The cause of OHS in unknown, but it is likely to involve a combination of a disorder of the brain's control over breathing and the effects of massive obesity on the chest wall. With the excess weight of massive obesity, the muscles of the chest wall can have difficulty expanding the thorax enough to exchange air efficiently. This results in a decreased ability to oxygenate the blood and retention of carbon dioxide (see respiratory acidosis ). Affected individuals suffer from chronic fatigue. This is from sleep loss, poor sleep quality, as well as chronic hypoxia (decreased blood oxygen).

"Pickwickian syndrome", also known as obesity hypoventilation syndrome is named after Joe , an obese, sleepy, red-faced minor character in Charles Dickens' book "The Posthumous Papers of the Pickwick Club." Massive obesity can cause reduction in chest wall movement, which in turn can lead to hypoventilation, hypoxia and hypercapnia (carbon dioxide retention.) The body attempts to compensate for the hypoxia by increasing the percentage of red blood cells (polycythemia), increasing the patient's risk of venous thrombosis and pulmonary embolism. Chronic hypoxia can also lead to constriction of the pulmonary arteries, leading to pulmonary hypertension, right-sided heart failure and peripheral edema. Because nighttime sleep is disturbed, patients frequently nod off uncontrollably during the day. Patients with Pickwickian syndrome are often severely compromised in their activities of daily living.

Description
About 2.5 million people in the U.S. suffer from sleep apnea (episodes of cessation of breathing during sleep) with men outnumbering women. The several conditions often associated with obstructive sleep apnea include obesity and a short thick neck, and reduction in muscle tone of the soft palate, the uvula (the small, conical, fleshy tissue hanging from the center of the soft palate), and the pharynx. The upper airway may be narrowed by enlarged tonsils or adenoids, a deviated nasal septum, nasal polyps, or congenital abnormalities. Even at high altitudes sleep disruption may occur because of low oxygen concentration. The Pickwickian syndrome consists of moderate to extreme obesity, alveolar (a hollow sac or cavity) hypoventilation, prolonged drowsiness, twitching, cyanosis (a dark bluish to purplish coloration of the skin), periodic breathing, secondary polycythemia (excessive red blood cells caused by low oxygen), and right sided heart failure. Details of the origin and development of this obesity hypoventilation syndrome are unknown, as is its true incidence. Studies show that the syndrome can exist in non-obese patients as well.

Who actually coined the term "Pickwickian" syndrome? Peretz Lavie Ph.D. In 1956 Burwell and colleagues (1) reported on an obese patient with heart failure and respiratory acidosis who was also extremely sleepy. Because this patient reminded them of the picturesque character Joe, the sleepy, red-faced, fat boy from Charles DickenÕs book "The Posthumous Papers of the Pickwick Club (1837), they called this syndrome the Pickwickian syndrome. The term was enthusiastically adapted by the medical community and numerous papers had been reported on Pickwickian patients by the mid-70s. In fact, following Burwell et al.'s publication, almost every sleepy patient was indiscriminately described in the medical literature as "a Pickwickian patient." This was the case even in cases where the underlying pathology was narcolepsy. Some years before, the same practice prevailed when every sleepy patient was termed "narcoleptic".
Although Burwell et al. have been widely credited for coining the term the "Pickwickian Syndrome", careful reviewing of the medical literature reveals that the term "pickwickian" was used to describe fat, sleepy patients already well before Burwell et al.Õs publication, not once, but several times. Moreover, whereas Burwell et al. did not ascribe their patientsÕ problem to a breathing disorder in sleep, the earlier descriptions of "pickwickian patients" leave no doubt to what was the underlying disorder of those sleepy patients. On February 8, 1889, Richard Caton of Liverpool, England, presented to the Clinical Society of London what he misnamed "a case of narcolepsy." The patient was a 37-year old poulterer who complained of intense drowsiness, appearing coincidentally with a weight increase. His condition became quite severe:
