    DEFINITION:
    An osteochondritis involving the patellae resulting in knee pain and swelling.
    EPIDEMIOLOGY:

      PATHOGENESIS:
      1. Background
      • most patients show a mild degree of patellofemoral malalignment which with activity causes instability of the patella and gradual destruction of the patellar cartilage
      • considered an overuse syndrome not involving avascular necrosis or an inflammatory process (develops over a period of time)
      • usually unilateral but at times bilateral
      CLINICAL FEATURES:
      1. Knee
      • pain - anterior, underside of patellae, acute and chronic
      • crepitance - worse with flexion (climbing stairs, running)
        • detected with manipulation of patellae
      • instability - unable to keep knee in flexed position for any length of time, history of knee giving out
      • positive patellar inhibition test - patient refuses to actively extend knee when patellae compressed against the femoral condyles - displacement of patellae with knee extension
      • no decreased range of motion of affected knee
      • chronic pain may result in disuse atrophy of the quadriceps

    INTRODUCTION
    Advances in arthroscopy and knee replacement have led to high expectations and unprecedented demand. The knee is the most commonly injured joint and can often be treated conservatively, but many doctors, including consultants, frequently get the diagnosis wrong on first seeing a patient. Accurate identification of patients who need an orthopaedic opinion is needed. The aim of this report is to clarify history-taking and examination of the painful knee and to indicate appropriate management.
    THE HISTORY
    In acute knee injuries the history is all important. For example , is typical of rupture of the anterior cruciate ligament (ACL): examination in such instances is limited due to swelling and guarding. The average delay in diagnosis is over 18 months! In contrast, examination is more revealing in patients with chronic knee problems when the joint is nicely relaxed. Indeed, it is the history which may be misleading. For instance

    Abstact
    A 13 year old boy who has sustained a left lateral knee contusion while playing football, followed 2 weeks later by a mild knee blow inflicted bu a school bully. X-rays revealed an osteogenic sarcoma. This case illustrated the importence of the affected extremity when the diagnosis in doubt.
    The possibilites in injury are, a lateral collateral ligament strain or tear, physeal plate fracture, or a possible ACL,PCL, or lateral maniscus tear. Other possibilities include, discoid miniscus, patellofemoral disorder in a dislocation, medial patellar retinaculum symptoms, plica syndrome, and iliotibial band friction syndrome.
    Test results, by X-ray, demonstrate a permative bone lesion on the lateral aspect of the distal femur and a Codman's triangle superiorly-a trianglular area where the posterium, elevated by the bone tumor, rejoins the cortex of a normal bone. A CT scan showed a permative lesion of the postrolateral aspect if the distal femur extending into the soft tissue and raising the cortex laterally. A bone scan was normal, with the eception of greatly increased uptaske at the distal femur.
    The differential diagnosis is a malignant tumor, or ostiogenic sarcoma. The reason for the differential diagnosis is the athletes sex and age. This usually occurs in males from the ages from 10-20. These tumors may be secondary to trama or exposure to radiation. This tumor occurs in the metaphyseal area of lomg bones- the sight of the patients problem. Upper femur and proximal tibia, and the hemerus. The patient will experiance pain, swelling, and weight loss. In this case there was none.

    The pain associated with this disorder may worsen with descending stairs climbing or deep squating. Sitting with your knees flexed for a long period of time in a car or movie theatre may also aggravate the symptoms. Causes A variety of underlying anatomic abnormalities may result in patellofemoral pain syndrome. It can be caused by softening or cracking of the articular cartilage under the kneecap. This is known as chondromalacia. A malalignment of the kneecap or tight tissue around the kneecap can also create tension that may lead to patellofemoral pain syndrome.
    Usually, this disorder arises without a history of significant trauma. Occasionally, the pain may begin after a blow to the knee or a minor injury.

    Causes
    Weakness of the quadriceps muscle (especially the vastus medialis obliqus or VMO) Softening of the retropatellar cartilage (chondromalacia patella) Patellar malalignment (tilting or lateral tracking) Excessive tightness of the lateral retinaculum Overuse (such as intense running without proper training) Direct trauma (A blow to the anterior aspect of the knee cause a contusion) Diagnosis
    HISTORY
    Complaints of pain in the front of the knee Diffuse pain, not typically localized Pain made worse by descending stairs or prolonged sitting Giving way or grinding may be other complaints
    PHYSICAL EXAM
    Check for effusion (knee swelling) Look for quadriceps atrophy (compare to the opposite side) especially the VMO Check patellar tracking with knee flexion and extension (patellar should stay centered) Palpate for area of maximal tenderness Check for restricted medial patellar motion or excessive lateral patellar motion Check quadriceps flexibility (heel to butt with patient on stomach, compare to other side)
