Dear Martha RN,
There arer several ways to approach your brother's problems, depending om which symptom is the primary focus. FRom the perspective of ulcer disease, the common causes include aspirin, H. pylori infection, smoking and a gastrin-secreting tumor. Although ulcer disease can be associated with nausea, vomiting and abdominal pain, the skin rash and colitis would not be expected problems. Conditions that involve multiple organs (in this case, gastritis, colitis, skin rash)are often the presentation for a vasculitis. This can be an autoimmune condition (e.g. like lupuis) or secondary to medications.
Given th eseverity of symptoms e.g. weight loss, dehydration, it is important that you continue to seek medical care.

A large amount of excess acid is produced in response to the overproduction of the hormone gastrin, which in turn is caused by tumors on the pancreas or duodenum. These tumors are usually malignant, must be removed and acid production suppressed to relieve the recurrence of the ulcers.

Peptic ulcer refers to an area of the stomach or duodenal lining which becomes eroded by the stomach acid. These are known as stomach and duodenal ulcers, collectively known as peptic ulcers.
Symptoms
You may notice no symptoms although you have an ulcer, but frequently they are associated with indigestion or pain in the upper abdomen or even lower chest. This pain may be worse before or after eating and frequently wakes you in the early hours of the morning. The pain may go to the back or sometimes be perceived elsewhere. If an ulcer bleeds you may vomit blood or partially digested blood (which looks like coffee grounds) or pass black stools (melaena), which contain changed blood, when you go to the toilet. If any of these happen call the doctor immediately (even out of hours).
Causes
Many factors make you more likely to have an ulcer.
  • It often seems to run in families.
  • Smoking increases the risk.
  • Heavy drinking.
  • Certain drugs eg aspirin and other anti-inflammatory drugs
  • We now know that a large percentage of people who have ulcers have a germ (Helicobacter pylori) in their stomach which they probably picked up many years previously, which makes the ulcer more likely to occur and more difficult to heal.
Diagnosis
Your doctor may treat you on the assumption of the diagnosis, but may well arrange either a barium meal X-Ray or a visual inspection using a fibre-optic tube passed down the throat (upper gastrointestinal endoscopy). The endoscopy is preferred by the experts as they can see the lining of the stomach and duodenum and take biopsies which allow them to rule out more serious conditions as well as looking for the presence of Helicobacter pylori.

are actually caused by a bacterial infection. Please be aware that this is relatively new, and some misleading and simply out dated pamphlets that are still in circulation (Save-On Drugs /1992). This bacterial infection is called H. pylori - (Helicobacter pylori) that infects the stomach. This infection is common throughout the world, and Its been suggested that it could of even been acquired as far back as your childhood. Usually, people infected have no symptoms, and develop no problems as a result. However, about 10 to 15 percent of the people infected with H. pylori will eventually develop a peptic ulcer.

Background
Non-steroidal anti-inflammatory drugs are frequently prescribed medications which can cause gastric and duodenal ulcers. Prevention with misoprostol and/or proton pump inhibitors is effective but expensive; more cost-effective methods of prevention are needed. The role of H pylori in the pathogenesis of NSAID induced ulcers is not clear. This study was undertaken to study the effectiveness of H pylori eradication in preventing NSAID induced peptic ulcer disease.
Methods
Patients Patients over 18 years of age requiring NSAID therapy for musculoskeletal disorders were recruited from outpatient clinics. Exclusions: prior use of NSAIDS (including ASA) for over one month; prior anti-H pylori therapy; any use of NSAIDS, anti-ulcer therapy, steroids, anticoagulants or cytotoxic therapy in the previous 8 weeks; history of gastric ulcer disease, gastric surgery or renal impairment. Pre-randomization endoscopy both rapid urease and histology were positive.
