Home Educational Topics Pernicious Anemia Pernicious anemia is caused by an inability to absorb vitamin B12 (cobalamin) that is naturally found in certain foods. These foods are all of animal origin and include meat, milk and dairy products, and eggs. Vitamin B12 is not found in plants. Although bacteria in the large intestine produce vitamin B12, it is not absorbed into the blood stream from this site. Most people need only 2 micrograms of vitamin B12 daily but the average diet provides about 5 to 30 micrograms a day. When it is ingested, vitamin B12 needs to be chemically linked to a substance called intrinsic factor, which is produced in the stomach. In the animation on the right , vitamin B12 is shown in pink and the intrinsic factor is shown in blue. They combine in the stomach and pass into the small intestine where the intrinsic factor helps the vitamin B12 get absorbed into the circulation. Through the circulation, the vitamin is transported to the liver where it is stored, being released back into the circulation as needed. Without intrinsic factor, vitamin B12 is not absorbed and pernicious anemia occurs. Other causes of vitamin B12 deficiency produce the same symptoms as pernicious anemia. Diseases of the small intestine that cause malabsorption may cause vitamin B12 deficiency. Vegans (individuals who consume absolutely no foods of animal origin) are at risk of vitamin B12 deficiency due insufficiency of this vitamin in their diet. But it takes a long time to deplete all the vitamin B12 that is stored in the liver. Even if the absorption of vitamin B12 is suddenly cut off (for example, as in gastrectomy, surgical removal of the stomach), there is enough vitamin B12 stored in the liver to last for one to five years.

A person can get PA from a hereditary deficiency of intrinsic factor, surgical removal of portions of the stomach, chronic inflammation of the stomach lining (atrophic gastritis), and rarely from lack of B12 in the diet (mainly found in meat and dairy products). My daughter also has P.A. It was diagnosed when she was 48. I have been on B12 many years - I am now almost 80. I receive monthly B12 injections, and would like to know if I still need these every month? My doctors don't check on the need as with other medication A: If you were diagnosed with PA at such a young age, chances are you need the B12 replacement now, at the age of 80, more than ever. In a survey of 548 older people, ages 67-96, about 20% had low levels of B12 on blood tests. Only half of those people were actually anemic, so they and their doctors were probably unaware of the B12 deficiency. Adequate B12 is important for other reasons than just prevention of anemia. The nervous system also utilizes B12, so a lack of B12 can result in numbness and tingling of the hands and feet, memory loss, disorientation, depression, and even personality change. Older people may have lower levels of B12 because they do not adequately absorb B12 from their food. Once malabsorption starts, the person will become deficient in one to two years. The best way to replace B12 is through injections. Pills are available but would not be absorbed any better than B12 in food unless given in fairly high doses. The usual dose is 1 mg per month given by injection. Ask your doctor to measure your blood level of B12. Most likely you will continue to require a monthly shot

Thank you. Dear Rose, Both pernicious anemia and blood group A have been associated in epidemiological studies with a higher risk of developing gastric cancer. On the other hand, whether blood group A is associated with a predisposition to developing pernicious anemia is not known. Genetic factors do seem to play a role in the development of chronic autoimmune atrophic gastritis associated with pernicious anemia. This type of gastritis is most common in Scandinavia where it eventually affects 0.13 percent of the population. Approximately 5-12 percent of these patients will develop gastric cancer. Pernicious anemia results from a lack of intrinsic factor in gastric secretions which, in turn, causes inadequate Vitamin B12 absorption. Vitamin B12 deficiency results.
