  • Polymyositis (poh-lee-mi-o-si-tiss) is one of a group of inflammatory (in-flah-muh-tor-e) muscle diseases. Polymyositis is an inflammation (redness, pain and swelling) of many muscles in your body. It is also an autoimmune (aw-toe-i-mune) disease. An autoimmune disease is when your body gets confused and thinks that something normal inside of you is bad. Your immune system responds by attacking it. The immune system is what protects your body from dangerous things like infection or disease. An autoimmune disease is like being allergic to yourself.
    With polymyositis, your immune system thinks muscle tissues are bad and attacks them. This causes muscle weakness and loss. Polymyositis usually occurs between 40 and 60 years of age. It happens in women twice as often as men. You may have mild polymyositis for a while before you even know it.
Causes: The exact cause of polymyositis is unknown. Scientists believe that polymyositis may happen for several reasons. Infections of the muscles caused by a virus may start the bad autoimmune response. Or, our environment or surroundings might have toxic or bad things that effect our immune system.

Polymyositis is an inflammatory muscle disease causing weakness and pain . Dermatomyositis is identical to polymyositis with the addition of a characteristic skin rash. Description Polymyositis (PM) is an inflammatory disorder in which muscle tissue becomes inflamed and deteriorates, causing weakness and pain. It is one of several types of inflammatory muscle disease, or myopathy. Others include dermatomyositis (DM) and inclusion body myositis. All three types are progressive conditions, usually beginning in adulthood. A fourth type, juvenile dermatomyositis, occurs in children. Although PM and DM can occur at any age, 60% of cases appear between the ages of 30 and 60. Females are affected twice as often as males. Causes and symptoms Causes The cause of PM and DM is not known, but it is suspected that a variety of factors may play a role in the development of these diseases. PM and DM may be autoimmune diseases, caused by the immune system's attack on the body's own tissue. The reason for this attack is unknown, although some researchers believe that a combination of immune system susceptibility and an environmental trigger may explain at least some cases. Known environmental agents associated with PM and DM include infectious agents such as Toxoplasma Borrella Lyme disease bacterium), and coxsackievirus. Most cases, however, have no obvious triggers (direct causative agents). There may also be a genetic component in the development of PM and DM.

Polymyositis is part of a group of muscle diseases that involve an inflammation and break down of skeletal muscle tissue. The onset of muscle weakness is slow and gradual. Limbs closest to the trunk of the body and neck muscles are weakened; involvement of muscle groups distal to the body varies. Difficulty swallowing is common, but inability to breathe due to muscle failure is not as common. Muscle pain is often reported, but is rarely a chief complaint. High doses of steroids and immunosuppressant drugs are used for treatment.
Childhood cases of polymyositis are rare. More women are affected by polymyositis than men are.

Can this be the result of my body getting accustomed to life without prednisone, or is it more apt to signify another muscle condition at work?
Answer: What is your sedimentation rate? CPK? Certainly can be adrenal insufficiency depending on the length of time you have been on the steroids. Try going to alternate day therapy-a high dose one day and a much lower dosage the next. This would answer your question as to whether more steroids would fix the problem. [posted 08/5/1999]
Question: My father has just come to a long road after months of tests to be diagnosed with Polymyositis. Could you please tell me what the treatment is and any important information he should know. Answer: Polymyositis is an inflammatory disease of the muscles. The cause is not known. There is a variation called dermato-polymyositis that also affects the skin. Treatment of this disease centers on steroids with other immune suppressants used if this is insufficient. There is an association between cancers and the onset of polymyositis that is, one should consider that there may be a concurrent tumor-especially of the colon. However, most patients do not develop cancers and most do not have concurrent tumors. While he may have a long course of treatment, most patients do not die with polymyositis, although it is possible in severe cases. Like many of the autoimmune disorders, there is a wide variation in severity of cases from mild to severe.

Polymyositis and dermatomyositis are related diseases, both involving inflammation swelling or irritation of the voluntary muscles, those that normally govern movement, such as the muscles of the arms and legs. Myositis means inflammation of muscle. Polymyositis means inflammation of many muscles, and dermatomyositis means inflammation of muscle and skin. The results of either condition are weakened muscles and often muscle pain. These conditions are seldom fatal, but in the most severe cases a person may need a wheelchair or require assistance with activities of daily living. As early as 1863, a disease involving muscle inflammation and a reddish rash on the face and upper body was described and given the name polymyositis. In 1887, an apparent case of polymyositis without any rash was described by the German physician P. Hepp. Another German, H. Unverricht, then proposed using the name dermatomyositis for cases of muscle inflammation with skin changes and polymyositis for those without them. This terminology remains in use today. Both conditions are among the 40 neuromuscular diseases in the Muscular Dystrophy Association's program.
