http://www.uuhsc.utah.edu/healthinfo/pediatric/diabetes/ppdhub.htm
Posterior Pituitary Disorders The posterior (back) lobe of the pituitary gland releases ADH (antidiuretic hormone) and oxytocin (a hormone to contract the uterus during childbirth and stimulate milk production). Antidiuretic hormone (ADH), which is also called vasopressin, helps the kidneys (and body) conserve the right amount of water. For example, when a person becomes dehydrated, more ADH is produced to help the body conserve the water it contains. Lack of ADH leads to too much excretion of water (diabetes insipidus), and too much ADH leads to excessive retention of water by the body (syndrome of inappropriate antidiuretic hormone secretion). Listed in the directory below you will find additional information regarding these posterior pituitary disorders, for which we have provided a brief overview. If you cannot find the information in which you are interested, please visit the page in this Web site for an Internet/World Wide Web address that may contain additional information on that topic.

Cambridge Area Pituitary Patients' Support (CAPPS) Group The first meeting of CAPPS, a new branch of the Pituitary Foundation, was held in May 2000 in the Eleanor Cripps Diabetes and Endocrine Centre in Clinic 12 at Addenbrooke's Hospital. The Cambridge group is the 33rd local support group set up around the country to raise patients' awareness to the many types of pituitary disorders. Carol Holmes, Endocrine Specialist Sister, said: "Sufferers who feel alone and anxious with their illness are welcome to attend the first of many Pituitary Group meetings. Join us for a cup of tea and chat to others who are probably feeling the same as you." "There are 170 people registered as sufferers from pituitary disorders in this region. Once the Group has been set up, help and advice will be given on free prescriptions, life insurance, pensions, job issues, travel and even possible driving restrictions will be discussed at future meetings." Gail Weingartner, the National Co-ordinator and Addenbrooke's Endocrine Consultant, attended the meeting. Facts and figures:
  • More than 40 000 people in the UK are thought to suffer from pituitary disorders, many of which are little known outside specialist clinics.

Prevalence of Pituitary Abnormalities May Affect 1 in 4 People Pituitary disorders left undiagnosed can reduce life-span E-Mail Story Print Story SPECIAL EMBARGO FOR RELEASE: 10 a.m. (ET) Thursday, January 23, 2003 Media Advisory: Dr. Ezzat spoke today at the American Medical Association media briefing on hormone disorders. To better understand the frequency of pituitary tumors, Dr. Ezzat and a number of colleagues performed a meta-analysis of 13 different studies (eight autopsy studies and five medical imaging studies) that were designed specifically to examine and describe the nature of abnormalities within the pituitary gland. Across eight autopsy studies, the overall prevalence of pituitary abnormalities was 14.4 percent, and across the five imaging studies, the prevalence of pituitary abnormalities was 22.5 percent (one of the more recent autopsy studies used in the meta-analysis revealed that, consistent with the MRI findings, pituitary abnormalities were found in roughly one out of four subjects). The researchers found an overall prevalence of 16.7 percent when the numbers of patients from all studies were compared. Pituitary malfunctions result from infections, inflammation (especially around the time of pregnancy), injuries, cancerous spread and most commonly, non-cancerous pituitary tumors, called adenomas, according to Dr. Ezzat. Some pituitary tumors may be hormonally inactive and may be discovered incidentally during the course of MRI performed for unrelated reasons, such as head injury. While some pituitary tumors may be very small (microademomas), they have the potential to cause health complications through excessive hormone production.

Pituitary Dysfunction May Be Cause of Some Unexplained Illnesses Expert diagnosis and treatment of pituitary disorders can be life-changing E-Mail Story Print Story SPECIAL EMBARGO FOR RELEASE: 10 a.m. (ET) Thursday, January 23, 2003 Media Advisory: "Pituitary hormonal disorders and pituitary tumors can cause an array of relatively common health problems that often go undiagnosed or misdiagnosed for years. My patient and co-presenter, Sharmyn McGraw, was seen by multiple doctors for seven years pursuing treatment for symptoms associated with an undiagnosed pituitary tumor," says Dr. Kelly, associate professor of neurosurgery at University of California Los Angeles (UCLA) School of Medicine and director of the UCLA Pituitary Tumor and Neuroendocrine Program. Ms. McGraw illustrates several of the numerous debilitating changes her body went through during her seven-year struggle with an undiagnosed pituitary tumor. "Before my diagnosis with Cushing's disease life was horrid. I went from a petite size two to an obese size 22 in less than a year. I gained as much as 11 pounds in four days despite the fact I worked out with a trainer and had a healthy diet. My hair fell out by the handful, and I suffered unbearable depression and anxiety. Some doctors advised even more diet and exercise while others advised prescription drugs and a physiological evaluation that made me feel like a hypochondriac." For individuals with pituitary hormonal problems or a pituitary tumor, Dr. Kelly recommends seeking initial evaluation from an endocrinologist with specific expertise in pituitary disease or at a specialized pituitary clinic where endocrinologists and neurosurgeons see patients together during the same office visit: "Patients are best managed by a team approach that ensures all the bases are covered in terms of the hormonal and neurological issues, to determine the best treatment for a given patient, be it pharmacological therapy, surgery or radiation therapy and whether hormone replacement therapy is needed. Long-term follow-up is also needed given that pituitary tumors can recur over time and pituitary failure may develop after treatment."

Pituitary Disorders
The pituitary gland has been called the body’s "master" gland. It manages long-term growth, day-to-day functions, and reproductive processes. The gland produces eight hormones. It hangs from the base of the brain. When the gland does not work right, it may produce either too much or too little of one or more of the hormones. This causes changes elsewhere in the body. DISORDERS OF THE PITUITARY GLAND RESULT FROM:
  • CONGENITAL AND GENETIC PROBLEMS: These usually affect the supply of the hormones. For example, a low supply of growth hormone slows the growth of a child. Dwarfism or other problems can result. INJURY: A head injury may stop the gland from working. IMPAIRED BLOOD SUPPLY: When the blood supply is cut off by a growing tumor or aneurysm, the gland may not be able to function. RADIATION: Decreased hormone production from the pituitary gland may result from radiation therapy used to treat a tumor. TUMORS: Pituitary tumors are usually benign or noncancerous. One type of tumor reduces the production of specific sex hormones. This can lead to a failure of sexual function. Monthly periods may stop in women and the amount of sperm may be reduced in men. Another type of tumor causes an excess of hormones. Rapid growth before adulthood may occur. The hands, feet, jaw, and skull may overgrow in size. Infertility and impotence may result. Diabetes may develop as well. If the gland enlarges, it can press on surrounding tissue and cause other symptoms, such as vision problems.

Project 1: Identifying the genetic basis for X-linked pituitary hypoplasia (XPH)
Incomplete development of the pituitary gland during embryogenesis results in congenital pituitary hypoplasia. We are studying a severe X-linked form of this disorder, X-linked pituitary hypoplasia (XPH), which only affects males. We have shown using cytogenetic and linkage analysis of XPH families that this disorder results from increased dosage of a gene located at Xq26-q27. Using a mouse model system, we are investigating genes at Xq26-q27 for expression within the developing pituitary, and are testing the function of XH candidate genes in transgenic mice. Generating a mouse model for XPH will lead to a greater understanding of the developmental origin of this disorder and provide a significant insight into function of the causative gene
