is characterized by repetitive episodes of upper airway obstruction that occur during sleep, usually associated with a reduction in blood oxygen saturation. Prevalence:
Obstructive sleep apnea syndrome is most common in middle aged, overweight males. Although an accurate figure is not available, a prevalence of 1-2% of the population is suspected.

The North Karelia Project has carried out a coordinated, community-based intervention in order to reduce the prevalence of cardiovascular risk factors (hypercholesterolemia, smoking and hypertension). The evaluation has used standardized cross sectional population surveys at 5-year intervals. From 1972 to 1992, among men aged 30-59 years, the prevalence of smoking changed from 52 to 32 %, the mean total cholesterol from 7.1 to 5.8 mmol/l, the mean blood pressure from 147/92 to 143/84 mmHg and the age-adjusted mortality rate has reduced for ischemic heart disease by 48 % and for cancer by 42 %.

    Anesthesia and the Morbidly Obese As an Anesthesiologist, it is important to know how to care for the morbidly obese. Morbid obesity is defined as any one who weighs more than twice their ideal body weight. The morbidly obese constitute 3-5% of the U.S. population. More and more people are seeking surgical intervention such as Gastric Bypass after all other methods have failed such as dieting, exercise, behavioral modification techniques and drugs. The morbidly obese patient presents a challenge to the Anesthesiologist. They usually present with significant coexisting medical problems such as hypertension (high blood pressure), diabetes, coronary artery disease (heart disease), congestive heart failure, sleep apnea, Pickwickian syndrome (hypoventilation syndrome), Cholelithiasis (gallstones), fatty liver and difficult intubation (difficulty placing breathing tubes in for surgery). The morbidly obese patient who presents to the Anesthesiologist for Gastric Bypass surgery should have an extensive preoperative evaluation and preparation. The preoperative evaluation should include a detailed history, physical and appropriate diagnostic studies. Special attention should be paid to the cardiovascular (heart), respiratory (lungs), hepatic (liver), and endocrine (pancreas, thyroid) systems. The cardiovascular (heart)system evaluation should look for hypertension (high blood pressure), signs of heart failure (shortness of breath, swelling in the lower extremities). And electrocardiogram (EKG) and chest x-ray should be performed. Any abnormality of these tests should lead to more detailed testing such as a stress test, echocardiogram, and cardiac catheterization to exclude coronary artery disease and pulmonary hypertension.

Individuals with sleep apnea may go many seconds without breathing. They may typically wake up, gasp for air, and then return to sleep, sometimes repeating the process hundreds or even thousands of times during an evening. The most prominent result of this is chronic daytime fatigue and drowsiness. People with sleep apnea may lack the energy to function normally and are much more likely to fall asleep while driving.     Pickwickian syndrome This syndrome was named after a character in one of Charles Dickens' works (Dickens was a 19th century English writer who wrote Great Expectations and other works). One character of his, Mr. Pickwick, had a peculiar type of labored breathing which 20th century clinicians recognized as being associated with sleep apnea. The three clinical components of Pickwickian syndrome are day time sleepiness, lack of oxygen and excess carbon dioxide in the blood. This disorder, if left unchecked, can eventually lead to heart failure.

MEDLARS is the name of the system used to produce and search MEDLINE and some 50 other NLM databases. Some 3,700 international biomedical journals are indexed by MEDLINE. 31,000 articles are added each month. Currently, MEDLINE contains over 8 million citations. MEDLINE begins with 1966.
