Dermatomyositis and Polymyositis What is Myositis?
Myositis is a connective tissue and autoimmune disease. The body's own immune system makes proteins called "antibodies" for protection against viruses, bacteria and other foreign bodies (antigens). In myositis the immune system is unable to tell the difference between antigens and its own cells and tissues. The immune system makes "auto-antibodies" directed against its own cells and tissues. The build up of these "auto-antibodies" in the tissues causes inflammation, pain and muscle damage. The cause of myositis is still unknown. TYPES OF MYOSITIS

Polmyositis and Dermatomyositis are related illnesses affecting muscle and connective tissues of the body. Joints may also occasionally be involved. A rarer form of Polymyositis is known as Localised Nodular Myositis. A more serious and rare form of myositis is Inclusion Body Myositis.   • Polymyositis is the inflammation (i.e., irritation, pain, swelling, and damage) of the muscles closest or proximal to the trunk of the body (i.e., limb girdles, neck, and pharynx). It is a progressive condition, with periods of relapse (gets worse) and remissions (gets better).

Polymyositis is an inflammatory muscle disease causing weakness and pain . Dermatomyositis is identical to polymyositis with the addition of a characteristic skin rash. Description
Polymyositis (PM) is an inflammatory disorder in which muscle tissue becomes inflamed and deteriorates, causing weakness and pain. It is one of several types of inflammatory muscle disease, or myopathy. Others include dermatomyositis (DM) and inclusion body myositis. All three types are progressive conditions, usually beginning in adulthood. A fourth type, juvenile dermatomyositis, occurs in children. Although PM and DM can occur at any age, 60% of cases appear between the ages of 30 and 60. Females are affected twice as often as males.
Causes
The cause of PM and DM is not known, but it is suspected that a variety of factors may play a role in the development of these diseases. PM and DM may be autoimmune diseases, caused by the immune system's attack on the body's own tissue. The reason for this attack is unknown, although some researchers believe that a combination of immune system susceptibility and an environmental trigger may explain at least some cases. Known environmental agents associated with PM and DM include infectious agents such as Toxoplasma Borrella Lyme disease bacterium), and coxsackievirus. Most cases, however, have no obvious triggers (direct causative agents). There may also be a genetic component in the development of PM and DM.

What are polymyositis and dermatomyositis? Polymyositis and dermatomyositis are disorders involving inflammation of the voluntary muscles. As a result of this inflammation (myositis), muscles undergo degenerative changes, which may lead to severe disability. In polymyositis, the inflammation is found in many muscles, thus the term "poly". It is present over a prolonged period of time. In dermatomyositis the muscle inflammation is usually accompanied by a skin rash, therefore the prefix "dermato". What are the symptoms of each of these disorders? The symptoms of polymyositis and dermatomyositis vary widely from person to person, with respect to severity, rate of progression and complications. Spontaneous remissions may occur, although as a rule, the natural course of the disease is progressive unless effective treatment can be found. It has been suggested that the more acute the onset, the better the prognosis. In dermatomyositis, a reddish rash appears, typically on the face (cheeks and eyelids), elbows, hands, knees and upper chest. The skin changes in appearance, becoming shiny, tight and thin. A person's fingers may become very blue in cold weather. In polymyositis, there is no skin rash.

Polymyositis is a condition which affects many muscles ( poly = many), mainly the larger muscles of the body, such as those around the shoulders, hips and thighs. This booklet also deals with dermatomyositis . This is a disease where a particular skin rash occurs ( derm = skin) with muscle inflammation that is similar to polymyositis. This booklet discusses:

They are a group of conditions in which there is inflammation of the muscles. Resulting disability is very variable: sometimes weakness is mild but at the other extreme there may be major disability. Generally speaking, polymyositis (PM) and dermatomyositis (DM) respond to treatment. DM occurring in childhood may have important differences and a separate MDC leaflet is available entitled 'Juvenile Dermatomyositis'. Inclusion Body Myositis (IBM) is another important cause of chronic muscle inflammation, mainly in older male subjects and is also dealt with in another MDC leaflet. With very rare exceptions PM and DM are not inherited. Both PM and DM can affect individuals at any age. What are the symptoms?

Polymyositis (PM) is a rare condition, characterised by inflammation of muscle. In the majority of cases a faulty immune system is thought to cause the disorder which results in the body attacking its own muscle cells. There are several treatment options available and most individuals with PM respond well to these. Research has centered on establishing well-designed clinical trials to evaluate current treatment protocols and provide scientific evidence that the benefits outweigh the side effects. Background information: Polymyositis (PM) falls within a group of muscle disorders called inflammatory myopathies. The term PM comes from two words, myositis which means inflammation of muscle and poly meaning many i.e. it is characterised by the inflammation of many muscle tissues. Inflammatory myopathies are thought, in the majority of cases, to be autoimmune disorders caused when the body's own immune system becomes faulty and begins to attack its muscle cells, but the exact cause has not been established. When this occurs the body sees its own muscle cells as foreign and produces highly specific molecules called antibodies to target muscle cells and destroy them. This is the same mechanism that operates in healthy individuals when they have an infection, the bacteria or virus is recognised as foreign and the body produces specific antibodies to target the bacteria or viruses and destroy it. It is a rare condition with an annual incidence rate in the region of 1 per 100 000 and appears to show a female predominance of 2:1, especially amongst African-American woman.

Polymyositis Polymyositis is a chronic disease characterized by inflammation and by degenerative changes to muscles and skin. This causes weakness and muscle wasting. The disease is seen more frequently in women than in men, and occurs most often between ages 30 and 50. The cause is unknown. Polymyositis can have a sudden or slow onset. A person with polymyositis will typically complain of the following symptoms:
  • weakness in the hips and shoulders swelling and redness in the joints of the hands and ankles cold hands or feet chronic shortness of breath, even at rest problems swallowing and fever, muscle weakness, weight loss, and joint pain may be the first to occur before the other symptoms
Medical evaluation may include the following:
  • history and physical exam blood tests biopsy of the involved area to check for inflammation and electromyography, or EMG, which is a test that looks at muscle strength and function

Polymyositis (PM) is found mostly in people over the age of 20. More women than men have PM. PM is thought to be an autoimmune disease, meaning the body's immune system, which normally fights infections and viruses, does not stop fighting once the infection or virus is gone. The immune system then attacks the body's own normal, healthy tissue through inflammation (IN fluh MAY shun), or swelling. PM does not cause a rash. Muscle weakness usually happens over days, weeks or months. The weakness begins with muscles closest to and within the trunk of the body. Neck, hip, back and shoulder muscles are examples. Some patients also have weakness in muscles farther from the trunk, like hands and fingers. Many PM patients have trouble swallowing, or dysphagia (dis FAY jee yuh). Some patients have trouble breathing and muscle pain. What tests will the doctor run to see if I have polymyositis?

    What is Polymyositis?
    Polymyositis is an inflammatory muscle disease that causes varying degrees of decreased muscle power. The disease has a gradual onset and generally begins in the second decade of life. Polymyositis rarely affects persons under the age of 18. The most common symptom is muscle weakness, usually affecting those muscles that are closest to the trunk of the body (proximal). Eventually, patients have difficulty rising from a sitting position, climbing stairs, lifting objects, or reaching overhead. In some cases, distal muscles (those not close to the trunk of the body) may also be affected later in the course of the disease. Trouble with swallowing (dysphagia) may occur. Very rarely, the muscles ache and are tender to touch. The disease may be associated with other collagen vascular, autoimmune or infectious disorders
