fat layer on the ribs and the visibility of a waist and tuck-up. Breed characteristics should be taken into account of course, as some breeds have more prominent ribs and tuck-ups when in optimum condition. In general, dogs with protruding bones and the appearance of muscle loss are too thin.

Between 5-25 percent of children and teenagers in the United States are obese (Dietz, 1983). As with adults, the prevalence of obesity in the young varies by ethnic group. It is estimated that 5-7 percent of White and Black children are obese, while 12 percent of Hispanic boys and 19 percent of Hispanic girls are obese (Office of Maternal and Child Health, 1989). Defining Obesity in Children and Adolescents
Obesity is defined as an excessive accumulation of body fat. Obesity is present when total body weight is more than 25 percent fat in boys and more than 32 percent fat in girls (Lohman, 1987). Although childhood obesity is often defined as a weight-for-height in excess of 120 percent of the ideal, skinfold measures are more accurate determinants of fatness (Dietz, 1983; Lohman, 1987). A trained technician may obtain skinfold measures relatively easily in either a school or clinical setting. The triceps alone, triceps and subscapular, triceps and calf, and calf alone have been used with children and adolescents. When the triceps and calf are used, a sum of skinfolds of 10-25mm is considered optimal for boys, and 16-30mm is optimal for girls (Lohman, 1987).

Web posted at: 6:33 p.m. EDT (1833 GMT) From CNN Medical Corrrespondent Elizabeth Cohen ATLANTA (CNN) The National Institutes of Health has some advice for people who are no more than 30 pounds overweight: quit trying to lose weight. The obesity experts say that being 30 pounds overweight isn't all that dangerous, and studies show most people gain it back anyway. The NIH this week published information relating to the strict set of guidelines it set out in June for determining who is considered overweight. The guidelines, which were seen as controversial,are based on the Body Mass Index (BMI) , a height-to-weight formula that ignores whether the weight is from fat or muscle. It also ignores whether someone has a large or small frame. The weights are the same for men and women. The new NIH guidelines mark a turning point in the way doctors think about weight loss, Dr. Xavier Pi-Sunyer , chairman of the NIH obesity panel and director of endocrinology at St. Luke's-Roosevelt Hospital in New York, said in an interview with CNN.

Weight Control and Diet December 2001 WHAT IS OBESITY? Stable weight depends on an even balance between energy intake from food and energy expenditure. Energy expenditure occurs during the day in three ways:
  • As energy expended during rest ( basal metabolism ). This accounts for about two-thirds of expended energy, which is generally used to maintain body functions, such as maintaining body temperature and muscle contractions in the heart and intestine.
  • As energy used to metabolize food ( thermogenesis ), accounting for about 10% of expended energy.
  • As energy expended during physical activity.
When a person's caloric intake exceeds his or her energy expenditure, the body stores the extra calories in the fat cells present in adipose tissue. These adipose cells function as energy reservoirs, and they enlarge or contract depending on how people use this energy. If people do not balance energy input and output by adopting healthy eating habits and regular exercise, then fat builds up, and they may become overweight.
Measurement of Obesity Obesity is determined by measurement of body fat, not merely body weight. People might be over the weight limit for normal standards, but if they are very muscular with low body fat, they are not obese. Others might be normal or underweight, but still have excessive body fat. Different measurements and factors are used to determine whether or not a person is overweight to the degree that it threatens health:

Controlling the global obesity epidemic
The challenge
At the other end of the malnutrition scale, obesity is one of today’s most blatantly visible – yet most neglected – public health problems. Paradoxically coexisting with undernutrition, an escalating global epidemic of overweight and obesity – “globesity” – is taking over many parts of the world. If immediate action is not taken, millions will suffer from an array of serious health disorders.
Obesity is a complex condition, one with serious social and psychological dimensions, that affects virtually all age and socioeconomic groups and threatens to overwhelm both developed and developing countries. In 1995, there were an estimated 200 million obese adults worldwide and another 18 million under-five children classified as overweight. As of 2000, the number of obese adults has increased to over 300 million. Contrary to conventional wisdom, the obesity epidemic is not restricted to industrialized societies; in developing countries, it is estimated that over 115 million people suffer from obesity-related problems.

Obesity: The excessive accumulation of body fat. Table 1-5 ). Now, it is usually defined in terms of the body mass index (BMI)weight (in kilograms) divided by the square of the height (in meters).
Epidemiology
The prevalence of obesity in the USA is high and rising higher. In the past decade, the overall prevalence rose from 25 to 33%, an increase of 1/3. Prevalence varies significantly by sex, age, socioeconomic status, and race (see also Obesity in Ch. 275). Prevalence is 35% among women and 31% among men, and it more than doubles between the ages of 20 and 55. Among women, obesity is strongly associated with socioeconomic status, being twice as common among those with lower socioeconomic status as it is among those with higher status. Although prevalence among black and white men does not differ significantly, obesity is far more common among black than among white women, affecting 60% of middle-aged black women compared with 33% of white women.
Etiology
In one sense, the cause of obesity is simpleexpending less energy than is consumed. But in another sense, it is elusive, involving the regulation of body weight, primarily body fat. How this regulation is achieved is not yet fully understood.
