Community's Sleep Lab can help
Snoring is not a laughing matter. You or your spouse may be suffering from a sleep-related breathing disorder. For more than 16 years, Community Hospital's Sleep Lab has been available for the testing and treatment of three major sleep disorders - sleep apnea, Pickwickian Syndrome and Upper Airway Resistance Syndrome. These conditions are potentially life-threatening and can cause extreme daytime fatigue. Sleep-related breathing conditions can steal hours of productive sleep away every night. These conditions are also known to increase the risk of car accidents and emotional distress. What is Obstructive sleep Apnea?
  • a serious, potentially life-threatening condition
  • affects up to 4% of all males and 2% of all females, especially those women post-menopause
  • results in excessive daytime sleepiness, morning headaches, irratability and depression
  • can lead to accidents while driving or at the workplace What is Pickwickian Syndrome?
  • a serious, potentially life threatening condition present in many severly obese individuals
  • characterized by heavy snoring
  • often involves obstructive sleep apnea

Depending on its severity and its cause, sleep apnea is associated with a variety of heart and breathing problems, including heart arrhythmias, high blood pressure, hardening of the arteries, even heart failure and death. It is a peculiarity of this noisy disease that it announces itself to everyone within earshot except its victims.
The medical definition of sleep apnea is a temporary stoppage of breathing during sleep, occurring at least 30 times during the night. The stoppage must be longer than the usual interval between breaths while asleep. Ten seconds is normal for adults.

Causes, incidence, and risk factors Return to top The cause of OHS in unknown, but it is likely to involve a combination of a disorder of the brain's control over breathing and the effects of massive obesity on the chest wall. With the excess weight of massive obesity, the muscles of the chest wall can have difficulty expanding the thorax enough to exchange air efficiently. This results in a decreased ability to oxygenate the blood and retention of carbon dioxide (see respiratory acidosis ). Affected individuals suffer from chronic fatigue. This is from sleep loss, poor sleep quality, as well as chronic hypoxia (decreased blood oxygen).

    History
In 1837 Charles Dickens undoubtedly described the first sleep apnea patient in "The Posthumous Papers of the Pickwick Club" . One of his characters (Joe) was described as follows, "... and on the box sat a fat and red-faced boy, in a state of somnolency." There are several other references in Dickens' book to this character suggesting he had severe sleep apnea:
  • "Joe - damn that boy, he's gone to sleep again."
  • In response to the firing of enormous guns at a military exercise, "everybody was excited except the fat boy, and he slept soundly as if the roaring of cannon were his ordinary lullaby . . . Be good enough to pinch him, sir - in the leg, if you please; nothing else wakes him."
  • Joe "snores as he waits at the table . . . the snoring of the fat boy penetrated in a low and monotonous sound from the distant kitchen"

    Obesity Overview
    What is Obesity? ... Glossary
There are several serious and sometimes life-threatening health problems associated with morbid obesity co-morbidities
In light of the increased health risks, the NIH states that having a BMI greater than 35 along with significant co-morbidities qualifies as morbid obesity.
    Co-Morbidities of Morbid Obesity
These associated health problems can be life-threatening:

    Pet Obesity
Most references report that 20-30% of dogs and cats are overweight with one report as high as 40%. If your animal falls into this category and would therefore benefit from a weight reduction program, the information that follows should help you achieve your objectives. Obesity cannot occur unless the amount for food (calories) exceeds the animal’s nutritional requirements. Simple put, if a pet is overweight, it is eating too much. Any overweight animal should be evaluated to make sure there are no metabolic diseases that are contributing to the problem. Thyroid abnormalities are the most common disease that causes obesity and until it is corrected, dietary management will probably not resolve the problem.
    Hazards of Obesity
    • Overweight animals have shorter life spans than normal weight animals Hastens development of joint disease and lameness Causes heart disease (Pickwickian Syndrome*) Predisposes to Diabetes Increases risk of Heat Stroke Increases anesthesia risk
A longer, more active life should be motivation enough for a caring pet owner to treat an overweight pet.

Charles Dickens was probably the first to describe the features of sleep apnea in his famous comic narrative The Posthumous Papers of the Pickwick Club (4). The fat boy Joe was snoring heavily, he was falling constantly asleep and was described as having slow perception. Damn that boy...! In 1889, William Hill (5) recognized the symptoms of the syndrome, describing the stupid lazy-looking kid who frequently suffers from headaches at school, breathes through his mouth instead of his nose, snores and is restless at night. He speculated that some of their backwardness was secondary to some hampering of the cerebral functions rather than deafness. Hill also found adenotonsillectomy to be helpful for these children. In 1965 Menashe et al.
