WHAT ARE PEPTIC ULCERS?
The stomach, intestines, and digestive glands produce acid and various enzymes, including pepsin, that break down and digest the starch, fat, and protein in food. The stomach itself is composed mostly of protein and must be protected from the same acid and enzymes, or it too can be attacked and broken down. Ulcers develop when an imbalance occurs between the digestive juices used by the stomach to break down food and the various factors that protect the lining of the stomach and duodenum (the part of the small intestine that adjoins the stomach). A peptic ulcer is an open sore or raw area in the lining of the upper part of the small intestine (duodenal ulcer) or the stomach (gastric ulcer). They average between one-quarter to one-half inch in diameter. In the US, duodenal ulcers are three times more common than gastric ulcers.
The primary digestive juices, hydrochloric acid and pepsin, are powerful substances necessary for breaking food down for use by the body. Acid is always present in the stomach, but, except at meal times, in relatively small amounts. A common misbelief is that excess acid is solely responsible for producing ulcers. Hydrochloric acid, secreted in the stomach, does indeed play a part in the development of ulcers, but it is not the only culprit. Pepsin, the other major digestive fluid, is an enzyme that breaks down whatever proteins are presented to it, including, if exposed, the stomach's own tissues.

Stomach ulcers or Peptic Ulcers are caused by the action of stomach acid on the lining of the mucous membranes of the stomach wall. Symptoms vary but can include: a localised gnawing, burning pain, heartburn, local tenderness, nausea, vomiting and diarrhoea. The pain usually begins just after eating or within twenty minutes of a meal. Heavy drinking, smoking, stress, and taking aspirin may all contribute to the formation of an ulcer. The following complementary therapies may be used in the treatment of the stomach ulcer:
Diet and Nutrition:
Cut down on any acidic forming foods such as coffee, red meat, salt, hot, spicy foods (eg.curries), citrus fruits, vinegar and picked foods, and steer clear of aspirin. Replace with more alkaline foods: lots of fresh fruit and vegetables, rice, lean animal and vegetable proteins. Drink plenty of mineral water to keep your stomach acid at the correct level - at least 8 glasses a day between meals.
Western Herbalism:
Take a decoction of slippery elm (available from health foods shops). This is a demulcent which means it has a soothing effect on the mucous membranes of the gut. Take 1/2 tsp in warm water 4-6 times a day.

Nearly 25 % of people in our community suffer from dyspepsia, which means symptoms of bad digestion like abdominal pain or burning, fullness and gas. These symptoms are commonly referred to as those of "acidity " and most people resort to self medication. It is important to remember that any new dyspepsia of more than one month duration should be investigated. Further, at age above 45 years and the presence of "alarm" symptoms like loss of appetite, weight loss and difficulty in swallowing investigations are mandatory. Peptic ulcer is responsible for about 10 % of these symptoms. It is important for patients suffering from dyspepsia to be evaluated by their physician as apart from ulcers, cancer of stomach may also present in the same way. Peptic ulcer implies the development of a breach in the lining of stomach or upper small intestine called duodenum and to the naked eye it looks like a wound. This illness effects nearly 10% of people in our country. It is commonly found in young people at the prime of their age and has been said to be associated with " hurry, worry and curry". The factors responsible for causing ulcers include cigarette smoking, use of painkiller drugs, physical and mental stress and a diet rich in chilies, coffee, colas and rice. However recent research has shown that the most important factor is the presence of a spiral shaped bacteria in the stomach called Helicobacter pylori. This bacterium enters the stomach by the oral route and is usually acquired at a young age. The organism may be present in about 40 % of healthy people but transformation into disease like peptic ulcer and stomach cancer occurs only in few.

What is a stomach or duodenal ulcer?
An ulcer is an open sore, or lesion, usually found on the skin or mucous membrane areas of the body. An ulcer in the lining of the stomach or duodenum, where hydrochloric acid and pepsin are present, is referred to as a peptic ulcer
  • When the ulcer is in the stomach, it is called a gastric ulcer
  • When the ulcer is in the duodenum, it is called a duodenal ulcer
What causes gastric and duodenal ulcers?
In the past it was believed lifestyle factors, such as stress and diet caused ulcers. Later, researchers determined that stomach acids hydrochloric acid and pepsin contributed to ulcer formation. Today, research shows that most ulcers (80 percent of gastric ulcers and 90 percent of duodenal ulcers) develop as a result of infection with a bacterium called Helicobacter pylori (H. pylori).

What is an ulcer?
An ulcer in the stomach (A). Duodenum - (B). Oesphagus - (C). An ulcer is damage to the inner lining (the mucosa) of the stomach or the upper part of the intestine (duodenum). A bacterium, Helicobacter pylori, is the main cause of ulcers in this area.
Why do people get ulcers?
The most common cause is infection with Helicobacter pylori bacteria
The second most common cause is damage inflicted by aspirin or non-steroidal anti-inflammatory drugs (NSAIDs, eg diclofenac or naproxen ) used by many for arthritis , rheumatism, backache headaches and period pain
Ulcers can also occur in people weakened by severe disease (such as chronic respiratory disease or major trauma). This is thought to result from poor oxygenation to the lining of the stomach. Occasionally (in Europeans), a stomach ulcer is caused by cancer and rarely, some other specific illness is found to be responsible. Such conditions include:

A peptic ulcer is a crater-like open sore in the lining of parts of the digestive tract. There are two types:
  • Stomach ulcers, also called gastric ulcers, occur in the stomach. Duodenal ulcers occur in the duodenum, the uppermost part of the small intestine near the stomach.

What are the main symptoms?
The classic and most common symptom of peptic ulcers is pain. Ulcer pain is thought to affect at least 70% of people with duodenal ulcers and around 50% of those with stomach ulcers. This pain may:
  • Be a gnawing, burning sensation in the abdomen between the breastbone and navel. Feel like a constant hunger or sensation of pressure in the abdominal area. Range from a mild ache to severe discomfort. Come on after a meal, anywhere from 30 minutes to a couple of hours afterwards. Last anywhere from minutes to hours. Be felt in a wide area, radiating from the abdomen and spreading to the back and sides. Be relieved by eating or taking antacids (e.g. Rennies, Gaviscon or Milk of Magnesia).

    Dr. Rita Louise Peptic Ulcers A Peptic ulcer is a term used to describe a point in the lining of the stomach or duodenum that has been eroded, leaving an open wound in the associated tissues that bleed.
