Causes and Risks The cause of this disorder is unknown. It is thought that an autoimmune reaction or a viral infection of the skeletal muscle may cause the disease. It can affect people at any age, but most commonly occurs in those between 50 to 70 years old, or in children between 5 to 15 years old. It affects women twice as often as men. Muscle weakness may appear suddenly or occur slowly over weeks or months. There may be difficulty with raising the arms over the head, rising from a sitting position, or climbing stairs. The voice may be affected through weakness of the throat muscles. Joint pain inflammation of the heart , and pulmonary (lung) disease may occur. A similar condition, called dermatomyositis , is evident when a dusky, red rash appears over the face, neck, shoulders, upper chest, and back. A malignancy may be associated with this disorder. The disorder is of unknown cause. When cutaneous disease is also present the term dermatomyositis is used. Polymyositis and dernatomyositis are considered to be collagen vascular diseases because of their clinical, pathological, and radiological features. Diagnostic criteria include: proximal symmetric muscle weakness that progresses over a period of weeks to months; elevated serum levels of muscle enzymes or an elevated level of urinary creatinine excretion; an abnormal electromyogram; and

Polymyositis
Definition
Polymyositis is an inflammatory muscle disease causing weakness and pain . Dermatomyositis is identical to polymyositis with the addition of a characteristic skin rash.
Description
Polymyositis (PM) is an inflammatory disorder in which muscle tissue becomes inflamed and deteriorates, causing weakness and pain. It is one of several types of inflammatory muscle disease, or myopathy. Others include dermatomyositis (DM) and inclusion body myositis. All three types are progressive conditions, usually beginning in adulthood. A fourth type, juvenile dermatomyositis, occurs in children. Although PM and DM can occur at any age, 60% of cases appear between the ages of 30 and 60. Females are affected twice as often as males.
Causes The cause of PM and DM is not known, but it is suspected that a variety of factors may play a role in the development of these diseases. PM and DM may be autoimmune diseases, caused by the immune system's attack on the body's own tissue. The reason for this attack is unknown, although some researchers believe that a combination of immune system susceptibility and an environmental trigger may explain at least some cases. Known environmental agents associated with PM and DM include infectious agents such as Toxoplasma Borrella Lyme disease bacterium), and coxsackievirus. Most cases, however, have no obvious triggers (direct causative agents). There may also be a genetic component in the development of PM and DM.

POLYMYOSITIS
Ronald P. Messner DEFINITION. Polymyositis is an inflammatory disease of skeletal muscle of unknown etiology, characterized by symmetric weakness of the limb girdles, neck, and pharynx. Patients with this illness can be divided into five clinical categories (Table 443-1). In this chapter the term "polymyositis" is used to characterize the whole group of patients, while "adult polymyositis" is used to denote type I patients. INCIDENCE. Polymyositis has an annual incidence of approximately five cases per million population. There is no relationship to birth order, family size, socioeconomic level, or geographic location. Familial cases are unusual. Age distribution is bimodal, with a small peak between ages 10 and 14 and a larger peak around age 50. Patients with myositis associated with malignancy are slightly older, with a mean age just over 60. In adult polymyositis and dermatomyositis, females outnumber males by two to one. In cases associated with overlap syndromes the female dominance is even more pronounced. The sex ratio is equal in myositis associated with malignancy. PATHOLOGY AND PATHOGENESIS.

    Dermatomyositis and Polymyositis
Dermatomyositis and polymyositis are autoimmune disorders in which the skeletal muscle is damaged by an inflammatory process dominated by lymphocytic infiltration. The term polymyositis is applied when the condition spares the skin, and the term dermatomyositis when polymyositis is associated with a characteristic skin rash. One-third of cases are associated with various connective tissue disorders, such as rheumatoid arthritis, lupus, polymiositis, polymyosites The precise cause of these diseases is unknown, but interplay between host genetic factors, viral infection of muscle, and autoimmune mechanisms is probably contributory. Familial occurrence of these diseases, and the increased frequency of HLA-DR3 and -DRw52 antigens in patients, suggest A lymphocyte-mediated disease resembling polymyositis has been reported in laboratory animals injected with muscle antigens together with Freund's adjuvant (experimental) A widely used classification of the dermatomyositis-polymyositis group. Other diseases uncommonly associated with polymyositis are sarcoidosis, giant cell myositis with thymoma, and myositis in systemic infections due to viruses, toxoplasma, or parasites. A focal
