Its prevalence has increased so significantly in recent years that many consider it a major health concern of the developed world. In the United States alone, 20-27% of all children and adolescents have obesity. Many factors, including genetics, environment, metabolism, lifestyle, and eating habits, are believed to play a role in the development of obesity. Obesity-hypoventilation syndrome is a relatively uncommon finding in children with obesity, with an estimated frequency of 1-3%. Stated from a different point of view, 10% of patients with obstructive sleep apnea are obese. No firm diagnostic criteria to define obesity-hypoventilation syndrome exist; this fact, along with limited pediatric studies and discrepant definitions of obesity and abnormal pediatric polysomnographic findings, make the literature somewhat difficult. However, obesity, sleep disordered breathing, and hypercarbia during wakefulness are features generally described with obesity-hypoventilation syndrome. Other features include excessive daytime sleepiness, hyperactivity, poor school performance with difficulty attending to tasks and impaired memory, hypoxia, and signs of cor pulmonale.

2 May 2000 Volume 132 Number 9 LETTERS Risk for Obstructive Sleep Apnea Letters on Pages Letter 1 to the Editor: The fine study by Netzer and colleagues and commentary by Millman demonstrate the underrecognition of sleep disorders. Obstructive sleep apnea, however, is not a new disorder. While the association of the Pickwickian syndrome is derived from the description of Charles Dickens's character Joe the fat boy in The Pickwick Papers , sleep apnea may date much further back than this depiction in 1836-1837. Pliny the Younger, an ancient Roman, described the eruption of Vesuvius in 79 AD and related the death of his uncle in a letter well known to classicists . While Vesuvius roared, his uncle "went to rest certainly slept, for as he was a stout man his breathing was rather loud and heavy and could be heard by people coming and going outside his door." He didn't awaken despite the clamor, and others had to wake him. Two slaves helped him to get up, but he collapsed. Pliny attributed this to the dense fumes that "choked his breathing by blocking his windpipe which was constitutionally weak and narrow and often inflamed"; his uncle was later found dead . Pliny's uncle may well have had contributory chronic bronchitis. Pliny's description notes the risk factors for snoring, sleepiness, and obesity. It is not muddied by contributing alcohol use (as seen, for example, in Shakespeare's Falstaff and the giant in the tale "Jack and the Beanstalk," who snored when drunk) and would meet many of the criteria in the Berlin Questionnaire.

10.0: Headache associated with metabolic disorder There are a variety of types of headache that may be associated with different derangements of metabolism. This subject is only covered briefly in this chapter and the interested reader is referred to other more extensive headache texts such as Dalessio (1993) and Raskin (1988). 10.1: Hypoxia Headache Associated with Hypoxia Dalessio (1993) points out that experimentally induced cerebral hypoxemia, (Wolff and Lennox, 1930), especially when coupled with an increase in carbon dioxide tension in the blood, results in extreme dilatation of cerebral vessels, notably of the arteries and arterioles. This observation is probably related to the fact that some persons at high altitudes (Bancroft et al., 1922; Monge, 1942) complain of headache that persists for hours or days until physiologic adjustments have been achieved, or until the individual returns to a lower altitude. Such headaches are usually prevented by ingestion of acetazolamide prior to ascent to high altitude. Associated with the intense throbbing headache is a sensation of fullness of the head, hot flushes of the face, photophobia, injection of the ocular mucosa, and deep cyanosis. It is likely that such headaches are due to cerebrovascular distention. There are a variety of specific situations in which headache may occur from hypoxia alone or from hypoxia and hypercapnia together, as described below.
