It is an autoimmune disorder in which the immune system produces antibodies against specific proteins in the skin and mucous membrane. These antibodies produce a reaction that leads to a separation of epidermal cells (acantholysis). The exact cause of the development of antibodies against the body's own tissues (autoantibodies) is unknown. Sometimes pemphigus appears due to reactions to medications, although this is rare. Pemphigus may be a side effect of ACE inhibitors (such as Elanapril) or chelating agents (such as penicillamine). Pemphigus is uncommon. It occurs almost exclusively in middle-aged or older people, of all races and ethnic groups. About one-half of the cases of pemphigus vulgaris begin with

Pemphigus Wat is het? Pemphigus is een zeldzame blaarvormende ziekte. In Nederland wordt jaarlijks bij ongeveer 120 mensen de aandoening vastgesteld. Patienten met pemphigus zijn meestal van middelbare leeftijd. De blaren kunnen voorkomen op de huid en op de slijmvliezen, zoals het mondslijmvlies. Pemphigus gaat vaak met jeuk gepaard. Er worden 3 soorten pemphigus onderscheiden: pemphigus vulgaris (komt het meeste voor) pemphigus foliaceus paraneoplastische pemphigus (een vorm die wordt gezien bij patienten met kanker) Hoe ontstaat het? Pemphigus ontstaat door een autoimmuun reactie, dat is een reactie van het lichaam tegen het eigen weefsel. Bij pemphigus is deze reactie gericht tegen bepaalde bestanddelen van de opperhuid, waardoor de opperhuid plaatselijk loslaat en blaren gaat vormen. Waarom de autoimmuun reactie ontstaat is nog niet bekend. In sommige gevallen wordt het veroorzaakt door geneesmiddelen (penicillamine, captopril). Bij pemphigus vulgaris zijn de blaren vrij slap en met vocht gevuld. De blaren gaan door stoten of krabben makkelijk stuk. Advanced Search Tips from Other Journals Previous Next Insights into Pathophysiology and Treatment of Pemphigus Pemphigus occurs in three major types, all characterized by blistering and ulceration caused by the action of autoantibodies against adhesion molecules that anchor keratinocytes to one another and to the underlying basement membrane. The target of the IgG autoantibodies is a group of transmembrane proteins called desmogleins. A review by Nousari and Anhalt emphasizes the differences in pathophysiology, clinical features, optimal therapy and prognosis between pemphigus vulgaris, paraneoplastic pemphigus and pemphigus foliaceus. In pemphigus foliaceus, blisters occur only on the skin, and only antibodies to desmoglein 1 are found. The condition may be drug induced, particularly by penicillamine. A geographically limited form of pemphigus foliaceus also occurs in clusters, presumably related to an unknown environmental agent. The same therapeutic agents are used, but treatment is usually less aggressive and the prognosis better than in patients with pemphigus vulgaris. Patients with paraneoplastic pemphigus produce antibodies against a range of structural proteins, including desmogleins. The condition is progressive and involves internal organs, especially the pulmonary system, in addition to skin and mucous membranes. Paraneoplastic pemphigus occurs with neoplasm, usually a B-cell lymphoproliferative disorder, but treatment of the malignant neoplasm does not appear to alter the progression of the condition. Death usually occurs within two years. Combination therapy with steroids and cyclosporine may alleviate symptoms.

Background Pemphigus vulgaris is the most common variant of pemphigus, accounting for 80% of cases. These patients may have a dramatic presentation with flaccid blisters on an erythematous base occurring on the trunk, groin, axilla, scalp, face, and points of pressure. Oral and mucosal lesions which may be the initial presentation are seen in 80-90% of cases. Affected mucosal surfaces include the esophagus, vulva, cervix, anus, larynx, and conjunctiva. There may be an early urticarial, non-blistering phase which may be difficult to diagnose both clinically and histologically. The association with internal malignancies and other diseases has been well-studied. Associations have included Castleman's disease, myasthenia gravis, thymoma, scleroderma, Grave's disease, and oral herpes infection. The disease may be exacerbated or initiated by several agents, including minor trauma, ionizing radiation, burns, PUVA therapy, and pesticides. Offending drugs include rifampicin, norfloxacin, ampicillin, interleukin-2, penicillin, captopril, enalarpril, and drugs with thiol groups. A new variant, paraneoplastic pemphigus , has a unique set of serologic and histologic changes which should be excluded.
